A case of successful bilateral decompression of both recurrent laryngeal nerves two days following thyroidectomy (with injury to the nerves) is reported. The ideal time for surgical intervention in such cases is discussed. Electrodiagnostic tests for objective documentation of nerve injury are mentioned. The authors believe that the recurrent laryngeal nerves should be explored in comparable patients when tracheostomy is indicated for protection of the airway.

Permanent functional disturbances of the inferior alveolar nerve after sagittal osteotomy of the ramus of the mandible form an increasingly common feature of the reported series. Reference is made to earlier experimental research in a description of a preliminary series in which this complication was avoided by cleaving the two cortical substances with a thin cement spatula used as a scalpel. The results were extremely encouraging. There were no reports of permanent functional disturbances of the alveolar nerve, while the incidence of temporary defects was also reduced.

A case of plasmacytoma of the clivus is presented. The patient was asymptomatic until a coincidental traumatic lesion produced manifestations of vagus nerve dysfunction. Conduction was blocked in some fibers of the vagus nerve while, at the same time, episodes of activity were triggered in fibers of the same nerve trunk, resulting in an unusual combination of symptoms. The symptomatology of the case is discussed.

Aberrant regeneration of the oculomotor nerve usually follows injury to the nerve by posterior communicating arty aneurysms or trauma. A case of idiopathic third nerve palsy with pupillary involvement occurred in an otherwise healthy 38-year-old man. Follow-up examination 32 months later showed evidence of oculomotor function with aberrant regeneration.

Traumatic third nerve palsy is an unusual condition. It is a serious prognostic feature, not for life but for permanent, serious neurological deficit. It can only be diagnosed with confidence if it is known to have been present from the moment of impact and if adequate investigation excludes a compressing haematoma.

A 29-year-old man sustained a metallic (98% iron) foreign body in the optic nerve head, which resulted in a massive vitreous hemorrhage and a secondary branch retinal artery occlusion. An initial pars plana vitrectomy was successful in removing the vitreous hemorrhage, but numerous attempts with pars plana and intravitreal electromagnets failed to dislodge the foreign body. A second operation using a microscalpel and foreign body forceps with the bimanual pars plana technique was successful in removing the steel foreign body from the optic nerve-head.

In 12 cats and two Macaca mulatta monkeys, areas of chronic focal injury were induced in the trigeminal root by implantation of a chromic suture in the nerve near its entry into the brain stem. Experimental lesions were examined histologically and electrophysiologically at 1 week, 3 weeks, and 6 weeks after surgery. At 3 weeks following implantation, lesioned nerves showed areas of focal inflammation surrounding implanted sutures with prominent demyelination of adjacent axons. A total of 497 trigeminal units were studied, 304 of which had associated demyelination of the root. In these 304 cell

In order to determine the incidence of cranial nerve injury during carotid endarterectomy, preoperative and postoperative otolaryngologic examinations were performed in a prospective series of 240 operations. Thirty patients experienced a total of 38 cranial nerve injuries, involving the recurrent laryngeal nerve in 14, the hypoglossal nerve in 13, the marginal mandibular nerve in six and the superior laryngeal nerve in five. Postoperative dysfunction affected one cranial nerve in 25 patients, two nerves in four and three nerves in one patient. Most cranial nerve injuries were asymptomatic or

Total retinal detachment and neovascular glaucoma occurred after a penetrating injury to the left globe that left the eye blind and painful. A retrobulbar injection of alcohol and enucleation modified but did not relieve the orbital discomfort. At surgery, we discovered a neuroma with cartilaginous metaplasia involving the supratrochlear nerve. Excision of this mass produced relief of symptoms. This is, to our knowledge, the first reported case of a neuroma with cartilaginous metaplasia involving the supratrochlear nerve.

The authors believe that a dental practitioner can be adept at observing insidious signs of neurologic impairment secondary to trauma of the head and neck. If approached logically in a systematic order, the course of neurologic assessment of a patient with traumatic dental injuries can be achieved in a short time. If neurologic damage is suspected, immediate medical referral is indicated, as impending neuropathologic crises of secondary injury may be prevented with definitive early care. Medical personnel will benefit in their diagnostic course from information gathered by the initial provider

Endolymphatic saccotomy, vestibular neurectomy via middle fossa, labrinthectomy and translabyrinthine vestibular neurectomy have been used by the authors in 54 patients with episodic vertigo, chiefly from Ménière's disease. Over a mean follow-up period of 15 months hearing retention and vertigo control were of the same general order as in the reports of other surgeons. Our current ideas on indications for the different surgical procedures are discussed.

The reported peripheral nerve complications of therapeutic irradiation in humans include brachial and lumbar plexus fibrosis and cranial and peripheral nerve atrophy. We have encountered 9 patients with malignant (7) and atypical (2) peripheral nerve tumors occurring in an irradiated site suggesting that such tumors represent another delayed effect of radiation treatment on peripheral nerve. The neoplasms appeared 4 to 41 years following radio-therapy, at a time when the patients (6 females and 3 males) were 16 to 70 years old and cured of their original tumors. Eight patients had received irr

Pattern shift visual evoked response (PSVER) latency has been shown to be an extremely sensitive but nonspecific measure of optic nerve dysfunction. The test has been most useful in detecting unsuspected demyelinating lesions of the optic nerve, but abnormal results have been reported in a variety of other conditions affecting the optic nerve, including ischemic optic neuropathy. We report a patient with left internal carotid artery occlusion, possibly secondary to neck trauma, with normal ophthalmological examination and abnormal PSVER. The suggested mechanism is subclinical optic nerve ische

A patient is described who devloped a bilateral carpal tunnel syndrome and a trigeminal sensory neuropathy as the initial manifestations of mixed connective tissue disease. Either condition can occur in other connective tissue diseases, but this combination has not been reported previously in mixed connective tissue disease, in which, however, trigeminal sensory neuropathy is frequently seen.

When facial nerve paralysis complicates a mandibular fracture, it may be difficult to locate the point of injury, since the nerve may be injured in the fallopian canal by a secondary temporal bone fracture or in soft tissue by mandibular fragments. Following a review of eight previously reported cases, this paper presents two additional cases, one with bilateral, complete paralyses and the other with a unilateral paresis. In the first, the condyles were driven posteriorly, resulting in bilateral temporal bone fractures, a unilateral external canal stenosis, and a unilateral sensorineural heari

A technique is described for performing a radical neck dissection by elevating the muscle out of the neck to facilitate the complete removal of all the neck structures without injury to the muscle and without significantly increasing the operative time. The neck dissection can be performed as a "classical" radical neck dissection by removing all the neck structures en bloc, including the jugular vein and spinal accessory nerve. Alternatively, a "functional" radical neck dissection can be done in selected cases by dissecting out and preserving the jugular vein and/or the spinal accessory nerve.

The jugular foramen syndrome (JFS) specifically refers to paralysis to the IX-XIth cranial nerves. In a more general meaning, however, any combination of palsies affecting the last four cranial nerves has been referred to as a JFS. The anatomy of the jugular foramen and the structures which traverse it are reviewed. Two cases histories of a JFS are described; the first with chronic otitis media, and the second with a glomus tumor. A variety of neoplasms, vascular insults, infections, and trauma have been reported to cause JFS. Treatment is directed toward the underlying cause. A variety of rad
