Facial nerve preservation in patients with large acoustic neuromas treated by a combined middle fossa transtentorial translabyrinthine approach.

With large acoustic neuromas, the primary goal of surgery is safe total removal of the tumors, and the secondary goal is preservation of nearby neural structures, including the facial nerve. In a series of 15 consecutive patients with large cerebellopontine angle tumors, all of which were more than 2.5 cm in diameter, tumor excision was performed by a one-stage combined middle fossa-translabyrinthine approach. There were 13 acoustic neuromas, 10 of which were more than 4 cm in diameter, one petrous apex meningioma 4 cm in diameter, and one facial neuroma 3 cm in diameter. The tumors were total

Motor speech deficit following carotid endarterectomy.

Stroke as a complication of carotid endarterectomy has been extensively reviewed. Considerably less attention has been directed to local injuries of the cranial nerves and their branches. Verta, Hertzer, Imparato, DeWeese, and Matsumoto have reported experience with these injuries. DeWeese found a 9.7% rate of cranial nerve injury, while in Hertzer's series, 15% of patients had nerve dysfunction in the early postendarterectomy period. In 1980, Liapis in a preliminary report found that when postoperative examination was supplemented by detailed evaluation by speech pathologists, the incidence o

Late sequelae of iatrogenic damage to chorda tympani nerve.

Four and a half years following middle ear surgery involving damage to the chorda tympani nerve we conducted a follow-up study in 10 out of 14 patients in whom we had previously investigated gustatory and submandibular gland function, pre- and immediately postoperatively. Following transection of the chorda tympani nerve (5 patients) the secretory volume of the affected gland has risen again but is still lower than that of the healthy gland. The secretory volume of the latter is increased, possibly by compensatory hypertrophy. The still existing dysgeusia, not noticed by the patients, is occas

Atypical clinical presentation of oculomotor (III) nerve palsy.

The atypical clinical presentations of three patients with an oculomotor (III) nerve palsy are outlined. The first patient is a 49-year-old with a painful, pupil-sparing ophthalmoplegia of sudden onset due to a pituitary adenoma which had eroded into the cavernous sinus. The second patient is a 7-year-old with a traumatic III nerve palsy who despite full recovery of her oculomotility, has been symptomatic due to a permanent internal ophthalmoplegia. The third patient is an 8-year-old who has undergone prolonged optometric therapy for an "atypical esotropia," but in fact has a congenital III ne

Value of oculomyodynamometry in traumatic diplopia. Direct ocular muscle force measurements.

In 21 patients with traumatic diplopia, oculomyodynamometry (OMD) was used to examine the force of the extrinsic ocular muscles. In nine patients with blow-out fracture of the orbit, the greatest impairment in the active force and function of the incarcerated muscle was found in linear fractures. In 12 cases of traumatic paresis of the cranial nerves, OMD enabled the degree of paresis of particular extraocular muscles to be assessed, was helpful in establishing a correct diagnosis as well as prognosis for the restoration of function of the affected muscles, and provided data useful in planning

Unilateral disk edema following tripod fracture repair necessitating optic nerve decompression.

Compression of the intracanalicular portion of the optic nerve as a complication of repair of a zygomaticomaxillary fracture is possible in the setting of multiple orbital fractures. This is a case report of optic canal compression of the optic nerve as a results of a delayed repair of a tripod fracture using the lateral approach. This occurred in the setting of a severely fractured orbit and resulted in florid unilateral disk edema, visual loss, and an afferent pupil defect. After optic canal decompression, vision was restored and the disk edema resolved. However, the afferent pupil defect ha

Speech rehabilitation in total paralysis of the tongue.

A 31-year-old woman attempted suicide by cutting her throat. In doing so, she severed the hypoglossal nerves bilaterally. The patient received full speech rehabilitation following nerve suture and logopedic training. The surgeon who is faced with deep wounds of the anterior part of the neck, especially cut wounds (due to accident, suicide, or murder), should be aware of the possibility of a severance of the hypoglossal nerve, and perform surgical exploration of the tongue later when a paralysis of the tongue has been diagnosed with the aim of doing a nerve suture. With modern technique, interf

[Optic nerve tract injuries (author's transl)].

The length of the optic nerve tracts exposes them to damage from cranial injuries. The intraorbital segment of the optic nerve is vulnerable, as bone protection is incomplete and frontal injuries frequent, while indirect lesions of the optic nerve due to trauma raise diagnostic problems in comatose patients. Study of photomotor reflexes and VEP supplies valuable information. Unilateral mydriasis is not a sign of blindness, but of a lesion of the IIIrd nerve, which, may be compressed during engagement of the temporal lobe in the tentorium cerebelli region. The mydriasis indicates the side of th

Dizziness after head trauma: clinical and morphologic findings.

Twelve patients with balance problems resulting from head injury were treated by a translabyrinthine or middle fossa vestibular neurectomy. The clinical, otoneurologic, and surgical data combined with morphologic findings on the removed operative eighth nerve specimens were evaluated with the aim of deciding the site of primary lesion in each case. In six patients a peripheral lesion was interpreted. Four of them had features of delayed endolymphatic hydrops syndrome, the fifth patient might have suffered a fracture of the stapedial footplate with associated perilymphatic fistula. The sixth pa

Accessory nerve palsy: an uncommon etiology.

Accessory nerve palsy due to surgery in the anterior triangle of the neck is uncommon. Two such cases 1, after a carotid endarterectomy and the other after rhytidectomy, are described here. Both cases were treated with analgesics and physical therapy. Followup evaluation after 1 year revealed complete clinical and electrophysiologic recovery in only the first case. Recognizing accessory nerve palsy after surgical procedures in the necks is emphasized as important for proper management. An attempt is made to explain the reason for lack of improvement in the second case.

Transmastoid decompression of the facial nerve in temporal bone fracture.

Among 41 cases of facial palsy caused by closed heart injury, temporal bone fractures were surgically confirmed in 36 cases, of which there were two mixed fractures and 34 longitudinal fractures. The fracture involved the geniculate ganglion area in 20 cases (55%). In 15 cases, decompression of the facial nerve was carried out using the transmastoid supralabyrinthine approach with disarticulation of the incus; in only five cases was the middle fossa approach used. The technique avoids the craniotomy for the middle fossa approach and is a reliable method of treatment in the majority of patients
