Pathophysiological mechanisms responsible for intraoperative and postoperative hearing deficits associated with cerebellopontine (CP) angle operations were explored experimentally in dogs. The CP angle operative manipulations performed were the same as those experienced by human patients, and auditory evoked potentials were monitored intraoperatively. As a result of the operative manipulations, petechial or confluent hemorrhages occurred at the compressed portions of the cochlear nerve, and intravascular clots were often observed. Disintegration of the nerve fibers was verified by ultrastructu

A report is given on transient and permanent (6 months) impairment of cranial nerves after paediatric head injuries (N: 741). There is a link between severity of the injury, fractures on the base of the skull, its foramina and channels, and the frequency of cranial nerve involvement. One should try to establish whether a posttraumatic dysfunction of the cranial nerves is primary or secondary in nature, i.e. due to raised intracranial pressure or haemorrhage. In children after head injuries often the cranial nerves of the oculomotor system are affected (20.2%/7.0%)--transient (permanent), follo

The anomalous position of a nonrecurrent laryngeal nerve predisposes the nerve to injury during thyroidectomy and to compression by a thyroid mass. We present three cases in which a seemingly benign thyroid mass traumatized a nonrecurrent laryngeal nerve resulting in either vocal cord paralysis or a vague pressure sensation over the larynx. Some of these patients feel as if they need to clear a foreign body and present with a chronic cough. Normally the nerve is protected from thyroid masses as it passes through the tracheoesophageal groove. In all three patients, surgical excision of the thyr

Carotid body tumors are a rare but potentially difficult surgical entity. Their pathology, physiology, and natural history are reviewed along with surgical results reported in the literature. A surgical approach for removal of these tumors is presented which differs significantly from the recommended techniques in that emphasis is placed on intraoperative monitoring of cerebral blood flow, the the selective use of shunts, a tumor-adventitial plane of dissection, preservation of the carotid artery complex, and mobilization of the parotid gland. Thirteen cases using these techniques are reviewed

Bilateral facial paralysis is a rare disorder, and its work-up should include a complete neurotologic assessment plus neurologic consultation and lumbar puncture. The various causes and their treatment are discussed and the treatment of longstanding facial paralysis is reviewed.

A patient with bilateral traumatic hypoglossal palsy is presented. Evaluation of the atlanto-occipital articulation is difficult. Prompt radiographic diagnosis of this injury can be made by computed tomography. In this case, there was bony avulsion of the alar ligament from the occipital condyle. We suggest a severe blow to the head in a lateral direction.

Eight patients with acoustic neuroma and five patients with hemifacial spasm, who had undergone hypoglossal-facial nerve anastomosis 1-14 years previously, were reviewed clinically and electrophysiologically with an electrically elicited blink reflex. Functional recovery from the anastomosis, as rigorously judged on a scale of good, fair and poor, was fair to poor. Electrically it was found that the blink reflex was present in eight patients, suggesting facial nerve re-innervation of the facial musculature. This was confirmed by the mild recurrence of hemifacial spasm in four patients. Implica

The nervus intermedius (NI) appears to be the main conduit for the associated symptoms of cluster headache (CH) and perhaps for the pain as well. Subtle injury of the facial nerve and NI might initiate mechanisms responsible for CH. Five patients with chronic CH unresponsive to medication underwent surgical decompression of the root exit-entry zone of the facial nerve, and in two patients the trigeminal nerve root was also decompressed. In two patients, the pain syndrome was markedly relieved for as long as two years. In one patient, initial improvement was obscured by narcotic addiction. In t

Immediate repair of carotid artery lacerations are imperative not only to prevent exsanguination but also for restoration of impaired cerebral blood flow. The preoperative neurological situation is responsible for the expected cerebral damage. We are reporting the case of a five year old boy, who survived without cerebral damage despite deep preoperative unconsciousness.

The risk of complications occurring during arthroscopic examination of the temporomandibular joint is not well documented. We therefore performed arthroscopic examinations of the upper compartment of the temporomandibular joint of fresh cadavers and during subsequent dissection studied possible damage to the facial nerve, superficial temporal vessels, disk, and articular surfaces. Damage to the facial nerve was not seen in any joint. The distance between the puncture site and any branch of this nerve was 3 mm or more in all but one of the specimens. The puncture site was repeatedly located imm

Contemporary otomicrosurgical techniques have made total removal of acoustic tumor with preservation of the seventh and sometimes the eighth cranial nerves possible. The four approaches currently used in acoustic tumor surgery are the middle cranial fossa, the translabyrinthine, the suboccipital, and the combined translabyrinthine-suboccipital. This review examines the surgical results in the removal of more than 600 acoustic tumors and outlines a rationale for the choice of approach. Tumor size on computed tomographic scan and auditory reserve establish the parameters used in planning the sur

The final outcome of 33 facial palsies (4 bilateral, 25 unilateral) after temporal bone fracture is reviewed. 11 immediate and 22 delayed paralysis were found. Out of the 22 delayed palsies 20 did heal spontaneously. The remaining two patients underwent tympanotomy because of CSF otorrhea. Simultaneously decompression of the facial nerve was performed. 17 of 22 delayed palsies showed complete and 5 incomplete recovery with slight functional defects only. 7 of 11 facial palsies with immediate onsets after injury were treated surgically (6 cases by transmastoid approach, 1 case by transmastoid a

In seven eyes of six patients (four men and two women, 55 to 72 years old), with macular edema associated with severe carotid artery stenosis, the ipsilateral carotid obstruction was 90% or more. Concomitant fundus signs, such as midperipheral retinal hemorrhages, retinal arterial narrowing, and neovascularization of the optic disk, were observed. All eyes demonstrated leakage of fluorescein dye from the retinal vessels and optic disk.

During the last decades the treatment of the ageing face has evolved from a mere action at the skin level, towards an additional correction of the ptosis of the underlying soft tissues, and procedures involving the bony-cartilaginous framework. Any new rhytidectomy technique with deep undermining should be examined with regard to high risk areas. The areas of risk relate to the branches of the facial nerve and to sensory innervation. These are described on the basis of the current literature, clinical observations and cadaver dissections. The expected improvement in results by the use of techn

An injury of the inferior alveolar nerve is not an uncommon occurrence. We envisage the physiopathologic process of this involvement. One case of injury of the nerve by endodontic overfilling is presented and analyzed. Prednisone therapy appears to shorten the course of the disease, prevent the secondary fibrosis, and lessen the severity of sequelae.

Optimal rehabilitation of stroke disability depends on a detailed analysis of neurologic impairments, associated medical conditions, psychosocial status, activities of daily living, and environmental barriers. Such analysis will permit design of the most appropriate management program, aimed at minimizing disability, maximizing function, and returning the stroke patient to a gratifying existence in spite of residual impairment and disability.

Blindness can result from pressure on the optic nerve in the optic canal, either from trauma or from neoplasm. The usefulness, timing, and technique of surgical decompression of the optic canal remains controversial. The present paper describes eight patients with optic nerve compression, who had the optic nerve decompressed through a transorbital approach. Because this technique is technically simple and is associated with a low morbidity, it offers a treatment alternative to this difficult group of patients.
