Complete third nerve paralysis and decreasing vision secondary to subdural hematoma.

We treated a patient with a subacute subdural hematoma who manifested only a complete third cranial nerve paralysis and decreased visual acuity. The gravity of delayed diagnosis in such a situation should prompt the ophthalmologist to consider subdural hematoma as the underlying cause in cases of third nerve paralysis.

[Surgical intervention in traumatic orbital syndromes].

Craniofacial traumata may be accompanied by retroorbital fractures with severe ophthalmologic complications. Each of the clinical syndromes is characterized by a combination of ophthalmoplegia, paralyzed pupillary function and visual disorder. These deficiencies are caused by injury of muscles, nerves and vessels and compressive retroorbital hematoma. In the case of such damage, the poor prognosis can be improved by surgical intervention consisting in the evacuation of the retroorbital hematoma.

Rehabilitation of the face following traumatic injury to the facial nerve.

Etiology, diagnosis, and surgical management of facial paralysis due to traumatic injury of the VIIth cranial nerve are discussed. Sixty patients are reviewed who underwent some type of surgical procedure for the repair of the facial nerve. These cases are categorized according to etiology, which includes temporal bone fractures, iatrogenic injuries, and penetrating wounds of the head and neck. The results of a poll of eight leading otologists on their approaches to several aspects of the surgical management of these injuries are presented in the Discussion section. The diagnostic and prognost

Neurological complications of anticoagulation therapy: a neurologist's review.

Anticoagulant-induced bleeding may involve the brain, spinal cord, or peripheral nerves. It may occur with either coumarin or heparin anticoagulants, in the absence of trauma, and while prothrombin times, clotting times, or partial thromboplastin times are in accepted therapeutic ranges. Bleeding involving the brain occurs usually as a subdural hematoma, with intracerebral or subarachnoid hemorrhages occurring less frequently. There may be diagnostic problems in the recognition of these anticoagulant-related subdural hematomas. The spinal cord involvement usually appears in the form of an extr

Utilization of a central auditory processing test battery in evaluating residual effects of decompression sickness.

Six professional divers were examined as patients who presented peripheral auditory, vestibular, or central symptoms attributed to incidents which occurred while diving. Each diver received a central auditory processing (CAP) test battery consisting of: (1) a dichotic sentence listening task, (2) a monotic filtered word task, (3) a binaural fusion task, and (4) an alternating speech task. The CAP test results, in all cases substantiated other neurological/vestibular test battery results which we have reported in detail previously.

Neurologic disturbances in Paget disease of bone: response to calcitonin.

The neurologic manifestations of Paget disease and the therapeutic effect of calcitonin were studied in 49 patients. Twenty-four patients (49%) had neurologic disorders involving cranial nerves other than the auditory system, brainstem, spinal cord, or spinal roots and nerves. Eighteen of the 24 patients (75%) showed significant subjective or objective improvement after calcitonin treatment. The effect of calcitonin treatment on spinal cord compression was dramatic in three of six patients. The observations made of these patients support previous data suggesting that the neurologic signs and s

Trends and profiles in stapes surgery.

Stapedectomy has become recognized as the procedure of choice in the surgical treatment of clinical otosclerosis. Based on results obtained in stapes surgery performed in 1977, profiles were established for hearing improvement, vertigo, tinnitus, chorda tympani injury and temporary threshold shift of high tones. Tympanometric and stapedial muscle reflex tests in cases of proved clinical otosclerosis also are discussed. The profiles indicate that stapedectomy performed on a regular basis and under ideal conditions is highly successful for the improvement of hearing. The incidence and probable c

Clinical observations in cyclodeviations.

Cyclodeviations differ from other strabismus forms in several aspects. Adaptive mechanisms other than suppression cause the patient to remain asymptomatic in spite of ophthalmoscopically demonstrable torsion of the globe around the sagittal axis. These mechanisms include cyclofusion and a monocular sensory adaptation causing the images seen by the paretic eye to appear vertically and horizontally aligned even though they do not fall on the vertical and horizontal anatomic-geometric retinal meridians. Pure excyclotropias, ie, those not associated with vertical deviations, respond well to anteri

Selective traumatic infarction of the human anterior hypothalamus. Clinical anatomical correlation.

A case of assult with bilateral manual avulsion of the eyes was followed by highly selective infarction of the anterior hypothalamus. The hypothalamic infarction occurred as a result of avulsion of part of the optic chiasm together with the anterior perforating arteries passing through it; Following this assault, symptoms of hypothalamic dysfunction included altered thermoregulation, alternating diabetes insipidus, and inappropriate antidiuretic hormone (ADH) secretion, altered patterns of sleep and arousal, and changing cardiac arrhythmias. The case casts light upon the vascular supply of the

[Avulsio bulbi (author's transl)].

It is reported about a 14 year old girl, sustaining an avulsion of the left eye-ball by a coat-rack. The optic nerve was cutted 18 mm retrobulbarly. In perimetry of the fellow-eye a temporo-superior defect was found as proof of traumatisation of the anterior-loop-fibers in the chiasm.

The superior oblique: a challenging extraocular muscle.

Isolated paralysis of one or both superior oblique muscles in a consecutive series of 43 patients was found to occur most often by indirect injury to the skull. The etiology in the group of spontaneous cases was mostly vascular. With rare exceptions the latter recovered spontaneously. Those with traumatic palsy may be due to "contrecoup" trauma at the decussation of the nerves or by decompensation of the cerebral fusional mechanism, with dissociation of the images of the two eyes and subsequent vertical deviation. The traumatic cases never recover spontaneously and recession or myectomy of the

Clinical observations on 600 cases of recurrent laryngeal nerve paralysis.

A statistical review was made on 600 cases of recurrent laryngeal nerve paralysis who visited the author's clinic during the last 16 years. The yearly incidence, the age and sex distributions and the classifications of the causes of paralysis were presented and discussed. A special attention was given to the topics of associated laryngeal paralysis, bilateral recurrent laryngeal nerve paralysis and the so-called epidemic paralysis. A brief comment was also made on diagnosis and treatment of recurrent laryngeal nerve paralysis in general.

Optic atrophy and glaucomatous cupping.

We reviewed 170 eyes of 112 patients with optic atrophy from various causes. Special attention was directed towards measured cup:disk ratios as well as presence of glaucomatous-like cupping of the optic nerve head. We observed a small but significant increase in nerve head cupping in eyes with optic atrophy when compared to contralateral eyes, as well as to eyes of 50 diabetic patients. No characteristic glaucomatous disk changes were documented. We evaluated these findings with respect to possible causes of glaucomatous disk and field changes.

Ocular neovascularization. The Krill memorial lecture.

The various vascular systems of the eye can undergo new vessel formation. In this presentation, I discuss new vessel growth in the cornea, lens, iris, ciliary body, choroid, retina, and optic nerve head. No single factor can explain all cases of ocular neovascularization; instead there are multiple factors which can affect the various susceptible vessels. Among the known vasculognic factors are: inflammation and its products, a hypoxic retina diffusable factor, the "tumor angiogenic factor," and possibly an aging factor. The different ocular beds possess differing sensitivity to the various va

An unusual late complication of Gasserian ganglion cecompression surgery. Case report.

The authors report an unusual complication following sugery for decompression of the Gasserian ganglion. Eight years postoperatively this patient developed a left temporal lobe granuloma caused by a piece of wood used to plug the foramen spinosum at surgery. The patient's clinical and radiological findings were suggestive of a glioma involving the temporal lobe.
