[Study of ganglioside therapy in maxillofacial surgery].

56 patients have been treated with cerebral gangliosides by the Authors. They underwent a maxillo-facial operation due to various pathological reasons (traumas, tumors, malformations). They presented secondary symptoms such as neurological deficits of the V and VII paired cranial nerves. The recovery from neurological sensitive, motor deficit have been positive, and the Authors have been encouraged to the therapeutical use of gangliosides in the mentioned pathology and to a wider experiment of the test product.

Surgery of the facial nerve.

Microsurgical technique and new temporal bone operative procedures have revolutionized surgery of the facial nerve. The purpose of this paper is to describe the diagnostic evaluation of the patient with a facial paralysis including topographic site of lesion studies. Surgical techniques for exposing the facial nerve from the internal auditory canal to the stylomastoid foramen are discussed in detail.

Total blindness after trivial frontal head trauma: bilateral indirect optic nerve injury.

A 10-year-old girl developed bilateral blindness and partial third nerve paresis immediately following a closed head injury. Bilateral optic atrophy developed subsequently. This is the first report of an association between second and third nerve injuries after minor head trauma in the absence of a preexisting lesion. The pathophysiology of indirect injury to the optic nerve under these circumstances is uncertain, but the lesions in this patient seemed to be due to ischemia.

Bilateral paroxysmal positioning nystagmus.

The term "bilateral paroxysmal positioning nystagmus" (BPPN) refers to the occurrence of paroxysmal positioning nystagmus when the patient's head is placed in both right and left head-hanging positions. BPPN was found in at least 15% of 114 patients with benign positional vertigo. Head injury, central nervous system disease or both were causative in over half of patients with the finding. In some instances, paroxysmal positional nystagmus may be found when the abnormal ear (side) is uppermost rather than directed toward the floor; this fact may have important implications if singular neurectom

Traumatic hypopituitarism associated with bitemporal hemianopia in a prepuberal child.

The case of a boy affected by post-traumatic hypopituitarism associated with bitemporal hemianopia is described. While traumatic bitemporal hemianopia is not an extraordinary occurence, traumatic injuries have been rarely reported among the causes of hypopituitarism. This may be because the cranial trauma in most of the cases is too severe to permit survival. After having considered the diagnostic problems the authors briefly review the peculiar vascular supply to the chiasma and anterior diencephalon and try to consider the pathogenetic mechanism of the syndrome. Direct contusion necrosis and

Facial nerve injury secondary to lateral displacement of the mandibular ramus.

We describe two cases of facial nerve injury after lateral external dislocation of the mandibular ramus, an event not found in the reviewed literature. We discuss the possible pathogenesis and outline the therapy.

[Extra-dural hematoma of the posterior fossa (author's transl)].

10 cases of posterior fossa extra-dural hematoma are reported and 100 other published cases reviewed. It appears that the sole clinical evaluation frequently leads to wrong or delayed diagnosis. In only one out of five cases the cerebellar signs and the palsy of one or several cranial nerves (VII, IX, X, XI, XII) are prominent. In contrast, all other cases do not present specific signs. Furthermore, the presence of a concomitant supratentorial traumatic lesion may contribute to prevent the finding of evidences for cerebellar extra-dural hematoma. The possibility for such hematoma must always b

[Extrusion into the inferior dental canal of filling material. Apropos of 18 cases].

The purpose of this article is to draw the reader's attention to a fairly frequent complication in root treatments with suspicious clinical symptoms and in which painful complications are particularly to be feared. This complication appears therefore to be a stomatological emergency, treatment for which is aimed at bringing about regeneration of the traumatised nerve, prognosis being dependent on prompt surgical measures, the site of the complication and the anatomical damage done to the lower dental nerve.

Immediate visual deterioration after attempts at radical excision of pituitary adenomas.

Thirty cases of pituitary tumour have been operated on in the last five years by a transfrontal approach, using microsurgical technique. Three cases of postoperative blindness are described, with an analysis of possible contributory factors and causative mechanisms, prognosis, and measures to lessen the risk of this complication. The possibility of an impairment in the chiasma blood supply is discussed. Although the use of the microscope makes the removal of the so-called capsule an easier surgical task, it is recommended that this structure should be left undisturbed.
