Traumatic chiasmal syndrome.

We studied 11 patients with the uncommon finding of traumatic chiasmal syndrome after closed head trauma. Visual field defects varied from complete monocular blindness and contralateral temporal hemianopia to subtle bitemporal arcuate scotomas. The degree of visual loss was not necessarily related to the severity of the craniocerebral trauma. Diabetes insipidus was present in half of these patients, but unlike the visual abnormalities, this complication was transient.

Ischaemic optic neuropathy.

Ischaemic optic neuropathy is a well recognised cause of sudden visual loss in middle and late life. It is characterised by painless visual impairment, pale swelling of the optic disc and nerve fibre bundle field defects. Although some cases are due to cranial arteritis, the majority of patients suffer from non-arteritic diseases, particularly hypertension. The present study consists of a reveiw of 22 cases of ischaemic optic neuropathy. Apart from cranial arteritis and vascular disease, migraine and trauma appear to have a causal relationship to the disorder.

[Trauma of the facial nerve at the geniculate ganglion (author's transl)].

Fractures of the pyramid, longitudinal fractures in the majority of cases, lead to a damage of the facial nerve in the surrounding of the geniculate ganglion. This area has to be controlled surgically when a facial nerve paralysis occurs after a head injury.

Traumatic bitemporal hemianopsia: case report.

Bitemporal hemianopsia due to trauma to the optic chiasm is uncommon following head injury. In the patient described, a hemianopic field defect was seen 12 days after frontal basilar skull fracture. We feel the cause is intrachiasmatic contusion, edema, and hemorrhage. Serial ophthalmologic examinations and possibly use of steroids are recommended.

[Histo-morphological findings at the chiasma opticum in cases of craniocerebral trauma (author's transl)].

36 cases with pituitary haemorrhages were found histologically in 52 cases of lethal craniocerebral trauma. Simultaneously we found primary and secondary traumatic changes of chiasma opticum in 10 cases. The genesis and significance of the results are discussed.

Traumatic aneurysm of the internal carotid artery and epistaxis. (Review of literature and report of a case).

A review of 108 years of world literature revealed 69 cases fulfilling the picture of a clinical syndrome of head injury, a latent period followed by epistaxis and cranial nerve lesions. A similar case has been added which is the fourth case to be reported to be due to a bullet injury. The applied anatomy of the intracranial internal carotid artery (ICA) and the aetiology, clinical syndrome, pathology, diagnosis, prognosis and management of ICA aneurysms have been briefly discussed.

Blowout fracture of the medial orbital wall, with entrapment of the medial rectus muscle.

A case of fracture of the orbital wall, with entrapment of the medial rectus muscle and paresis of the 6th nerve in association with a large fracture of the orbital floor, is presented. The diagnosis was made primarily by the forced duction test.

[The computerized tomography in findings posttraumatic hematoma (author's transl)].

The authors report two cases of posttraumatic hematoma in the orbit. The first patient presents an unilateral amaurosis through indirect trauma of the optic nerve. The computerized tomography reveals a hematoma of the optic nerve sheath, confirmed by surgery. The second patient presents a posttraumatic limitation of the ocular motility and the computerized tomography reveals a hematoma of two extraocular muscles.

Neoplastic autovagotomy causing gastric stasis.

Neoplastic autovagotomy causing atonic gastric stasis is extremely rare. Two cases are reported in which marked gastric stasis was complicated by a bezoar and a gastric volvulus respectively. Both cases were associated with a left hilar bronchial carcinoma, and malignant invasion of the vagus nerve may have been the underlying cause.

Paraparesis in hereditary multiple exostoses: case report.

The authors report a case of hereditary multiple exostoses (HME) with neurologic complications, and review the literature. A 23-year-old man exhibited a worsening spastic paraparesis with sphincter dysfunction. The cranial nerves and the exteroceptive and deep sensations were apparently undamaged. The family history, the physical examination, and the systemic radiologic examination revealed all the characteristics of HME. The neurologic complication was caused by an exostosis, arising from the C2 right hemilamina, compressing the spinal cord. The patient quickly improved after a laminectomy.

Gunshot injury to the temporal bone: an analysis of thirty-five cases.

By retrospective analysis of 35 surviving gunshot injuries to the temporal bone and by presentation of 6 representative cases, the management and reconstructive procedures of these injuries in the Tulane University affiliated hospitals are presented. The most frequent single injury was facial nerve paralysis (16 cases), followed by external canal injury and conductive hearing loss. Anacusis occurred in 12 cases. In conductive hearing loss the intact posterior wall tympanoplasty with mastoidectomy (16 cases), or the modified radical mastoidectomy (5 cases), allowed middle ear reconstruction. Tr

Optic nerve injury in fracture of the canal.

A case of unilateral blindness following blunt injury to the skull is presented. The patient died 4 days after the initial injury, presenting the rare opportunity of a detailed histopathological study of the acute features of the condition. The findings are discussed in the context of current theories of the pathogenesis of optic nerve injury in fractures of the optic canal.

Clinical and experimental correlations in temporal bone fracture: the facial nerve.

In this review of 50 patients with fracture of the temporal bone, 11 patients had ipsilateral facial paralysis, three of immediate and eight of delayed onset. Eight patients with complete paralysis of the facial nerve had facial nerve decompression; six of these had incomplete return of function. Only one patient with a delayed complete facial paralysis had complete return of function. The poor functional results in patients with delayed complete facial paralysis due to temporal bone fracture encourage facial nerve decompression in all stable patients with the complete facial paralysis of temp

Mechanisms of hyperventilation in head injury: case report and review.

We report the case of a head-injured patient with spontaneous hyperventilation who had recurrent episodes of relative hypoventilation associated with increases in intracranial pressure. Detailed ventilatory studies were performed during the 2nd week after injury. Our findings in this patient prompted us to review the possible mechanisms underlying the observed changes. We suggest that spontaneous hyperventilation in head injury is secondary to a decrease in cortical inhibitory influences on respiratory control mechanisms and that the transient episodes of relative hypoventilation observed in o

Extratemporal facial rehabilitation.

Treatment of the paralyzed face has made significant advances over the past 30 years. Evaluation of the paralyzed face with the concepts of early and delayed extratemporal facial rehabilitation and a review of the multiple methods of re-animation are discussed. Dynamic reconstruction is preferred to static methods except under special circumstnces. The many modalities of treatment include nerve grafting or crossover, regional muscle transposition or free muscle grafts, muscle interdigitation, and nerve implantation with the static methods of fascial stripping and rhytidectomy used in exception

Trigeminal neuralgia and multiple sclerosis: demonstration of the plaque in an operative case.

Trigeminal neuralgia is unique to humans. The most common cause seems to be an injury to the myelin of the trigeminal nerve root entry zone as it extends for several millimeters lateral to the pons. Jannetta has developed an elegant retromastoid microsurgical approach to this region. He has identified a compression-distortion phenomenon of this nerve root entry zone, usually from an anomalous position of the superior cerebella artery. Trigeminal neuralgia can also occur in association with multiple sclerosis, when the plaque lies in this same location. The historical evidence for this explanat
