Endolaryngeal devices for perioperative identification and functional testing of the recurrent nerve.

It has previously been shown that identification of the recurrent laryngeal nerves reduces the risk of injury to the nerves during thyroid surgery. To facilitate identification and to test the conductivity of the recurrent nerves we have used electrical stimulation of the nerves and simultaneous recording of the vocal cord movements with two different endolaryngeal devices. The experience gained with these devices indicates that it is possible to reduce the frequency of injury to the nerves, and the method accelerates the progress of the operation, especially in cancer surgery and repeat opera

A safe technique for thyroidectomy with complete nerve dissection and parathyroid preservation.

A series of 1,000 consecutive thyroid operations is presented, without a case of permanent recurrent laryngeal nerve injury. Emphasis is placed on the identification of variations and complete dissection of the recurrent laryngeal nerve, including peripheral branches and technical aspects of the dissection. Sixty-five percent of the cases had multiple terminal branches of the recurrent laryngeal nerve and five cases on the right side had a nonrecurrent course. A low incidence of hypoparathyroidism is presented, due in part to the avoidance of ligating the inferior thyroid artery in continuity

Experimental studies for correction of superior laryngeal paralysis by fusion of the thyroid to cricoid cartilages.

Contraction of the cricothyroideus muscles (CTMs), innervated by the superior laryngeal nerves (SLNs), modulates the voice by tilting the thyroid cartilage anteriorly onto the top of the cricoid and tensing the vocal cords. Either unilateral or bilateral paralysis of the SLNs is disabling for individuals with above-average voice demands. Some patients never compensate for this paralysis; there is no surgical procedure recognized to correct it. This study tested the hypothesis that surgical fusion of the thyroid and cricoid cartilages anteriorly can correct the problems of SLN injury by duplica

Topographic anatomy of the external branch of the superior laryngeal nerve. Its importance in head and neck surgery.

The authors have studied the anatomy of the external branch of the superior laryngeal nerve in its entirety on 40 fresh cadavers, and they have drawn the following conclusions: the nerve ramifies from the vagus immediately below the nodose ganglion or in the ganglion itself. The nerve splits into two branches approximately 1.5 cm below the ganglion nodosum. In four cases, both branches originated from the vagus itself. In one case, anastomosis of the external branch of the superior laryngeal nerve with the recurrent nerve was found. The external branch of the superior laryngeal nerve is not us

Injection injuries of the recurrent laryngeal nerve.

We describe a new etiology for vocal cord paralysis. Two patients, chronic drug abusers, had in time lost access to the usual peripheral veins, and proceeded then to have the drugs injected repeatedly into the jugular veins in the neck. Both presented with persistent hoarseness and were found to have a unilateral vocal cord paralysis. Neck exploration was undertaken to evaluate this pathology, to rule out other obscure disease, and to decompress the recurrent laryngeal nerve within the carotid sheaths. Severe fibrosis was encountered in the neck of one patient and mild fibrosis in the other. A

Accessory nerve injury.

A series of 13 patients with an injury of the accessory nerve in the posterior cervical triangle is reported. In 11 patients the nerve was damaged during a lymph node biopsy and in two cases there was a sharp glass injury. Paralysis of the trapezius muscle occurred with resulting deformity and loss of function of the shoulder. Nine patients were operated on. In five cases neurolysis, in two cases neurorrhaphy and in two cases reconstruction with a sural nerve graft were performed 3-17 months after the injury. The result was good or fair in six operative cases. Five of these were neurolyses. In

Monitoring auditory nerve potentials during operations in the cerebellopontine angle.

Direct monitoring of auditory nerve potentials was performed in 19 patients undergoing retromastoid craniectomy and microvascular decompression of cranial nerves. In addition, brain stem auditory evoked potentials (BSEPs) were monitored in these patients. No patient suffered significant hearing loss. Direct monitoring of auditory nerve potentials complements the recording of BSEPs because the auditory nerve potentials can be visualized without averaging many responses. Therefore the effect of any intraoperative manipulation that is harmful to the auditory nerve can be detected instantaneously.

Technique of hearing preservation in small acoustic neuromas.

The ideal operation for acoustic neurinoma would not only provide total excision without injury to the brain stem and with preservation of facial nerve function, but would also allow retention of useful hearing in those patients who come to operation with intact hearing function. Documented preservation of useful hearing in the rather extensive literature concerning acoustic neurinomas is rare. An operative technique has been developed utilizing a retromastoid approach, brain stem auditory-evoked potentials and direct auditory monitoring, facial nerve electromyography, and microsurgical techni

Return of function after intracranial suture of the trochlear nerve. Case report.

An excellent functional result was obtained after microsuture of a trochlear nerve which was inadvertently divided during the course of a craniotomy. Successful intracranial suture of this nerve has not previously been reported. Ophthalmoscopic detection of excyclotorsion of the eye on the side of the trochlear nerve palsy is discussed, along with its value in the differential diagnosis of acquired vertical diplopia.

Visual abnormalities with multiple trauma.

The diversity of pathogenetic mechanisms involved in posttraumatic visual impairment was reviewed in a study of the hospital records of 24 patients admitted with multiple injuries. Most major visual abnormalities occurred in young people (average age 33 years) who presented with a wide range of overall severity of injury (injury severity score 13-47) and involvement of the central nervous system (Glasgow coma scale 5-15). Bilateral or monocular blindness developed in 63% of patients. Seventy percent of the injuries involved the anterior visual pathways with damage to the optic nerve alone acco

Dizziness in childhood.

Dizziness in childhood is not an infrequent symptom. Accurate history taking and close co-operation between otologist, paediatrician and neurologist are necessary in the approach to the dizzy child. Most cases of childhood dizziness settle in time and investigations should be carefully selected; those with severe and persistent dizziness or ataxia should be thoroughly investigated including: EEG, ENG, calorics and CT scan. The conditions causing dizziness in children are discussed and are illustrated with case histories from our series of 27 children. Dizziness of unknown aetiology, serous oti

Deafferentation pain after posterior rhizotomy, trauma to a limb, and herpes zoster.

After incisional or alcoholic destruction of trigeminal posterior rootlets, constant dysesthesias of major degree referred to some part of the markedly denervated zone develop in 5 to 15% of the patients. The full severity may not appear for weeks or months. There is no allodynia or hyperpathia of the denervated zone. Bulbar trigeminal tractotomy with sparing of touch sensation produces severe dysesthesias in a tiny percentage of the patients, as does selective destruction of pain fibers by radiofrequency heating or glycerol. Spinal posterior rhizotomy elicits in less than 4% a lasting dysesth

[Regressive multiple paralysis of the cranial nerves and lymphocytic meningitis. 8 cases].

Eight cases of spontaneously regressive cranial nerve paralysis related to the meningo-radiculitis reported by F. Rohmer et al. in 1974 are described. The epidemiological, clinical features, and the CSF changes which should allow early diagnosis are detailed, but this can only be confirmed by the spontaneous regression of the condition. The problems in determining the etiology are emphasised.

Cranial nerve injury during carotid arterial reconstruction.

In a series of 109 carotid arterial reconstructions cranial nerve injury was observed in 14 of 102 patients. Ipsilateral peripheral hypoglossal nerve injury occurred in nine patients with carotid occlusive disease. The marginal mandibular nerve was injured in three patients and recurrent laryngeal nerve dysfunction was noted in four patients. Two cranial nerves were injured in two patients. Full recovery of hypoglossal dysfunction was seen within 2-52 weeks (average 20 weeks). The nerves are injured by retraction to clear the operative field or by postoperative haematoma. Risk factors include

[Post-traumatic optochiasmatic syndrome].

A case of post-traumatic opto-chiasmatic syndrome is described, and the principal characteristics of the affection outlines. This rare syndrome complicates severe cranial injuries, and is simple to diagnose when confronted with a patient with bitemporal hemianopsia frequently associated with diabetes insipidus or paralysis of certain cranial nerves. Suggested mechanisms for the lesion include sagittal stretching of the chiasma or an ischemic focus.
