Auditory-nerve potentials from ear canals of patients with otologic problems.

Summary--The feasibility of making rountine measurements of human auditory-nerve potentials in an office environment is demonstrated. Using a portable device for stimulus generating and response recording, auditory-nerve potentials are recorded from subjects with normal and abnormal hearing by means of an electrode placed on the skin of the ear canal. The results can be compared with those of others obtained under laboratory conditions. Preliminary results indicate that in many instances, the nature of the hearing deficit is related to the latency and size of the click-evoked auditory-nerve po

Palliation in the management of laryngeal cancer. Surgical concepts in palliation of advanced disease.

Every science has limits to its operation, including medical science concerning malignancy. Beyond a certain stage of a disease, palliation is the only recourse. The word "palliation" amounts to an acceptance of defeat by a clinician while trying to salvage his cancer patients. It means that the patient should breathe and eat without much pain till death takes pity on him. Palliative surgery in laryngeal cancer amounts to doing tracheostomies and gastrostomies and administering painkillers. Most of my cases belong to this category. I extended the accepted parameters of surgical excisions for p

Evaluation of unilateral exophthalmos with ultrasonography: analysis of 258 consecutive cases.

Ultrasound examination has emerged as a valuable technique for defining soft tissue abnormalities of the eye and orbit. As high frequency sound waves (5 to 20 megahertz) are projected through soft tissues, echoes are produced at tissue interfaces. These echoes are displayed on oscilloscopes as either one-dimensional amplitude spikes (A-mode ultrasonography), or as dots integrated into a two-dimensional image representing a thin section through the entire orbit (B-scan ultrasonography). A-mode indicates tissue substance, while B-scan illustrates tissue morphology. With this technique, orbital t

[The management of intratemporal facial nerve lesions (author's transl)].

Intratemporal lesions of the facial nerve can be divided into operative lesions, lesions due to birth trauma, trauma through the external auditory canal, gunshot injuries, lesions caused by fractures of the temporal bone and lesions due to intratemporal tumours. Bell's palsy and palsies due to inflammation are not included in this paper. The lesions caused by fractures are discussed in more detail. In longitudinal fractures the nerve is always damaged at the site of the geniculate ganglion and not at the pyramidal segment, as accepted until now. Of the lesions caused by tumours the neurinomas

Radiofrequency percutaneous Gasserian ganglion lesions. Results in 140 patients with trigeminal pain.

Percutaneous radiofrequency ablation of the Gasserian ganglion or posterior root, or both, was performed in 140 patients. Of the 135 patients with trigeminal neuralgia, satisfactory analgesia was achieved in 121. Postoperative complications included unintentional first-division analgesia (10), transient sixth-nerve palsy (1), neuroparalytic keratitis (2), and anesthesia dolorosa (2). The phenomenon of facial blush may be helpful in avoiding unwanted first-division analgesia. In four of five patients with other forms of neuralgia, the procedure did not relieve pain; the fifth patient experience

[Bilateral traumatic paralysis of the abducent nerves].

Two cases of bilateral traumatic abducens palsy are reported. The onset of the ocular symptomatology, several hours after the head injury, suggests a possible involvement of the branches of the meningeal dorsal artery supplying the extradural trait of the 6th cranial nerve.

Mechanisms of reflex cardiac arrest in tetraplegic patients.

Four patients with physiologically complete high cervical spinal-cord lesions, sustained within the previous 6 weeks, were observed. All needed intermittent positive-pressure ventilation. In the stage of spinal shock, stimuli to the trachea induced bradycardia, and in two patients cardiac arrest resulted. The bradycardia occurred when the patients were hypoxic, and seemed to be due to a vaso-vagal reflex. Normally this reflex is opposed by sympathetic activity, and during hypoxia by increased pulmonary (inflation) vagal reflex activity due to increased breathing. In these patients, however, co

Optic nerve anomalies in basal encephalocele.

Basal encephalocele should be suspected in child when hypertelorism, defect of the bridge of the nose, and other midfacial anomalies are present. Optic nerve anomalies may occur with basal encephalocele. Those previously described are pale discs, coloboma of the optic nerve head, and pit of the optic disc. Two additional disc anomalies that are associated with basal encephalocele are megalopapilla and optic nerve dysplasia. A mass in the nose or nasopharynx under these circumstances is most likely an encephalocele and biopsy is contraindicated.

[Functional survival of the human Corti organ after dissection of the eight nerve (author's transl)].

The behaviour of the functional unit haircell - peripheral neurit before and after the dissection of the cochlear nerve in two cases of cerebello-pontine angle tumors is described. An immediate alteration does not seem to be found when the blood supply via internal acoustic meatus is not severed. The survival time after the dissection of the acoustic nerve is somewhat more than seven weeks. This is longer than related morphological findings indicate in the cat where the morphological survival time of fibre population I (95% of all fibres) is not more than four weeks. On the other hand, morphol

Compications in acoustic neuroma surgery.

The findings in a series of 80 cerebellopontine angle tumors are presented. Complications and results are covered in detail. A new method of controlling postoperative cerebrospinal fluid (CSF) leak is presented. Care of the eye in postoperative facial paralysis is emphasized, and a new type of moisture chamber is introduced.

Late sequelae of radiation therapy in cancer of the head and neck with particular reference to the nasopharynx.

Sequlae of radiation therapy may be late in occurring and varied in their manifestations. Although some are untreatable and progressive, the risk of development of some other sequelae can be minimized by careful application of radiotherapy or by ancillary measures, such as dental decay prophylaxis. Some of the serious sequelae secondary to radiation therapy of the nasopharynx have been summarized. These include radiation myelitis, paralysis of the cranial nerves, stricture of the pharynx, radiation-induced cancer, and necrosis with fatal hemorrhage.

Occult schwannomas of the vestibular nerve.

Five small occult schwannomas of the vestibular nerve were discovered on routine examination of 893 serially sectioned temporal bones of 517 individuals. Three arose from the superior division, one from the inferior division, and the other had a multicentric origin from both divisions of the nerve. Location and size of these tumors indicate that clinical diagnosis would have been difficult or impossible by any method of study. The finding of vestibular schwannomas in 0.9% of individuals in this series, indicates the high incidence of this tumor in the general population. The much lower inciden

An evoked electromyographic test for peripheral facial palsy.

The purpose of the present study was to determine the degree of nerve degeneration in patients with facial palsy from the number of spikes in M-wave which were induced from the facial muscle by means of a bipolar concentric needle electrode when the facial nerve was stimulated. Ten normal cases between 14 and 72 years of age and 46 cases with facial palsy between 12 and 73 years of age were examined. Histopathological findings of ten denervated nerves were studied to determine to what extent this method may reflect the state of nerve degeneration.
