Scanning laser polarimetry reveals status of RNFL integrity in eyes with optic nerve head swelling by OCT.

PURPOSE: Optical coherence tomography (OCT) shows retinal nerve fiber layer (RNFL) thickening in optic nerve head (ONH) swelling, but does not provide information on acute axonal disruption. It was hypothesized that scanning laser polarimetry (SLP) compared with OCT might reveal the status of axon integrity and visual prognosis in acute RNFL swelling. METHODS: Threshold perimetry, OCT, and SLP were used to prospectively study eyes with papilledema (24), optic neuritis (14), nonarteritic anterior ischemic optic neuropathy (NAION) (21), and ONH swelling (average RNFL value by OCT was above the

Risk assessment of inferior alveolar neurovascular bundle by multidetector computed tomography in extractions of third molars.

PURPOSE: This study aimed to assess the reliability of multidetector computed tomography (MDCT) in determining the surgical risk of the inferior alveolar neurovascular bundle in extractions of third molars. METHODS: The sample comprised thirty-three individuals (63 third molars) who underwent preoperative evaluation by MDCT before extraction of impacted mandibular third molars. MDCT was used to determine the relationship between the roots of the third molars and the mandibular canal, and the course of the mandibular canal. Inferior alveolar nerve (IAN) exposure and the presence of hemorrhage

An animal model for trigeminal neuralgia by compression of the trigeminal nerve root.

BACKGROUND: Microvascular compression of the trigeminal nerve root is a major cause of most trigeminal neuralgia (TN) in patients; however, no reliable animal model to further study the pathogenesis of TN currently exists. OBJECTIVE: Our objective was to establish a novel and practical animal model for TN by chronic compression of the trigeminal (CCT) nerve root in rats, which would provide a better animal model to mimic the clinical feature of TN on the research of the pathogenesis of TN. STUDY DESIGN: A randomized, double blind, controlled animal trial. METHODS: Sixteen adult male Sprague

Coronectomy: a surgical option for impacted third molars in close proximity to the inferior alveolar nerve.

BACKGROUND: Damage to the inferior alveolar nerve (IAN) during the extraction of impacted mandibular third molars in close proximity to the mandibular canal is a postoperative complication that most frequently occurs in patients 40 years and older. METHODS: The authors evaluated the postoperative complications of 43 coronectomies of impacted mandibular third molars in 37 patients (17 men and 20 women, mean age [standard deviation] 31 [2] years). The authors used cone-beam computed tomographic images to determine that all of the teeth that underwent a coronectomy were in close proximity to the

Schwannoma originating from lower cranial nerves: report of 4 cases.

Four cases of schwannoma originating from the lower cranial nerves are presented. Case 1 is a schwannoma of the vagus nerve in the parapharyngeal space. The operation was performed by the transcervical approach. Although the tumor capsule was not dissected from the vagus nerve, hoarseness and dysphagia happened transiently after the operation. Case 2 is a schwannoma in the jugular foramen. The operation was performed by the infralabyrinthine approach. Although only the intracapsular tumor was enucleated, facial palsy, hoarseness, dysphagia and paresis of the deltoid muscle occurred transiently

Traumatic chiasmal syndrome.

A case of closed head trauma, harbouring bitemporal hemianopsia is presented. The MRI evaluation describes chiasmal contusion as the cause behind visual deficits. Chiasmal injury may occur even in the absence bony chip impingement and features of hypo-pituitarism. Such a situation warrants MRI evaluation of optic pathways.

Preauricular transzygomatic anterior infratemporal fossa approach for tumors in or around infratemporal fossa lesions.

Various surgical approaches to the infratemporal fossa (ITF) have been reported. Among them, the preauricular transzygomatic anterior ITF approach (anterior ITF approach) has been used for exposure of the antero-superior part of the ITF. The purpose of this article is to show anatomical dissections using the anterior ITF approach and to evaluate our surgical experience using this approach. An anatomical study of the anterior ITF approach was performed using six sides of three cadaveric heads. Clinical course was retrospectively reviewed for 34 patients who underwent microsurgical resection of

Quantitative retinal protein analysis after optic nerve transection reveals a neuroprotective role for hepatoma-derived growth factor on injured retinal ganglion cells.

PURPOSE: Retinal ganglion cell (RGC) degeneration is an important cause of visual impairment and can be modeled by optic nerve transection, which causes the death of 90% of RGCs within 14 days postaxotomy. We performed a proteomic study to identify and quantify proteins in the rat retina after optic nerve transection. Our goal was to isolate potential targets for therapeutic intervention to prevent RGC degeneration. METHODS: iTRAQ proteomics was used to analyze adult rat retinas at 1, 3, 4, 7, 14, and 21 days postaxotomy. Hepatoma-derived growth factor (HDGF), a target identified by iTRAQ, wa

An uncommon clinical feature of IAN injury after third molar removal: a delayed paresthesia case series and literature review.

After an inferior alveolar nerve (IAN) injury, the onset of altered sensation usually begins immediately after surgery. However, it sometimes begins after several days, which is referred to as delayed paresthesia. The authors considered three different etiologies that likely produce inflammation along the nerve trunk and cause delayed paresthesia: compression of the clot, fibrous reorganization of the clot, and nerve trauma caused by bone fragments during clot organization. The aim of this article was to evaluate the etiology of IAN delayed paresthesia, analyze the literature, present a case s

A case of bilateral lower cranial nerve palsies after base of skull trauma with complex management issues: case report and review of the literature.

INTRODUCTION: Fractures of the skull base can cause lower cranial nerve palsies because of involvement of the nerves as they traverse the skull. A variety of syndromes have been described, often involving multiple nerves. These are most commonly unilateral, and only a handful of cases of bilateral cranial nerve involvement have been reported. CASE REPORT: We describe a 64-year-old man with occipital condylar fracture complicated by bilateral palsies of IX and X nerves associated with dramatic physiological derangement causing severe management challenges. Apart from debilitating postural hypo

Gustatory changes in patients with chronic otitis media, before and after middle-ear surgery.

OBJECTIVE: We investigated gustatory changes in patients with chronic otitis media, before and after middle-ear surgery. METHODS: This prospective study included 38 patients with unilateral chronic otitis media. We used taste testing solutions to evaluate each patient's taste function. Intra-operative assessments of the chorda tympani nerve were also compared and analysed. RESULTS: Patients with chronic otitis media had significantly worse ipsilateral perception of sour, bitter and salty tastes. In patients with good intra-operative preservation of the chorda tympani nerve, there was signifi

Early extraction: a silver bullet to avoid nerve injury in lower third molar removal?

This retrospective study evaluated the effects of early extraction of immature lower third molar on preventing complications, particularly nerve injury following lower third molar removal. Patients were grouped according to age and radiographic results: group A (518 patients, ≤23 years, immature teeth with apical foramen not closed); group B (532 patients, >23 years, mature teeth with closed apical foramen). Group A included 230 males and 288 females (average age 17 years). In group A, 808 lower mandibular third molars were extracted bilaterally in 290 and unilaterally in 228 patients; the inc

Coronectomy vs. total removal for third molar extraction: a systematic review.

The objective of this systematic review was to compare the outcomes between coronectomy and total removal for third molar extractions with high risk of nerve injury and to help practitioners make prudent decisions on whether and how third molars should be removed. PubMed, Embase, Web of Science, CENTRAL, and SIGLE were searched from January 1990 to October 2011 for randomized or non-randomized controlled trials. Four studies met our inclusion criteria. The pooled risk ratio (coronectomy vs. total removal) was 0.11 (95% CI = 0.03-0.36), 1.03 (95% CI = 0.54-1.98), 0.55 (95% CI = 0.28-1.05), and

Coronectomy with the diagnostic use of CBCT to aid in the prevention of iatrogenic damage to the inferior alveolar nerve.

This case report describes the treatment of a 27-year old man who presented to the oral and maxillofacial surgery division of a U.S. Navy recruit training center. Cone beam computed tomography determined the true three-dimensional relationship of the impacted teeth to the inferior alveolar neurovascular bundle. The tomograms aided in the surgical planning to decrease the probability of iatrogenic postoperative nerve disturbance.

Sihler-stain study of buccal nerve distribution and its clinical implications.

OBJECTIVE: The aim of this study was to elucidate the precise distribution of the buccal nerve (BN) and its anatomic relationship with the infraorbital nerve (ION) and mental nerve (MN). STUDY DESIGN: Eight human specimens were subjected to Sihler staining, which is a highly accurate method for visualizing the distribution of nerve fibers without alteration of the nerve. RESULTS: It was found that the BN mainly proceeded medially from its point of entrance near the parotid duct opening to the angular area of mouth, giving off tiny branches along its trajectory. Some of these branches were di
