From the last 100 cases of esophageal atresia treated since october 1975, the authors emphasised the incidence of left or right recurrent laryngeal nerve palsy. This incidence is about 20%. Most often, an unilateral palsy is asymptomatic, but in a few cases many complications have been reported. Therefore it is necessary to insist on the surgical and anatomic factors which could increase the risk of a nervous injury and the technics to prevent it.

28 patients with dizziness after head trauma were treated by vestibular neurectomy. Comparisons between the results of surgery and preoperative symptoms and findings showed that the best results were achieved in patients with episodic true vertigo and profound hearing loss. Patients who suffered from constant unsteadiness and/or postural vertigo usually improved, if both audiological and vestibular tests had shown clear pathology. Neurectomy gave poor results if functional tests had shown no pathology or reduced vestibular reaction only. The results support the view that in patients with post-

Every structure contained within the temporal bone in close proximity to the facial nerve is at risk during intratemporal bone surgery on the facial nerve. We present a review of the causes of injuries to these structures and of ways to prevent such injuries. This review includes information drawn from pertinent literature, from the author's experience managing 139 patients undergoing temporal bone surgery for a variety of facial nerve disorders between 1974 and 1981, from an analysis of 43 of our patients whose hearing was evaluated before transmastoid facial nerve surgery and again 6 months

A trigeminal neurinoma which produced the signs and symptoms of a localized intratumoral hemorrhage without showing any extratumoral hemorrhage successive to a minor head injury is reported here. A 33-year-old man noticed pain and hypesthesia on the right side of the forehead and double vision on the day following an occipital trauma. A computed tomographic scan demonstrated a large high-density mass in the anterior part of the right cerebellopontine angle. A diagnosis of trigeminal neurinoma was made and the tumor was successfully removed. The largest part of the tumor consisted of a blood cl

The authors give a brief introduction on the subject of mucocele and emphasize the rarity of the ethmoid-sphenoid location (55 cases listed by Lundgren in 1961). The principal clinical features are discussed which are: ocular and periocular and sometimes, occipital pain; ocular symptoms, often bilateral, of compression of the optic nerve and of the 3rd and 4th cranial nerves; and only rarely, slight signs of nasal obstruction or of hypopituitarism. Of the various surgical approaches, the lower trans nasal septum operation seems to be the best choice. A case report of a patient with copious spo

Two patients developed traumatic orbital haemorrhage resulting in prolonged blindness. Orbital decompression successfully reversed the visual loss, though both eyes had no light perception for at least 3 1/2 hours. The clinical findings suggest damage to the posterior optic nerve as cause of the visual loss. The optic nerve tolerance time to ischaemic injury may be longer than previously thought.

A review is presented of the clinical features of glossopharyngeal neuralgia, as analysed from 304 reported instances. The sex-ratio of the condition is equal, the peak-age at onset is between 40 and 60 yrs, and left-side involvement predominates in females. The clinical features are detailed in 18 specific items. In a review of the aetiology it is argued that the so-called "idiopathic" type of IXth nerve neuralgia (which, upon careful analysis, may still rest upon either an obscure tumor or infection or elongated styloid process) should be primarily attributed to vascular elongation and loopi

Seven cases of nonrecurrent inferior laryngeal nerves have been presented from a review of 1,000 consecutive thyroidectomies over a 20 year period. In two of these seven cases, both a nonrecurrent nerve and an additional recurrent branch were present on the right side. This double nerve presentation has not been described before. Unless one is aware of this possibility, one might inadvertently injure the major nonrecurrent trunk, having identified only a small recurrent branch. We emphasize the need for a complete nerve identification technique.

The authors report a case of chiasmal compression resulting from hemorrhage into a pituitary adenoma following minor head trauma. The adenoma was still confined to the pituitary fossa and only in retrospect had caused any endocrine effects prior to the accident. The diagnosis in this case was established by computerized tomography, angiography, subsequent surgery, and pathological examination of the specimen. There was significant but partial recovery of vision following surgery.

The concept of a circumlinear vessel (CLV) of the ophtic disc was recently introduced into the opthalmic literature, and baring of the CLV was suggested as a sign of acquired disc injury, especially from glaucoma. In a review of fundus photographs of 27 eyes with optic nerve pits, only one eye was found in which the CLV was bared. This observation supports the concept that baring of the CLV is a sign of acquired disc change.

No consensus exists today on the management of immediate-onset post-traumatic facial nerve paralysis. Controversy surrounds the timing of surgical intervention and the role of electrophysiologic testing. Three patients are presented who sustained immediate, complete facial paralysis following closed head trauma. They did not have prompt facial nerve decompression. In each case, electroneurography and electromyography showed complete nerve degeneration and denervated muscle. Despite the results of the electrophysiologic tests, all patients underwent late surgical decompression of the nerve: one

A method for identification and functional recording of the recurrent nerve during neck operations is described. The basic principle implicates electrical stimulation of the recurrent nerve causing adduction of the vocal cord registered by means of a balloon placed between the vocal cords. Three different recording methods have been tested in animal experiments and during 26 neck operations on patients. The most simple method employed is recommended as a routine procedure, likely to reduce surgical damage to the recurrent nerve.

Carotid endarterectomy may be complicated by damage to the adjacent cranial and cervical nerves. The true incidence and severity of these complications is rarely reported. In a prospective study of 43 carotid endarterectomies there was a 25 per cent incidence of temporary vocal cord palsy, 5 per cent incidence of hypoglossal nerve damage and anaesthesia of the greater auricular and transverse cervical nerves was found in 60 per cent and 69 per cent of patients respectively 1 week postoperatively. There was some recovery of these nerve injuries over a 6-month period.
