[Frontobasal cranio-encephalic injuries].

The authors deal with the results accomplished by surgical procedure on 10 cases of fronto-basal trauma. X-Rays taken from the skull disclosed an involvement of bone structures of the fronto-basal region in all cases examined. The most outstanding clinical findings in these cases were: lesion of the olfactory nerve (7cases), bacterial meningitis (3 cases), cerebrospinal fluid cranio-nasal fistula (3 cases). The surgical technic was accomplished according to the procedure devised by Dietz and consisted of plastics of the anterior floor of the skull accompanied by galea-periosteal junction taken

[Chiasma injury following skull trauma (author's transl)].

After a discussion of the pathomechanism of frontobasal cranial injury with a consecutive chiasmal lesion and diabetes insipidus due to a fracture of the basis of the skull, a personal observation of bitemporal hemianopia with a transient diabetes insipidus after a frontal motorcycle crash is recorded. A short survey of the literature shows the controversial opinions on traumatogenic lesions of the chiasma, which may be injured directly or the bitemporal hemianopia is due to a posttraumatic vascular disturbance. For the case described by the author primary tearing of the chiasma due to sudden

Carotid-cavernous fistula: a complication of maxillofacial trauma.

Fistulas of the internal carotid-cavernous sinus are an unusual arteriovenous malformation. Two types exist: (1) spontaneous fistulas and (2) traumatic fistulas. This paper deals with the traumatic variety which is seen more commonly in men as a direct result of severe maxillofacial injury. Recent anatomic studies are correlated with the physical findings associated with carotid-cavernous fistula, ie, headache, chemosis, exophthalmos, pulsatile bruit, and multiple cranial nerve paralyses. A case report illustrates these findings. The differential diagnosis which varies from tumors, to cavernou

[Chiasma lesions in sport accidents (author's transl)].

With reference to a chiasma contusion, proved at autopsy, sustained by a football player after a temporal impression fracture due to contact of a knee with his skull, the pathological mechanism causing chiasma injuries in blunt head injuries is explained. Finally, from our own experience we report on 2 water-sports accidents sustained by young men (a jump from the trampoline, and a fall during water-skiing) where chiasma-lesions were diagnosed from typical field defects.

[The neurologic complications of Paget's disease].

There are several neurological complications of Paget's disease. They may concern the medulla, the cauda equina, and also the cerebral trunk, the cranial nerves, and the brain. Complications are rare when the great frequency of Paget's disease is considered. But they should be recognized because they constitute an aspect of the disease the treatment of which is usually well tolerated of even negligible. The signs of involvement of the medulla, of the cauda equina, and those which complicate the basilar impressions usually develop in a progressive and not very spectacular fashion. Nevertheless,

Neurogenic concept of bell's palsy. Medical decompression of facial nerve.

According to the author it is the 'transmyringal' portion of the chorda tympani nerve which in practice remains exposed to the cold atmospheric air and triggers the pathologic process involved in Bell's palsy, affecting the facial nerve secondarily, rather than a primary condition of the facial nerve. This theory was proved by creating a Bell's palsy (paralysis a frigore) with cold-air-stimuli to the tympanic membrane in experimental animals (two monkeys). On the basis of this observation the author recommend's 'medical decompression' of the facial nerve by means of infra-red fomentation of th

A comparison of methods of extra temporal facial nerve repair.

The development during the past 100 years of methods of repair of the divided facial nerve is discussed, from the early attempts to bring the nerve ends together inside a tube of vein. The success of the repair is enhanced by the care of the blood supply to the nerve and by measures to avoid scar developing between the ends of the nerve. Rest of the suture line is aided by the support of some form of tube, but unless this latter is kept below I cm. in length interference with the radial blood vessels to the nerve occurs. Intubation also encourages parallel growth of the nerve fibres and discou

Electrophysiological responses from the cochlea and inferior colliculus in guinea pigs after head injury.

Electrical responses from the inferior colliculus (ER) and round window (CP) to acoustical stimuli were compared in control and experimental guinea pigs which had received a single blow to the freely movable head. After the blow to the head, all of the animals showed signs of commotio cerebri, and some of them died. The experimental animals were divided into two groups so that the physiologic responses to sound could be evaluated one day (early group) and seven days (intermediate group) following the head blow. The animals of the early group had elevated ER thresholds over the entire frequency

Truamatic ophthalmoplegia: a complication of fractures of the zygomaticomaxillary complex. Review of etiology with report of an unusual case.

Ocular muscle imbalance is a common complication in fractures of the zygomaticomaxillary complex. Involvement can be direct; caused by entrapment of either the muscle or tendon in the fracture; or indirect, produced by central or peripheral damage to the cranial nerves supplying the muscles, massive orbital edema or hemorrhage. A case of complete paralysis of the extraocular muscles caused by a critical reduction of the size of the orbit as a result of fracture of the zygomaticomaxillary complex is described.

A new technique to correct facial paralysis.

In two cases of unilateral ficial paralysis, the advanced atrophy of the muscles forced us to try a new technique for dynamin reanimation. To our knowledge, this has not yet been reported in the literaturel. The technique combines a free nerve graft across the face with a free muscle graft or a muscle flap. The result in both our cases was good.

Postfacial palsy phenomena: faulty nerve regeneration or ephaptic transmission?

Certain postfacial palsy phenomena--'facial contracture', 'crocodile tears', etc.--have been examined in relation to faulty misrouting of the nerve fibers, which thus far has been offered as an explanation for such phenomena. Analogy is made with the problem of hemifacial spasm, occurring without antecedant facial palsy. It is seen that the two conditions have many things in common and are amenable to similar methods of therapy--such as surgical decompression of the facial nerve without interrupting its fibers or by pharmacological blocking of the facial nerve. Evidence is presented to support

[Surgery of injuries of the facial nerve on the lateral region of the face (author's transl)].

Injuries of the lateral facial region frequently cause a partial or complete destruction of the trunk or individual branches of the facial nerve. Operative methods are presented, which have proved to be especially successful for reconstruction of the facial nerve. For a successful outcome of the nerve suture together with a good functional result later on, an exact suture tehcnique is mandatory. The procedure is discussed. On the other hand, the indication for operation raises a number of problems, which mainly concern the extent of nerve injury and the period elapsed after the damage.

Percutaneous radiofrequency gangliotomy in the treatment of trigeminal neuralgia and other facial pain.

Thirty-eight patients with facial pain in the trigeminal distribution have been treated by percutaneous coagulation of the trigeminal ganglion. Although use has been made of radiological criteria, it has been found from the physiological data obtained from stimulation that motor and sensory phenomena provide more information of electrode placement and that good pain relief results when these criteria are met. The advantages of the method are: (i) minimal operative risk; (ii) control of the extent of the lesion created; (iii) small sensibility deficit; (iv) minimal complication rate; (v) short

[Indications and results of operative exposure of traumatically damaged optic nerves (author's transl)].

The indications for operative treatment of traumatically damaged optic nerves are given and the results from microsurgical transethmoid approaches are communicated. This operation method has such a low risk, that it can be also used on patients with recent head injuries. The early diagnosis of traumatic optic nerve damage and the earliest possible exposure of the nerve with pressure relief of the traumatised optic nerves are important for possible success. The development of microsurgery of the orbit, the possibility of a low-risk anaesthetic, the use of modern post-operative therapy and obser

Accessory spinal nerve injury.

In 7 cases of peripheral lesion of the spinal accessory nerve 4 were produced by malignancy, two by iatrogenical resection of lymph nodes, one by an en bloc dissection of the neck for arteriovenous malformation. Incapacity following the injury is quite marked and includes weakness of the sternomastoid and trapezius muscles, as well as pain presumably from traction on the brachial plexus. The importance of avoiding the nerve in surgical intervention in the neck must be strongly emphasized. A review of the literature suggests that in fresh injuries the spinal accessory nerve should be explored a
