The value of the radiobiological diagnosis in establishing the localization of a facial palsy resulting from temporal bone fractures has been investigated in 49 cases. Radiological confirmation was found in 21 out of 37 cases of longitudinal fracture and in 11 out of 12 patients (90%) with a transverse fracture. The fracture line was seen by polytomography alone in 1 out of 21 (5%) longitudinal fractures and in 5 out of 11 (45%) transverse fractures. A precise readiological localization of the facial nerve lesion was only possible if the fracutre line was running across the long axis of the py

After a brief statistical survey the anotomic basis is explained in a broad outline. In the description of a case with bilateral paralysis of the abducens nerve, the suffering of the patient in his working life is examined and the phenomenon of inacurrate depth projection, of "pointing past", is discussed. As a result the suitability for employment is more highly reduced for those suffering from bilateral paralysis of the abducens nerve, as compared to persons with one eye. During the report a 16 mm Color film was shown, showing the patient before and after the O'Connor operation. The possibil

The examinations of gaze and optokinetic nystagmus were porformed at bed side before and after craniotomy. Gaze nystagmus were observed regularly in 120 cases who got the intracranial surgical procedures in Nagoya university hospital. Postoperative nystagmus which were not seen before operation appeared in 16 cases of 91 patients with supratentorial lesions and 24 cases of 27 patients with infratentorial lesions. These postoperative nystagmus continued for from 3 days to 21 days. The state of the direction of spontaneous horizontal nystagmus and directional preponderance were examined on 23 ca

Two cases are described in which, after successful removal of a cerebellar haemangioblastoma followed by several years of freedom from symptoms, there developed a progressive spinal cord compression, leading to death. At necropsy the spinal cords in both cases and the brainstem in one case, were irregularly plastered with haemangioblastoma. Although there was no doubt that malignant spread had occurred from one or more primary tumours, the histology of the tumour tissue was in no way different from that of conventional haemangioblastoma.

Severe epistaxis following head injury occur from damage to the anterior ethmoidal or sphenopalatine arteries. However, the more massive, life-threatening posttraumatic epistaxis is that arising from ruptured aneurysm, arteriovenous fistula, or tear of the intracranial extradural portion of the internal carotid artery. The authors had opportunities to treat successfully 3 cases of massive delayed epistaxis from the aneurysm of this site following closed head injury. Case 1. A 23-year-old man was injured in a motorcycle accident on April 19, 1968 and taken to an emergency hospital, where the fi

Seven cases of adhesive arachnoiditis of the optochiasmatic cistern, verified surgically, with a history of head injury are presented. Neuroophthalmologic findings consisted of impairment of visual acuity and funduscopic atrophy rather than papilledema. The visual fields showed a damage of the periaxial rather than the axial type. Neuroradiologic examinations did not consistently give the findings considered classical of optochiasmatic arachnoiditis. Surgical lysis of the adhesions has given an improvement of the visual function, or at least an arrest in its impairment, in almost all cases. It

After a discussion of the pathomechanism of skull trauma by a blow leading to chiasmal lesion, frontobasal injuries of the skull are considered; it is shown that typical defects of the visual field resulting from chiasmal damage can be revealed not only in cases of serious industrial or traffic accidents with brain concussion and skull base fracture, but also following minor injuries without unconsciousness and radiologically visible bone fractures.

Three cases of unusual trismus are reported, occurring in patients with lesions of the central nervous system. On the basis of the clinical and electromyographical findings and observation of the course of the trismus this is interpreted as a symptom of a lesion of the brain stem, causing a dis-synergism of the masticatory muscles.
