Pigmentary epitheliopathy, disc edema, and lead intoxication.

A 52-year-old lead-exposed worker presented with disc edema, nerve fiber layer infarcts, and an acute bilateral pigment epitheliopathy that phenotypically resembled APMPPE. The patient had definite laboratory evidence of increased total body lead burden. The simultaneous involvement of retina, disc, and choriocapillaris suggests that what has been reported as the spectrum of APMPPE may represent a nonspecific vascular response to multiple injurious stimuli, including lead.

Trends in neurovascular complications of surgical management for carotid body and cervical paragangliomas: a fifty-year experience with 153 tumors.

Almost 75% of carotid body and cervical paragangliomas are adherent to or surround adjacent arteries and cranial nerves. Their resection can result in neurovascular injury, stroke, and excessive blood loss. To assess trends in neurovascular complications, we reviewed 153 carotid body and cervical paragangliomas that were surgically managed between 1935 and 1985. Results of the past 10 years were compared with two previous time periods: period I (1935 to 1965), when carotid artery reconstruction was uncommon at our institution, and period II (1966 to 1975), when methods of intraoperative electr

Cephalic tetanus: a case report and review of the literature.

Cephalic tetanus is a rare form of tetanus defined as trismus plus paralysis of one or more cranial nerves. The most frequently involved cranial nerve is the seventh. It accounts for 1 to 3% of the total number of reported cases of tetanus and has a mortality of 15 to 30%. The incubation period is 1 to 14 days, and approximately two thirds of cases progress to generalized tetanus. The mechanism of the paralysis is not completely understood. Treatment involves debridement of wounds, administration of penicillin and tetanus immune-globulin, aggressive supportive care, and initiation of active im

Role of vagal hyperactivity in gastric stress ulceration after acute injury to the cervical cord.

The role of vagal hyperactivity in the pathogenesis of gastric stress ulceration that occurs after acute trauma to the cervical cord is controversial. We have used a rat model of transection of the cervical cord to induce gastric stress ulceration and to examine the cause of the ulcers. Cervical cord transection did not increase gastric acid output or plasma levels of gastrin or pancreatic polypeptide, either immediately or up to eight hours later. However, gastric stress ulceration showed a time-related increase in ulceration. Vagal stimulation with 2-deoxy-glucose enhanced the gastric acid o

Observations on the recovery of sensation following inferior alveolar nerve injuries.

The recovery of sensation to the lower lip and chin following unilateral inferior alveolar or mental nerve injuries has been examined in 21 adult patients for periods of up to 45 months. Sensory testing was performed using light touch, pin prick and thermal stimuli as well as testing for sharp/blunt differentiation, localisation and two-point discrimination. The area of anaesthesia to light touch stimuli had disappeared by 4 months after nerve compression and by 3.5-8 months after nerve section, but persisted after nerve section if nerve regeneration was impeded. In this latter group there was

Teflon injection for vocal cord paralysis after intracranial operation.

Vocal cord paralysis may follow intracranial operation if the vagus nerve is damaged intraoperatively. If the resulting laryngeal incompetence is severe, hoarseness, dysphagia, and aspiration may develop. This is often followed by a pneumonitis requiring chronic gastric tube feeding. Teflon vocal cord injection has been shown to restore the sphincteric action of the larynx and enable patients to phonate and swallow properly again. Nine patients underwent this procedure for chronic aspiration and hoarseness. All had vagus injury attributable to antecedent intracranial operation, and all experie

Routine intraoperative facial nerve monitoring during otologic surgery.

We have used intraoperative monitoring and stimulation of facial nerve function routinely in 301 consecutive otologic and neurotologic cases. The device has been safe, simple to use, and practical. Facial contraction is detected by a strain-gauge sensor in the corner of the mouth and is signalled audibly to the surgeon. Electrical stimulation of the facial nerve can be delivered through a sterile probe, which produces a constant-current-square-wave impulse. The device has several advantages: it signals unintentional mechanical stimulation of the facial nerve during surgery; it allows mapping o

[Ocular findings in tick borreliosis].

Tick-transmitted borreliosis is by no means a rare infectious disease. It is found especially in southern parts of Germany. Eyes are seldom affected. The disease can lead to nerve palsy of the eye muscles. One case of panophthalmitis and one of optic disk edema have been described. The authors found a bilateral palsy of the abducens nerve in one patient with tick-transmitted borreliosis. Motility normalized under therapy with highly-dosed penicillin.

Histopathology of the facial nerve after longitudinal temporal bone fracture.

The temporal bone histopathology in a patient who suffered repeated head trauma and a longitudinal temporal bone fracture shows unusual features associated with the facial nerve. In the distal meatal segment, a type of traumatic neuroma was found with disorganized nerve bundles and distinct areas of Schwann's cell proliferation but lacking fibrosis. Periosteal new bone formation in the labyrinthine segment narrows the fallopian canal and protrudes into the nerve, which completely fills the canal. The tympanic and mastoid segments of the nerve show severe degeneration of nerve fibers and an inc

[Trans-synaptic retrograde atrophy of the optic nerves: a differential diagnostic problem of expansive intracranial processes].

Trans-synaptic retrograde atrophy (trans-synaptic degeneration) is a secondary degenerative reaction that can appear in various areas of the brain. Degeneration of the optic radiation, the optic tract and the optic nerve due to a cerebral lesion in the occipital region has been described since the last century as a result of faetal or early childhood lesions of the brain (porencephaly, hydrocephalus) and as various cortical lesions and injuries of the occipital pole. In addition to homonomus defects in the visual fields descending partial atrophy of the optic nerves appeared after a few years

Damage of the peripheral auditory system after operations in the cerebellopontine angle. A scanning electron-microscopic observation in dogs.

Cochlear nerve injuries caused by manipulations in the cerebellopontine angle were electrophysiologically and morphologically investigated in dogs. Operative procedures similar to those performed in the cerebellopontine angle of humans were applied in dogs. Auditory-evoked potentials were recorded throughout the experiments. Postoperatively, the temporal bones were studied with a scanning electron microscope. The portions of the cochlear nerve fibers and branches of the internal auditory artery that exit from the bony cochlea into the internal auditory canal were susceptible to traction force
