Major congenital ear malformations: surgical management and results.

Fifteen patients with major congenital aural atresia underwent operations using an anterior (trans-atretic bone) approach. Facial nerve monitoring was used in all cases and there were no instances of facial nerve injury. Postoperatively, two thirds of the patients had speech reception thresholds of 30 dB or better; the air conduction threshold (averaged for 500, 1,000, and 2,000 Hz) improved at least 25 dB in 80% of the patients and at least 30 dB in 60% of the patients (follow-up, 10 to 29 months). Minor revision surgery was necessary in three patients. On the basis of this series, several co

Optic nerve decompression.

Post-traumatic deterioration of vision requires thorough ophthalmologic evaluation, as well as computed tomographic scanning of the orbits and central visual pathways if no obvious ocular origins of the visual decrement are detected. When optic nerve trauma is a suspected etiology, the patient should be treated with megadose intravenous steroids, as well as optic nerve decompression performed after 12 to 24 hours if improvement of vision fails to occur.

Collet-Sicard syndrome after minor head trauma.

Occipital condyle fractures are rare and are usually associated with severe head and cervical spine injury. A 71-year-old man developed unilateral palsies of the 9th through 12th cranial nerves (Collet-Sicard syndrome) due to a fracture of the occipital condyle, which was diagnosed by computed tomography. He was treated conservatively and made a good recovery.

[Postoperative iatrogenic lesions of the external branch of the accessory spinal nerve].

Iatrogenic lesions of the spinal nerve are rare and generally occur during cervicotomy for aetiological assessment of adenopathy. The spinal nerve is essentially damaged at two sites: the supraclavicular fossa (where the nerve is very superficial) and in the antero-lateral triangle of the neck (where the nerve approaches the sterno-cleido-mastoid muscle). The lesion of the spinal nerve essentially presents in the form of paralysis of the trapezius; which should be examined systematically postoperatively and at subsequent consultations, as the clinical signs may only develop after a certain lat

Disappearing optociliary shunt vessels and neonatal hydrocephalus.

This is a report of two cases of optociliary shunt vessels in patients with neonatal hydrocephalus. Complete disappearance of the optociliary shunt vessels occurred after surgical procedures to normalize intracranial pressure. The clinicopathological significance, prognosis, and treatment of patients with acquired optociliary shunt vessels is discussed. This is also the first report of the association of optociliary shunt vessels and neonatal hydrocephalus.

Intraoperative monitoring of the facial nerve.

Anatomic preservation of the facial nerve, with maximal facial function, is one of the goals of acoustic neuroma surgery. Application of electrophysiologic monitoring techniques is useful in achieving this goal. Preoperative electromyography and nerve conduction studies provide important prognostic information for preservation of the nerve and postoperative function. Intraoperative electromyography alerts the surgeon to facial nerve proximity and potential injury. Direct nerve stimulation is utilized to confirm the location and integrity of the nerve. Matched-pair analysis of two groups of pat

Trochlear nerve palsy following minor head trauma. A sign of structural disorder.

Trauma-induced superior oblique palsy usually results from contusion or avulsion of the trochlear nerve or from decompensation of a congenital trochlear nerve palsy. Severe craniocerebral trauma is often associated with the former mechanism, whereas more minor closed-head injuries can decompensate a congenital phoria. We report a patient who developed an isolated trochlear nerve palsy following minor head trauma. Investigation revealed an unsuspected tentorial vascular malformation that was compressing the trochlear nerve in its subarachnoid course. In the absence of other features (e.g., docu

Bilateral hypoglossal nerve palsy due to vertical subluxation of the odontoid process in rheumatoid arthritis.

Two patients with rheumatoid arthritis involving the cervical spine developed a rapidly progressive dysarthria due to bilateral hypoglossal nerve palsies, in one patient as an isolated lesion, and in the other in conjunction with dysfunction of the vagus and glossopharyngeal nerves. Both patients showed upward movement of the axis, relative to the foramen magnum, which has been termed 'vertical subluxation of the odontoid' or 'cranial settling'. Mechanical injury to the nerves, secondary to this process, is the most likely mechanism.

[Treatment of atypical post-traumatic and postoperative facial neuralgias by chronic stimulation. Apropos of 2 cases, with review of the literature].

This article describes the case of two patients suffering from deafferentation pain after surgery or traumatic lesions in the area of the peripheric trigeminal branches on the Gasserian ganglion. Chronic electrical stimulation by an electrode implanted in the Gasserian ganglion led to a good permanent result in both cases, within a follow-up period of one year and one year and a half, respectively. The pre-operative test was carried out percutaneously via the foramen ovale, the definitive implantation by surgery with subtemporal access. This method can only be used when at least part of the ga

Diagnosis and management of traumatic optic neuropathies.

If a patient has vision immediately following trauma, with subsequent deterioration of visual acuity and/or field, and the presence of a relative afferent pupillary defect, compression of the optic nerve or its vascular supply is very likely. We currently lack a proven optimal treatment, but in the otherwise healthy patient, we suggest an intravenous (IV) loading dose of methylprednisolone 30 mg/kg, and a second 15-mg/kg dose 2 hours after the initial dose, followed by 15 mg/kg every 6 hours. Optic nerve decompression is indicated in this situation when corticosteroids have only a temporary ef

[Deglutition disorders caused by an injury to the foramen lacerum posterior].

Dysphagia due to a traumatic lesion of the foramen lacerum posterior is relatively rare. Two cases are described in which swallowing troubles were perceived when the post-traumatic coma improved; these were ascribed to a traumatic lesion of the foramen lacerum posterior injuring the cranial nerves. Treatment involves preventing inhalation and nasogastric feeding whilst starting active and early rehabilitation of swallowing. Recovery is possible in most cases without surgery.

Preservation of cochlear potentials in a deaf patient fifteen months after excision of an acoustic neuroma.

Intraoperative electrocochleographic monitoring has been used in an attempt to protect cochlear nerve function during acoustic neuroma excision. One patient is presented with an apparently intact cochlear nerve and waves N1 and N2 preserved at the end of surgery, but no hearing on subsequent testing. Fifteen months after operation, cochlear microphonics and the summating potential were still present, but N1 and N2 had disappeared. There had been no improvement in her hearing and there were no consistent brainstem auditory evoked potentials on the affected side. A possible explanation for these

Cranial and cervical nerve injuries: local complications of carotid artery surgery.

Injury to the nervous structures extending through the operative field or in areas adjacent to it constitutes a local complication in surgery of the carotid artery. These lesions may become, after an otherwise well-tolerated reconstruction, the source of severe and, occasionally even permanent, complaints. In a series of 536 operations of the carotid artery, 14.4% of transient and 6% of permanent injuries to some of the distal cranial and cervical nerves occurred. The highest injury rate was observed in the hypoglossal nerve (8.6%), followed by the marginal mandibular branch of the facial nerv

Facial nerve and parotid surgery.

A series of 100 surgical procedures on the parotid gland affected by tumours is reported, with a detailed analysis of facial nerve function after parotid surgery. A low percentage of temporary lesions (28%) affecting only one branch of the facial nerve, all of which regressed in a short time, and only one case of a permanent lesion affecting the whole nerve, anatomically intact, however, was observed. The authors confirm the safety of parotidectomy, if correctly performed.

Vocal cord paralysis associated with cystic fibrosis.

The course of the left recurrent laryngeal nerve through the chest brings it in close proximity with the heart and great cardiac vessels. Diseases of the heart and the great vessels are known to cause vocal cord paralysis, probably by mechanical injury to the recurrent laryngeal nerve. Pulmonary artery hypertension and dilation occur in up to 80% of patients with cystic fibrosis. We report a case of a 23-year-old woman with cystic fibrosis and left vocal cord paralysis. We believe that sudden pulmonary artery expansion produced recurrent laryngeal nerve injury and vocal cord paralysis. This is
