A number of recent studies have shown the effectiveness of tubulation, using neural progenitor cells or Schwann cells, for promoting nerve regeneration. However, the use of neural cells from other neural donor tissues has potentially serious clinical complications. Therefore, we focused on dental pulp as a new cell source for use in such artificial conditions. Previously, we showed that silicone tubes filled with dental pulp cells (DPCs) promoted facial nerve regeneration in rats. However, the use of silicone tubes requires a secondary removal operation because they may give rise to chronic in

Transverse myelitis (TM) may result in permanent neurologic dysfunction. Nerve transfers have been developed to restore function after peripheral nerve injury. Here, we present a case report of a child with permanent right upper extremity weakness due to TM that underwent nerve transfers. The following procedures were performed: double fascicle transfer from median nerve and ulnar nerve to the brachialis and biceps branches of the musculocutaneous nerve, spinal accessory to suprascapular nerve, and medial cord to axillary nerve end-to-side neurorraphy. At 22 months, the patient demonstrated ex

This study aimed to evaluate surgical results of vestibular schwannoma in elderly patients using a retrospective study in a Quaternary Neurotology and Skull Base Referral Center setting. The data of 232 elderly patients (above 65 years) operated on for vestibular schwannoma in the period between April 1987 and July 2009 were reviewed. Most patients were operated on via translabyrinthine approach (TLA) using transapical extension for large tumors. Other approaches used were retrosigmoid, middle cranial fossa, and transcochlear. Total tumor removal was achieved in the majority of cases, while pl

OBJECT: In large vestibular schwannoma (VS), microsurgery is the main treatment option, and complete resection is considered the primary goal. However, previous studies have documented suboptimal facial nerve outcomes in patients who undergo complete resection of large VSs. Subtotal resection is likely to reduce the risk of facial nerve injury but increases the risk of lesion regrowth. Gamma Knife surgery (GKS) can be performed to achieve long-term growth control of residual VS after incomplete resection. In this study the authors report on the results in patients treated using planned subtota

BACKGROUND: Voice changes after thyroidectomy are common but not always related to recurrent laryngeal nerve (RLN) injury. We evaluated if RLN neuromonitoring correlated with non-RLN injury-related changes in voice after thyroidectomy.
METHODS: Prospective multi-dimensional voice assessment was conducted on patients undergoing thyroidectomy before, 1-4 weeks, and 6 months postoperatively. Voice outcome (VO) was determined as normal (NormVO) or negative (NegVO) based upon combinations of patient-reported symptoms, videolaryngoscopy, a composite of acoustic measurements, and clinician-perceived

OBJECTIVE: The abducens nerve is frequently injured after head trauma and some investigators have attributed this to its long intracranial course. The present study aimed to elucidate an additional mechanism to explain this phenomenon.
METHODS: Twelve fresh adult cadavers underwent dissection of Dorello canal using standard microsurgical techniques. In addition, traction was applied to the nerve at its entrance into this canal before and after transection of Gruber ligament to observe for movement.
RESULTS: In all specimens, a secondary tunnel (i.e., tube within a tube) was found within Dore

OBJECTIVE: In the setting of known facial nerve sacrifice or injury, patients require precautions to prevent exposure keratitis and the morbidity that follows. One recommended treatment is surgical placement of a gold weight with or without lateral tarsal strip. In patients in whom the facial nerve has been sacrificed, it is unknown whether rehabilitation should be simultaneous or in the perioperative period.
STUDY DESIGN AND SETTING: Case series with chart review of patients who underwent immediate rehabilitation of the eye (gold weight and lateral tarsal strip) following facial nerve resect

Chronic Whiplash-Associated Disorders (WAD) present with frequent pain in the neck, head and shoulder regions but the presence of frequent jaw-face pain is unclear. The aim of the study was to investigate the frequency of jaw-face pain, pain in other regions, and general symptoms in chronic WAD patients. Fifty whiplash-patients and 50 healthy age- and sex-matched controls were examined by questionnaire for pain in the jaw-face, pain in other regions and other symptoms. In contrast to healthy, a majority of the WAD patients (88%) reported frequent pain in the jaw-face, in addition to frequent p

BACKGROUND: Animal tests, retro- and prospective clinical trials in neurosurgical departments have shown a beneficial effect of nimodipine on the preservation and recovery of facial and acoustic nerve function following vestibular schwannoma surgery. Encouraged by these positive results a pilot-study of nimodipine treatment in patients with a peripheral facial nerve (FN) paresis following maxillofacial surgery was performed. The rate and time of FN recovery were analysed and compared with the results in the literature.
METHODS: Thirteen patients (n = 13) suffering from a moderate (1/13) up to

Penetration of the optic apparatus by an anterior communicating artery (ACoA) aneurysm is unique. A 43-year-old woman with a history of visual disturbance due to a previous aneurismal rupture underwent surgical neck clipping for a recurred ACoA aneurysm, which had previously been treated using detachable coils. The operation confirmed that the recurred aneurysm and the packed coils had penetrated the chiasm and the right optic nerve (ON), which was distorted and thinned. The aneurismal neck was clipped and the coils were left in place because damage to the optic apparatus by penetration is usu

There is growing evidence of heterogeneity among responses to bitter stimuli at the peripheral, central and behavioral levels. For instance, the glossopharyngeal (GL) nerve and neurons receiving its projections are more responsive to bitter stimuli than the chorda tympani (CT) nerve, and this is particularly true for some bitter stimuli like PROP & cycloheximide that stimulate the GL to a far greater extent. Given this information, we hypothesized that cutting the GL would have a greater effect on behavioral avoidance of cycloheximide and PROP than quinine and denatonium, which also stimulate

PURPOSE: To summarize our experience and lessons of microvascular decompression surgery for trigeminal neuralgia caused solely by venous compression.
METHODS: Fifteen patients with idiopathic trigeminal neuralgia caused by venous compression only underwent microvascular decompression. The entire course of the trigeminal root was explored thoroughly; and coagulating and cutting techniques were preferred in decompressing the culprit veins. Their clinical features, outcomes and operative complications were analyzed.
RESULTS: The compressing veins included the transverse pontine vein in five cas

This study was undertaken to assess dysgeusia in patients who have undergone middle ear surgery for chronic suppurative otitis media (CSOM) when the surgeon's practice was to cleanly divide the chorda tympani without prior manipulation, if it in any way hindered operative access or the aims of surgery. We wanted to determine whether lower rates of taste disturbance might be obtained by early, clean division of the nerve. We followed 110 patients prospectively and asked them to complete a postoperative dysgeusia questionnaire 1 year after surgery. The "cut nerve" group included 81 patients, and

INTRODUCTION: Following the onset of facial palsy, physiotherapists routinely inspect the inside of the patient's mouth and cheek for complications such as ulceration or trauma. In several patients with complete facial nerve palsy, it was noticed that when the cheek was stretched there was subsequent spasm of the muscles of facial expression. This also occurred in patients whose facial nerve had been transected.
CASE REPORTS: We present four patients in whom this response was demonstrated. We consider the mechanism of this response and its relevance in the management of patients with facial p

INTRODUCTION: Facial myokymias (FM) are continuous, involuntary, undulating movements of the facial muscles associated with spontaneous electromyographic activity, such as fasciculations and myokymic discharges. They may occur in healthy individuals, or be secondary to multiple sclerosis, posterior fossa tumors, or an inflammatory process.
PATIENT AND RESULTS: We describe the case of a 31-year-old man who presented with headache, vomiting, low fever, and disorientation. Cerebrospinal fluid findings included low glucose and high protein content and lymphocyte pleocytosis, with positive culture

OBJECTIVE: Tanzania suffers from a severe lack of health practitioners trained in neurosurgical procedures. To address this problem, we have implemented an initiative, modeled after the experience of other pioneers in international neurosurgery, to establish cost-effective and sustainable neurosurgical care by teaching fundamental neurosurgical skills to local surgeons. In this report we describe our early experience in Northwest Tanzania and discuss the potential for this training model to improve neurosurgical care to a region in need.
METHODS: Between September 2009 and October 2010, three

The neurodegenerative disease glaucoma is characterised by the progressive death of retinal ganglion cells (RGCs) and structural damage to the optic nerve (ON). New insights have been gained into the pathogenesis of glaucoma through the use of rodent models; however, a coherent picture of the early pathology remains elusive. Here, we use a validated, experimentally induced rat glaucoma model to address fundamental issues relating to the spatio-temporal pattern of RGC injury. The earliest indication of RGC damage was accumulation of proteins, transported by orthograde fast axonal transport with
