Orbital emphysema: a potentially blinding complication following orbital fractures.

A case of visual loss due to orbital emphysema secondary to a blow-out fracture of the orbit is presented. Because vision returned to 20/20 following an optic nerve decompression procedure, we hypothesize that our patient developed a compressive optic neuropathy with ischemia due to the emphysema. Essential instructions concerning the injury that the emergency physician should give the patient suffering an orbital blow-out are also presented.

Cranial nerve injuries after carotid artery endarterectomy.

We present a seven-year experience with cranial nerve injuries due to carotid artery endarterectomy. A total of 433 carotid endarterectomies were done on 355 patients by senior surgical residents, with a staff surgeon assisting. Thirteen cranial nerve injuries were identified, five of which were permanent. Knowledge of the anatomic features of the cranial nerves and their branches in the operative field, as well as technical maneuvers during surgery, can reduce such injuries to a minimum.

Temporal bone fractures: a clinical diagnosis.

Temporal bone fractures may be complicated by intracranial haemorrhage, C.S.F. leakage and infection, damage to the middle and inner ear and damage to the seventh and eighth cranial nerves. Accurate early diagnosis is important to enable adequate investigation and prompt treatment of any complications. We present eight cases seen in a 12 month period in which a temporal bone fracture was not diagnosed at presentation in spite of a full clinical examination and standard skull radiographs. Five of these cases developed complications which resulted in their referral. The absence of a visible frac

Use of bacitracin for neurotologic surgery.

Wound infection, cerebrospinal fluid leak, and meningitis are serious potential complications of neurotologic procedures that transgress the posterior cranial fossa dura. A study of 236 patients was made to determine the effect of perioperative intravenous antibiotics and topical bacitracin irrigation on the incidence of these complications. Of the 236 patients, 170 (72%) underwent translabyrinthine resection of acoustic tumors, while 66 (28%) underwent retrolabyrinthine vestibular nerve section. Patients were divided into four groups: those who received no antibiotics, those who received peri

Surgery of tumors invading the cavernous sinus.

Potential injury to neurovascular structures within the cavernous sinus often prohibits total removal of various cranial base tumors. This report discusses the rationale of direct cavernous sinus surgery and describes a surgical technique refinement as evolved from experience in 18 histologically benign operative cases. The nature of tumor extension mandated entry to the sinus, either through the superior or lateral wall or both. The operative microscope facilitated dissecting and preserving the carotid artery and cranial nerves transversing the sinus. Venous bleeding was controlled by packing

Malignant glioma of the optic chiasm eight years after radiotherapy for prolactinoma.

A 41-year-old man had rapidly progressive visual loss caused by a malignant glioma that developed in the optic chiasm eight years after radiation therapy for a recurrent prolactinoma. Radiation-induced glioma should be considered as a cause of progressive visual loss in patients who have received irradiation in the region of the sella turcica.

[Pathomorphology of the nodose ganglia of the vagus nerves in burns].

Pathomorphology of ganglion nodosum was studied by means of neurohistological methods in 65 dead patients during different periods of burn disease and demonstrated a definite dynamic of structural modifications. In period of burn shock there were discovered dystrophic changes and necrosis of neuron separate groups, varicose thickness of a myelin sheath, in period of toxemia hypertrophied neurons with thick axons predominated, in period of septic toxemia and burn exhaustion predominated atrophic changes, necrotic neurons and grain-blockage disintegration of nervous fibres.

Facial nerve paralysis complicating bilateral fractures of the mandible: a case report and review of the literature.

A case report of a unilateral lower motor neurone facial nerve paralysis complicating bilateral fractures of the mandible is described. The injury was the result of a fall. The differential diagnosis is discussed together with the treatment undertaken. The importance of a thorough examination of the cranial nerves following trauma is emphasised. A review of the literature is presented with special reference to the relative merits and indications for the use of cortico-steroids and surgical exploration.

Radiographical documentation of direct injury of the intracanalicular segment of the optic nerve in the orbital apex syndrome.

In the radiographical evaluation of the orbital apex syndrome, standard radiographs, tomograms, and computed tomographic scans have proved useful in the demonstration of the bony pathology, especially for optic canal fractures. The limitation of these methods, however, remains in their inability to provide accurate delineation of the associated soft tissue pathology, including the presence of optic nerve sheath hematoma. Recent developments in computer technology and graphic imaging are now available to provide an accurate three-dimensional radiographical analysis of the extent of skeletal and

Complicated strabismus and adjustable sutures.

Clinical experience in performing over 500 adjustable strabismus operations is mentioned in this paper particularly as it relates to complicated strabismus. A maximum hang-loose recession of a rectus muscle has a limited effect, which is tabulated and the implications discussed. Aids in finding the lost medial rectus muscle are mentioned. Adjustable sutures are then exploited in managing the found "lost" medial rectus. Adjustable recession of both vertical recti in the affected eye were used in some cases of blow-out fracture to manage limitations of upward and downward gaze. Adjustable recess

Nasolacrimal drainage system obstruction after orbital decompression.

We reviewed 123 cases of orbital decompression in 63 patients with dysthyroid ophthalmopathy. Of 90 cases of transantral ethmoidal orbital decompression, 14 (16%) resulted in epiphora. The tearing began between 11 and 18 months after surgery. All patients had obstruction distal to the common internal punctum. The delayed onset suggested progressive cicatricial obstruction of the nasolacrimal drainage system. Damage to adjacent tissues probably caused scarring to extend into the system.

Local neurological complication during carotid endarterectomy.

In a prospective study of 52 carotid endarterectomies there were 13 temporary cranial nerve injuries in 12 patients (25%); 7 hypoglossal nerve injuries (13.5%), 3 were asymptomatic (5.8%), 2 had mild speech difficulty (3.8%) and 2 had severe difficulty with deglutition and articulation (3.8%). A full recovery was made within 3 months in all 7 patients. Although 10 patients (19.2%) developed a hoarse voice, indirect laryngoscopy only indicated damage to the ipsilateral recurrent laryngeal nerve in 3 patients (5.8%). Both the voice and vocal cord movement returned to normal within 6 months in 2

Facial paralysis: traumatic neuromas vs. facial nerve neoplasms.

Traumatic neuromas (TN) are benign proliferations of neural tissue that may occur without disruption of the facial nerve. The clinical presentation, as well as the radiographic appearance, may suggest neoplastic involvement of the facial nerve. Histologically, they may closely resemble neurilemomas (Schwannomas) or neurofibromas. Three cases of TN of the facial nerve associated with facial paralysis are presented here. Unlike previously reported cases, these tumors were not associated with chronic inflammatory middle ear disease. TN must be considered in the differential diagnosis and treatmen

Why do vestibular destructive procedures sometimes fail?

Vestibular nerve specimens and one temporal bone, from patients with vestibular symptoms after destructive surgery on the vestibular system, were studied by light microscopy. Surviving nerve axons in three specimens that followed retrolabyrinthine vestibular nerve section (RLVNS) were counted and compared to normative data. Results are consistent with persistence of the central processes of primary vestibular neurons in three specimens from patients who had persistent symptoms and ice-water caloric responses after RLVNS. Incomplete neurectomy probably results from anatomic variations in the pl

Bilateral trigeminal neuralgia: a 14-year experience with microvascular decompression.

Thirty-five patients with trigeminal neuralgia (TN) bilaterally underwent posterior fossa microvascular decompression (MVD) between 1971 and 1984. They comprised 5.0% of a larger series of 699 patients with TN who underwent MVD during that interval. Compared to the subgroup of 664 patients with only unilateral symptoms, the population with bilateral TN included a greater percentage of females (74% vs. 58%, p less than 0.1), a higher rate of "familial" TN (17% vs. 4.1%, p less than 0.001), and an increased incidence of additional cranial nerve dysfunction (17% vs. 6.6%, p less than 0.05) and hy

Complications of translabyrinthine vs. suboccipital approach for acoustic tumor surgery.

This study presents the operative results of surgeons with experience in both the translabyrinthine and suboccipital approaches to acoustic tumor removal for the purpose of resolving some of the controversy among centers favoring one approach over the other. The subjects were 171 patients with acoustic tumors who were treated at Virginia Mason Clinic from 1975 to 1986. The translabyrinthine approach was used in 64% of cases, the suboccipital approach in 35%, and the middle fossa approach in 1%. There were minor differences in morbidity and mortality between approaches. These data did not favor

Correlation between high-resolution computed tomography and surgical findings in congenital aural atresia.

Congenital aural atresia poses a great challenge, even to a competent otologic surgeon, due to innumerable types of malformations affecting the conduction and perception of sound. Roentgenographic evaluation by plain roentgenography is inconclusive in most cases; polytomography is helping to some extent. Recent generations of high-resolution computed tomographic (CT) scanners are probably the best. Seven cases of unilateral and three cases of bilateral congenital aural atresia were evaluated with high-resolution CT. Subsequently, ten ears were operated on. Surgical findings were correlated wit
