[Villaret's syndrome due to extra-cranial internal carotid aneurysm: a case report].

A 69 year-old male was admitted to our clinic with the chief complaint of hoarseness and difficulty in swallowing. These symptoms occurred about 5 months after penetrating neck injury. Neurological examination revealed right ninth to twelfth cranial nerves palsy and Horner's syndrome. Plain skull X-P demonstrated two broken pieces of glass below the right mastoid processus. The right carotid angiogram showed a 45 x 25 mm aneurysm originated from the right internal carotid artery just below the carotid canal. As the first operation, gradual occlusion of right internal carotid artery combined wi

[Facial nerve reconstruction--after the operation of C-P angle tumor].

Nineteen patients who undergo facial nerve reconstruction after the operation of cerebellopontine angle tumor from 1964 to 1981 were investigated. Eighteen cases were of acoustic neurinoma, and one was of low grade astrocytoma. Spinal accessory-facial nerve anastomosis was performed in thirteen cases. Cross facial nerve graft was done in three cases. Hypoglossal-facial nerve anastomosis, phrenico-facial nerve anastomosis, and intracranial direct anastomosis were done in one case each. In spinal accessory-facial nerve anastomosis cases, good result was obtained only in 30%, but using microsurgi

[Iatrogenic lesions of the spinal accessory nerve. Microsurgical repair].

Iatrogenic lesions of the spinal accessory nerve were explored and repaired in 10 patients. Symptoms included pain, trapezius muscle atrophy, restricted active abduction and drop shoulder; as they developed after cervical lymph node biopsy, diagnosis was usually easy. Depending on the operative findings, microsurgical repair involved suture, neurolysis or grafts. Clinical and electromyographic results are available from 7 patients followed up for 14 months on average: complete recovery was observed in 3 cases and satisfactory but incomplete recovery in 3; there was one failure. Since these les

Superficial fascia rhytidectomy. A restoration of the SMAS with control of the cervicomental angle.

The surgical correction of an anatomic defect should re-establish the normal relationship of the tissues which contribute to the defect and restore or improve function. When the repair of the anatomic defect is not maintained, it may be necessary to alter the tissues to accomplish a more lasting result. Restoration of the SMAS cephalad to the cervicomental angle by superficial fascia rhytidectomy with a complete transverse resection of a segment of the platysma muscle at the level of the cervicomental angle fulfills the criteria of the young face by improving the function and contours of the f

Neuropathology of spasmodic dysphonia.

Spasmodic dysphonia is a devastating voice disorder of unknown etiology, with a variable clinical presentation and response to treatment. Three independent evaluations of brain stem function were performed on spasmodic dysphonic patients, and age and sex-matched controls. Statistically significant (p less than 0.01 approximately 0.05) differences were noted between these groups, and the findings were consistent with impairment of somatic and visceral brain stem pathways. A significant correlation (p less than 0.05) was found between the severity of tested central nervous system impairment and

Optic nerve damage in head trauma: clinical and experimental studies.

A total of 33 cases with optic nerve damage due to blunt head trauma were followed for 6 months to one year with the average follow-up period of 9 months. Eleven cases were treated by surgical decompression of the optic canal and 22 cases were followed by medical treatment. The optic canal fracture was documented in 51% of the cases, and the incidence was not significantly different between both groups. When the visual acuity was zero at the time of the first examination, no visual improvement could be obtained. Omitting these cases, visual improvement was seen in 42.8% of patients treated by

Experimental studies of psychophysiological factors in sudden cardiac death.

Earlier research in the field of sudden cardiac death is reviewed. Such studies have largely oriented towards the provocation of myocardial injury and asystole in normal animals. However, such investigations constitute an inadequate model to describe the clinical appearance of sudden death, where underlying coronary disease is often present and the precipitating event is usually ventricular fibrillation rather than asystole. This report describes a series of studies designed to investigate the processes underlying cardiac vulnerability and the influence upon it of various psychological stresse

Pathological features of human facial nerve after central injury during tumor removal, with special reference to the non-motor component.

After injury to the intracranial portion of the facial nerve, undegenerated myelinated nerve fibres were seen to form a distinct bundle in the peripheral part of the transverse fascicular area at the level distal to the geniculate ganglion. The numbers and diameters of the fibres were similar to those seen in the nervous intermedius portion of the facial nerve in experimental animals. The average number of these apparently non-motor fibres was 2500. Their average diameter was 2.4 micrometer.

Persistent hiccups.

We present the case of a patient who presented with persistent hiccups induced by a hair irritating the tympanic membrane and activating the vagally mediated hiccup neural reflex arc. Commonly identified etiologies and treatments of this condition are then reviewed.

The influence of chronic vitamin A deficiency on human and animal ears.

After feeding young rats a diet deficient in vitamin A, we examined the inner ear with the electron microscope. There were changes in the cuticle of the outer and inner hair cells. Furthermore, there were changes in the reticular system of the intermediate zone and massive degenerative changes in the ganglion cells of the VIII nerve. In a second experiment with older animals we found no significant changes in the sensory cells, though there was new bone formation in Rosenthal's canal and damage to the ganglion cells, of a lesser extent than was evident in the first experiment, however. In a fu

Chemodectomas arising in temporal bone structures.

Eighteen patients with chemodectomas arising in temporal bone structures were evaluated and treated at the University of Florida. Seventeen patients have each been followed a minimum of 3 years. Patients were retrospectively staged as having "Локально" or "advanced" disease, depending on the presence or absence of bone destruction and/or cranial nerve involvement. Fourteen of the patients received radiation therapy as all or part of their therapy; 6 patients were treated with radiation therapy alone, 3 patients were irradiated immediately postoperatively for residual disease, and 5 patients had r

Oculomotor nerve anastomosis.

We report a patient who underwent anastomosis of the left oculomotor nerve, which had been inadvertently and bluntly severed during the resection of a left parasellar chromophobe pituitary adenoma. Neural regeneration was confirmed by the reappearance of voluntary adduction of the eyeball, clinical signs of aberrant regeneration, and the appearance of an action potential on electromyography of the levator palpebrae superioris muscle 1 year after end-to-end anastomosis of the nerve.

Superior oblique transposition for third nerve palsy.

Four eyes of three adult patients underwent anterior transposition and advancement of the superior oblique tendon without trochleotomy in the surgical management of oculomotor nerve palsy. In each of the cases, this procedure proved unsatisfactory, either because of inadequate horizontal alignment, postoperative hyperdeviations, or paradoxical ocular movements. Even in these difficult cases, "supermaximum" horizontal rectus muscle surgery appeared to result in satisfactory ocular alignment.

H-type tracheo-oesophageal fistula in children with special reference to surgical management and to repair of recurrent nerve injury.

The majority of tracheo-oesophageal fistulas are best treated by a cervical approach, in which there is an obvious risk of injuring the recurrent laryngeal nerve. Usually there is a temporary dysfunction, but if the nerve is completely divided, an attempt at microsurgical direct end-to-end anastomosis is worth while, which is here proven in a child of three weeks of age.

Gastric ulceration after fundic wrapping. Vagal nerve entrapment, a possible causative factor.

Transabdominal fundoplication is an effective operation for control of gastroesophageal reflux in the majority of patients. The operation is, however, associated with several sell-documented early and late complications. Recently, a few reports have appeared describing benign gastric ulceration (GU) occurring from one month to several years postplication. The etiology of GU in this setting is unknown, but preexisting delayed gastric emptying, pyloric incompetence, faulty wrap construction, local ischemia, and trauma to the vagus nerves have been incriminated. During a recent seven-year period,

Propranolol and thyroidectomy in the treatment of thyrotoxicosis.

For decades, the preparation of a hyperthyroid patient for surgery took several weeks or months utilizing thyroid blocking agents and iodine. In 1973, a preliminary report of 20 patients with hyperthyroidism treated with propranolol and thyroidectomy was presented. It was found that a thyrotoxic patient could be prepared for surgery, in an emergency, by intravenous propranolol in less than an hour, or electively by oral propranolol within 24 hours. Since then, 140 additional patients have been similarly treated. It continues to be true at this institution that propranolol, a beta-adrenergic bl
