The laryngotracheal airway is the bottleneck of the respiratory system. Whereas disease in the nose or in the bronchial airways will affect respiratory airflow in a limited way, laryngotracheal disease must always be considered potentially life threatening. The nasal airway is the normal access to the lungs. As such, it serves vital functions in modifying the condition of ambient air before its access to the lungs. Impairment of the efficacy of nasal function in that regard will inevitably cast an unaccustomed burden on the lower airways and may possibly play a role in the long-term developmen

Traumatic lesions of the chiasma are few when one considers its anatomical position. They happen after severe direct or indirect traumas with or without skull fractures, and with or without macroscopic lesions. Microscopic lesions are characterised by contusion. Clinically the visual loss goes together with bitemporal defects and with bilateral hemorrhagic papillar oedema. Early complications are hematoma of the optochiasmatic space and perichiasmatic meningitis: among delayed complications optochiasmatic arachnoiditis prevails.

From the otolaryngologist's point of view, there are multiple causes for the frequent symptom of facial and head pain: headaches due to ear diseases: pain extending to the ear region, with special regard to "referred otalgia" involving the cranail nerves V, IX, X; facial pain due to temporomandibular dysfunction; rhinological causes of facial and head pain, including posttraumatic trigeminal neuralgia and "facial sympathalgies"; the syndrome of the elongated styloid process. The diagnosis and therapy of the "typical" ENT diseases is not described in detail since the paper deals mainly with les

Glossopharyngeal neuralgia, asystole, and seizures occurred in a patient with an internal carotid occlusion and external carotid stenosis. Swallowing was the triggering mechanism for these events. Mechanical stimulation of the pharynx failed to reproduce the symptoms. An ischemic injury to the glossopharyngeal nerve in the region of the jugular foramen that resulted in an artificial synapse is the proposed etiology.

Postoperative improvement occurred as a result of transsphenoidal chiasmapexy in a patient with posthypophysectomy visual loss. Traction injury of the optic chiasm may have been caused by a deficient diaphragma sellae and inadequate packing and repair of the sella floor. A cartilaginous seal is recommended.

An analysis of the early postoperative course after tumour removal from the chiasmal-diencephalic area in 156 children aged 2 to 16 years is presented. Acute diencephalic disorders developed after the operation in half the children: in 53--as single diencephalic disorders and in 25--as an acute diencephalic syndrome which was more hazardous. To prevent acute diencephalic disorders glucocorticoid hormones were administered to the patients before and 7 days after the operation; mild hypothermia was applied during the operation and superficial (in some patients) during first three postoperative d

Patients in the early stage of severe head injuries occasionally present with a clinical picture analogous to that in vascular or neoplastic pontine lesions and they can be identified by careful clinical examination and by repeated electroencephalography (E.E.G.). The commonest features in these cases were preservation of consciousness, constriction of the pupils, disturbances of reflex ocular motility, severe pyramidal deficits, decerebrate rigidity, myoclonic manifestations, irregular breathing, and cranial nerve palsies. An E.E.G. tracing similar to the normal pattern was a common finding.

Dysarthria and aphasia represent primary symptoms of cerebral nerve dysfunction, and may particularly involve the facial and the hypoglossal nerves. In order for management to be effective, careful examination of the muscular functions of the oral cavity is required, with particular emphasis placed on the movements of the tongue and velum. Such evaluation, which utilizes both neuropsychologic and linguistic determinations, permit an accurate differentiation of the possible causes of aphasia to be made. During the evaluation, both verbal and non-verbal responses by the patient are included for

This study was conducted specifically to determine and document the various possible causes of postoperative aspiration. Such information may assist in better selection of candidates as well as provide insights that facilitate improvement of operative techniques for conservation surgery for cancer of the larynx. The major significant information derived from this study is the recognition and delineation of the part played by paralysis of the superior and recurrent laryngeal nerves as causes for aspiration following conservation laryngeal surgery.

The primary objective of this paper is to suggest various precautionary measures in order to decrease the occurrence of possible complications associated with the orbital bead implant operation for anophthalmic "enophthalmos". It is important that the subperiosteal insertion of glass beads by used primarily in an anophthalmic orbit. A preoperative x-ray of the patient's orbit will help the surgeon decide the ideal location in making entry into the subperiosteal space. Cautious dissection of the subperiosteal pocket is important in order to avoid damage to the levator muscle, blood vessels and

Although fractures of the condylar neck belong to the most frequent injuries of the jaws, and various nerves are found in close proximity to the temporomandibular joints, only little mention is made in the literature of neurological complications arising from accidents. We have therefore carried out a prospective study on nerve injuries in 237 fractures of the articular process in the period 1971-1975. We found two cases of post-traumatic Frey's syndrome, whereby the syndrome developed from a post-traumatic auriculotemporal neuralgia. We also found loss of function of auriculotemporal buccal a

The author reports 12 cases which illustrate unusual aspects of colo esophagoplasty in post-caustic esophageal stenosis. 8 cases respresented post-caustic pharyngolaryngeal stenosis in which he recommended anastomosis of the colon with the oropharynx according to a personal technical variant. In 1 case associated with amputation of the epiglottis, he carried out a double pharyngo-colic Y-shaped anastomosis. He classified the pharyngotomy types in relation to the 3 main nerves, the lingual, hypoglossal and superior laryngeal nerves. In 3 cases the author carried out successfully retrosternal tr

At the McGill University neuro-ophthalmology unit, 52 patients with superior oblique palsy were seen during the 2-year period October 1973 to August 1975; these included patients with congenital, traumatic, vascular, and other more rare causes of trochlear paralysis. Half of the patients with congenital palsy had diplopia, requiring treatment, although surgery was rarely necessary. Patients with palsy due to trauma, the major cause in our series, included four with bilateral involvement. With the exception of recovery in a single muscle of one patient in this group with traumatic palsy, no spo

A previously reported experimental study demonstrated that the intermediate (partly abducted) position of a paralysed vocal cord may be due to physiological inactivation, rather than paralysis, of the cricothyroid muscle. This inhibition was shown to be caused by interruption of vagal afferent impulses originating in pulmonary pressure receptors. The case reported here offered an unusual opportunity of studying this condition by serial section of a human larynx. A patient's left vocal cord was paralysed by cancer in the left pulmonary hilum and apex. There was aspiration and loss of voice. The
