The zygomaticotemporal nerve and its relevance to neurosurgery.

BACKGROUND: Although neurosurgical procedures are frequently performed in its territory, the zygomaticotemporal nerve (ZTN) is rarely mentioned in this literature, even though this nerve has been implicated in postsurgical pain syndromes and may become entrapped, resulting in chronic headache. The present study was performed to further elucidate the anatomy of the ZTN. METHODS: Twelve cadavers (24 sides) underwent dissection of the lateral temporal region to analyze the course, relationships, and landmarks for the ZTN. RESULTS: A ZTN was found on all but 1 left side. This nerve left the late

The stability of mandibular prognathism corrected by bilateral sagittal split osteotomies: a comparison of bi-cortical osteosynthesis and mono-cortical osteosynthesis.

This study evaluated the differences in surgical changes and post-surgical changes between bi-cortical and mono-cortical osteosynthesis (MCO) in the correction of skeletal Class III malocclusion with bilateral sagittal split osteotomies (BSSOs). Twenty-five patients had bi-cortical osteosynthesis (BCO), 32 patients had mono-cortical fixation. Lateral and postero-anterior cephalometric radiographs, taken at the time of surgery, before surgery, 1 month after surgery, and on completion of orthodontic treatment (mean 9.9 months after surgery), were obtained for evaluation. Cephalometric analysis a

Motor and sensory branching of the recurrent laryngeal nerve in thyroid surgery.

INTRODUCTION: Recognition of extralaryngeal bifurcation of the recurrent laryngeal nerve (RLN) is crucial, because inadvertent intraoperative division may lead to significant morbidity. The purpose of this study was to examine the incidence of extralaryngeal bifurcation of the RLN and the distance that the initial bifurcation occurs from the cricothyroid insertion site of the RLN. We also sought to demonstrate the location of the RLN branches containing a predominance of motor fibers. METHODS: This prospective study of 220 patients with data on 310 RLNs collected the type of operation, incide

Neurophysiologic intraoperative monitoring of the vestibulocochlear nerve.

Neurosurgical procedures involving the skull base and structures within can pose a significant risk of damage to the brain stem and cranial nerves. This can have life-threatening consequences and/or result in devastating neurologic deficits. Over the past decade, intraoperative neurophysiology has significantly evolved and currently offers a great tool for live monitoring of the integrity of nervous structures. Thus, dysfunction can be identified early and prompt modification of the surgical management or operating conditions, leads to avoidance of permanent structural damage.Along these lines

The role of interventional neuroradiology in the management of skull base tumours and related surgical complications.

The management of hypervascular skull base tumours is complex and requires a multidisciplinary approach. Skull base surgery may be challenging because of the risk of serious intra-operative bleeding and of potential injuries to lower cranial nerves and/or large cervical vessels. Over the last four decades, advances in neuro-interventional procedures have produced a range of adjunctive endovascular techniques in addition to conventional surgery. Digital subtraction angiography (DSA) allows for a better understanding of tumour vascularisation and its relationship with Surrounding vessels. Tumour

Stereotactic radiosurgery for AMD: a Monte Carlo-based assessment of patient-specific tissue doses.

PURPOSE: To define the radiation doses to nontargeted ocular and adnexal tissues with Monte-Carlo simulation using a stereotactic low-voltage x-ray irradiation system for the treatment of wet age-related macular degeneration. METHODS: Thirty-two right/left eye models were created from three-dimensional reconstructions of 1-mm computed tomography images of the head and orbital region. The resultant geometric models were voxelized and imported to the MCNPX 2.5.0 radiation transport code for Monte Carlo-based simulations of AMD treatment. Clinically, treatment is delivered noninvasively by three

Somatosensory evoked potential monitoring during endoscopic endonasal approach to skull base surgery: analysis of observed changes.

BACKGROUND: Intraoperative neurophysiological monitoring, including upper- and lower-extremity somatosensory evoked potentials (SSEPs), has been used to identify and prevent injury to neurovascular structures during conventional skull base surgery. The expanded endonasal approach (EEA) is a novel minimally invasive approach to skull base surgery. However, it carries the risk of injury to neurovascular structures, including the internal carotid artery, anterior cerebral artery, and cranial nerves. OBJECTIVE: To evaluate the value of SSEP monitoring to predict and/or prevent neurovascular defic
