Ventricular fibrillation.

Ventricular fibrillation is the most common mechanism of sudden unexpected cardiac death in persons with asymptomatic or symptomatic coronary artery disease. The electrophysiologic mechanisms reviewed in this article include: automaticity of pacemaker fibers, transformation of nonpacemaker into pacemaker fibers, "injury" currents and reentry. Some of the conditions facilitating ventricular fibrillation include bradycardia, long QT syndrome, electrocution, electrolyte imbalance, drugs, sympathetic stimulation and myocardial ischemia. Electrophysiologic studies during acute myocardial ischemia s

Current concepts in retrobulbar anesthesia.

Recently there have been major advances in the field of retrobulbar anesthesia. New agents which allow prolonged anesthesia and akinesia have been introduced. Several new techniques to administer retrobulbar anesthesia have been developed. The toxicity of local anesthetics and the complications arising from such injections have been studied, and ways to avoid and manage them have been expanded.

The sagittal split osteotomy of the mandible.

Modifications of the sagittal split osteotomy of the mandible have essentially reduced the major drawbacks of the procedure, such as condyle displacement, short-term skeletal relapse, and protracted maxillomandibular fixation and mental nerve dysesthesia. These techniques have proved effective over a period of 4 years in fifty-seven patients treated.

Facial reanimation after skull base trauma.

Three basic types of procedures are used by the author to reanimate the face paralyzed by interruption of the facial nerve at the skull base. In order of choice they are direct nerve repair or grafting; hypoglossal-facial nerve anastomosis; and regional facial reanimation (brow lift, implantation of a gold weight or spring in the eyelid, tightening of the lower eyelid, and temporalis muscle transposition to reanimate the mouth). The indications for and expected results of each approach are presented with special emphasis on the usefulness of regional reanimation.

Reversible visual loss following optic nerve injury.

A case of shrapnel injury to the optic nerve documented by computed tomography, resulting in total, but partially reversible visual loss, is presented. In view of the visual field defect and computed tomographic findings, a transient vascular insult is incriminated. The irreversible loss in the lower field of vision is related to the shearing effect of trauma on the vulnerable pial vessels supplying the upper half of the nerve.

[Vestibular neurectomy in Ménière's disease. Results and comments apropos of 77 cases].

The authors report 77 cases of vestibular neurectomy performed between 1972 and 1984 (40 male and 37 female) for incapacitating Meniere's vertigo. Neurectomy was via a sub-petrous (S.P.) approach in 55 cases, by posterior trans-labyrinthine (P.T.L.) approach in 9 and anterior trans-labyrinthine (A.T.L.) in 13 cases. A.T.L. was abandoned because of the frequency of leaks of CSF and of meningitis. The P.T.L. approach did not result in any facial paralysis, even transient. There were 2 CSF leaks, without meningitis and one of which required reoperation. For the 55 S.P. cases, hearing worsened in

Trismus in postoperative, posttraumatic and other brain stem lesions caused by paradoxical activity of masticatory muscles.

In fifteen cases of brain stem lesions caused by basal tumours, operations, injuries, arteriosclerosis or multiple sclerosis, a paradoxical activity of jaw closing muscles during jaw opening was observed. This inverse activity was the cause of severe trismus in 8 cases. It is the manifestation of a faulty programming of jaw movements in the trigeminal motor area in brain stem. This distinct new brain stem syndrome can only be detected by EMG investigation. EMG findings, pathological background, outcome and treatment possibilities are discussed in this paper.

Clinical study of the pattern electroretinogram in patients with optic nerve damage.

We have recorded both pattern and flash electroretinograms (ERG) in two patients with unilateral optic nerve damage due to optic canal fracture over one year. The pattern ERG from the affected eye was reduced in amplitude by approximately 40% in one patient, 60% in the other, compared with that from their other, normal eyes, while the flash ERG was normal in all eyes. Reduced pattern ERG responses may depend on retrograde degeneration following damage to the optic nerve. The pattern ERG, however, was not totally lost in the eyes with marked optic atrophy. There is a possibility that the patter

Transethmoid decompression of the optic nerve.

Delayed visual deterioration following trauma to the head can result from injury to the optic nerve or central visual pathways. Surgical decompression of the medial optic canal is indicated when central nervous system causes are ruled out and vision fails to improve on megadose intravenous steroids.

The neuro-otologist's view of the surgical management of acoustic neuromas.

The experience of our group in the management of 1667 patients with the diagnosis of an acoustic neuroma has been reviewed. Certain features of the operation have been discussed. It is our feeling at this time that with the techniques available to the neuro-otologist for the management of patients harboring acoustic neuromas, the goals of surgery should be total removal of the tumor with preservation of the facial nerve and avoidance of injury to the adjacent important brain structures. Our patients, at this time, should reasonably expect this type of result from their surgery. Hearing preserv

Glucose oxidase sticks and cerebrospinal fluid rhinorrhea.

The recognition of cerebrospinal leakage from a fistula is an important consideration for any physician caring for a head-injured population. Several procedures including radiographic, intrathecal dye, nuclear medicine tracer studies, computerized tomography with metrizamide injection, and immunological fixation have been reported to help in the diagnosis. Introduction of glucose oxidase test sticks has been traditionally touted to be a reliable bedside indicator of CSF rhinorrhea; this case study demonstrates a falsely negative result from glucose oxidase sticks.

Trauma to the nervous system and its sequelae in a one-year birth cohort followed up to the age of 14 years.

The one-year birth cohort of 12 058 children, 96% of all children born in the two northernmost provinces of Finland, Oulu and Lapland, in 1966 was studied prospectively up to the age of 14 years. During this period 14 children, 1.2 per thousand, were lost from the follow-up. Data on development, mortality, and morbidity were collected prospectively by means of questionnaires and from various registers, the most important of which was the National Hospital Discharge Register. A total of 299 cases of trauma to the nervous system (skull fracture, cerebral contusion, concussion, fracture of the ve

[Meningopolyneuritis (Garin-Bujadoux, Bannwarth) erythema chronicum migrans disease of the nervous system transmitted by ticks].

The clinical spectrum of this remarkable non-syphilitic spirochaetosis (spirochaeta infection) of the nervous system is described in light of 53 further cases with reference to our earlier description of 47 cases. As the etiological diagnosis is not possible in all cases the knowledge of clinical courses is especially important. Males are affected almost twice as often as women. The disease is especially prevalent in the sixth decade. Usually, the syndrome begins in the warm season with exquisite pain and other sensory irritations in combination with marked malaise. Initial signs during the wi

Delayed sequelae of pituitary irradiation.

Since 1958, 781 patients at Lawrence Berkeley Laboratory have received helium-particle stereotactic radiosurgery to the adenohypophysis. Autopsy findings in 15 of these patients are reported. Ten patients received pituitary radiation (average dose, 116 Gy in six fractions) for progressive neovascularization retinopathy due to diabetes mellitus. Evidence of a time-dependent course of progressive fibrosis in their pituitary glands was found. Five patients were treated for eosinophilic adenomas. Although they had lower average doses of radiation (56 Gy in six fractions), their pituitary glands sh

Spontaneous ("erosive") cerebrospinal rhinorrhea, with pneumococcal meningitis from anterior optic glioma. Case report.

A 6-year-old girl, operated upon for an optic nerve astrocytoma (resection limited to the intraorbital portion of the nerve), developed 3 years later symptoms and signs of chiasmatic invasion. Nine years after surgery (1980), she suffered from pneumococcal meningitis, due to cerebrospinal rhinorrhea that had been present in the last few years. A repeated neuroradiological investigation showed the site of the fistula and cerebral ventricles of normal size. The case is argued for bulb-to-chiasm resection of an optic nerve glioma: a spontaneous cerebrospinal rhinorrhea with its potential dangers

Aberrant regeneration involving the oculomotor and abducens nerves.

An unusual case of severe head trauma is reported with postoperative 'total' ophthalmoplegia of the right eye. Subsequent recovery led to an apparent transposition of right medial and right lateral rectus function. Optokinetic stimulation induced convergence nystagmus when the target was moved to the patient's left, and a divergence nystagmus when the target was moved to the patient's right. In addition, attempted lateral gaze induced ipsilateral pupillary constriction. Aberrant regeneration involving the right abducens and right oculomotor nerves is proposed as the underlying mechanism.
