Post thyroidectomy complications: the Hyderabad experience.

OBJECTIVE: Thyroidectomy is a very common surgical procedure worldwide and is performed by surgeons with varied training. The outcome and complication rates are largely dependent on surgeon's skill and experience, the extent of surgery, indication of surgery and number of thyroid surgeries performed at that particular centre. The objective of this study was to determine the frequency of postoperative complications after thyroid surgery in Hyderabad, Pakistan. STUDY DESIGN: It was a descriptive study and was carried out at 2 private hospitals including a teaching University Hospital over a per

[Facial nerve monitoring in the parotidectomy].

INTRODUCTION: Facial palsy (FP) is a feared complication of parotidectomy. Facial nerve monitoring (FNM) has been poorly evaluated since it was created at the beginning of the 90's. The authors described a retrospective series and discussed it reviewing the literature data. SUBJECTS AND METHOD: From January 2002 to January 2009, 75 parotidectomies were performed in 72 patients (34 males, 38 females), without FNM in 28 cases (group 1) and with FNM in 47 cases (group 2). RESULTS: In group 1, 9 FP were noted, 5 were transient and 4 were definitive. In group 2, 12 FP were noted, 9 were transient

Optic nerve MRI enhancement in posterior ischaemic optic neuropathy due to internal carotid artery dissection.

Posterior ischaemic neuropathy (PION) is characterized by infarction in the retrobulbar optic nerve. A 73-year-old man suddenly experienced blurred vision in his left eye and intermittent weakness in his right hand. He had visual defects of superior lateral quarter and inferior medial quarter areas in the left eye. Gadolinium-enhanced magnetic resonance imaging (MRI) revealed segmental enhancement in the left optic nerve. A cerebral angiogram showed a left internal carotid dissection (ICD). He did not have fever, and his laboratory and cerebrospinal fluid tests were normal. These findings were

Treating facial nerve palsy by true termino-lateral hypoglossal-facial nerve anastomosis.

Hypoglossal-facial nerve anastomosis is a time-proven technique for the repair of facial nerve palsy. Efforts have been made to reduce hypoglossal nerve injury, the main drawback of the technique. In this study, the anastomosis is a true termino-lateral neurorrhaphy with only an epineural window in the hypoglossal nerve sheath. A re-routing technique of the temporal facial nerve is also performed to allow a direct anastomosis to the hypoglossal nerve without the need for a jump graft. The first three results reported are very encouraging, with a satisfactory return of facial mimics and without

Vertebrobasilar dilatative arteriopathy (dolichoectasia).

Dolichoectasia (dilatative arteriopathy) describes marked elongation, widening, and tortuosity of arteries. The intracranial vertebral and basilar arteries are preferentially involved. Dolichoectatic arteries usually have an abnormally large external diameter and a thin arterial wall, with degeneration of the internal elastic lamina, multiple gaps in the internal elastica, thinning of the media secondary to reticular fiber deficiency, and smooth muscle atrophy. The most important clinical presentations of dilatative arteriopathy include acute brain ischemia; a progressive course related to com

A novel surgical approach to impacted mandibular third molars to reduce the risk of paresthesia: a case series.

PURPOSE: Extraction of impacted mandibular third molars (M3s) may cause temporary or permanent neurosensorial disturbances of the inferior alveolar nerve (IAN). Although the incidence of this complication is low, a great range of variability has been reported in the literature. Several methods to reduce or eliminate this complication have been proposed, such as orthodontic-assisted extraction, extraction of the second molar, or intentional odontoectomy. The purpose of this series of cases is to present a novel approach for a riskless extraction of impacted mandibular M3s in contact with the IA

The craniocaudal extension of posterolateral approaches and their combination: a quantitative anatomic and clinical analysis.

OBJECTIVE: The aim of this study was to describe quantitatively the properties of the posterolateral approaches and their combination. METHODS: Six silicone-injected cadaveric heads were dissected bilaterally. Quantitative data were generated with the Optotrak 3020 system (Northern Digital, Waterloo, Canada) and Surgiscope (Elekta Instruments, Inc., Atlanta, GA), including key anatomic points on the skull base and brainstem. All parameters were measured after the basic retrosigmoid craniectomy and then after combination with a basic far-lateral extension. The clinical results of 20 patients w

Coronectomy as the treatment of choice in wisdom teeth showing radiographic signs of close proximity to inferior dental nerve.

The aim of the study is to compare the prevalence of post-operative inferior dental nerve (IDN) deficit after coronectomy with total removal of wisdom teeth showing specific radiographic signs of close proximity to IDN. A randomized clinical trial comparing total removal and coronectomy of wisdom tooth was conducted. Analyses of the correlations of IDN deficit and various radiographic signs of wisdom tooth roots showing close proximity to IDN were performed. Two radiographic signs were found to be positive predictors ofintra-operative IDN exposure. Specific radiographic signs or the presence o

Cortical integrity of the inferior alveolar canal as a predictor of paresthesia after third-molar extraction.

BACKGROUND: Paresthesia is a well-known complication of extraction of mandibular third molars (MTMs). The authors evaluated the relationship between paresthesia after MTM extraction and the cortical integrity of the inferior alveolar canal (IAC) by using computed tomography (CT). METHODS: The authors designed a retrospective cohort study involving participants considered, on the basis of panoramic imaging, to be at high risk of experiencing injury of the inferior alveolar nerve who subsequently underwent CT imaging and extraction of the MTMs. The primary predictor variable was the contact rel

Radiation dose-volume effects of optic nerves and chiasm.

Publications relating radiation toxicity of the optic nerves and chiasm to quantitative dose and dose-volume measures were reviewed. Few studies have adequate data for dose-volume outcome modeling. The risk of toxicity increased markedly at doses >60 Gy at approximately 1.8 Gy/fraction and at >12 Gy for single-fraction radiosurgery. The evidence is strong that radiation tolerance is increased with a reduction in the dose per fraction. Models of threshold tolerance were examined.

Neurosensory function following mandibular nerve lateralization for placement of implants.

This prospective cohort study investigates the types and duration of neurosensory disturbances (NDs) related to mandibular nerve (MN) lateralization for placement of implants. From March 2001 to March 2007, 87 patients underwent implant placement surgery with MN lateralization. All patients had panoramic radiograph and CT scans taken preoperatively. Patients completed questionnaires to report NDs 1 week after surgery, and at the end of each month within the year after surgery. All 87 patients (110 posterior segments, 23 bilateral and 64 unilateral) participated in the study. All patients repor

Neutrophil responses to injury or inflammation impair peripheral gustatory function.

The adult peripheral taste system is capable of extensive functional plasticity after injury. Sectioning the chorda tympani (CT), a primary sensory afferent nerve, elicits transient changes in the uninjured, contralateral population of taste receptor cells. Remarkably, the deficits are specific to the sodium transduction pathway. Normal function is quickly restored in the intact nerve, in parallel with an influx of macrophages to both the denervated and uninjured sides of the tongue. However, changing the dietary environment by restricting sodium blocks the macrophage response and prolongs fun

Repair of ocular-oral synkinesis of postfacial paralysis using cross-facial nerve grafting.

We present the surgical techniques and results of cross-facial nerve grafting that have been developed in the repair of ocular-oral synkinesis after facial paralysis. Eleven patients with ocular-oral synkinesis after facial paralysis underwent the cross-facial nerve grafting with facial nerve transposition at a tertiary academic hospital between 2003 and 2009. The patient selection for the study was based on the degree of disfigurement and facial function parameter rating using the Toronto Facial Grading System. The procedures used were surgeries done in two stages. All cases were followed up

Exogenous modulation of intrinsic optic nerve neuroprotective activity.

BACKGROUND: To characterize the molecular and functional status of the rat retina and optic nerve after acute elevation of intraocular pressure (IOP). METHODS: Retinal ischemia was induced in rats by increasing the IOP (110 mmHg/60 minutes). Microarray analysis, quantitative RT-PCR (qRT-PCR) and immunohistochemistry were used to characterize retinal tissue. PLGA microspheres containing neurotrophic factors (BDNF, GDNF, or CNTF) or empty microspheres were injected into the vitreous of operated animals 1 day after elevation of IOP. Pupil light reflex (PLR) parameters and electroretinograms (ERG

Assessment of the risk of perforation of the mandibular canal by implant drill using density and thickness parameters.

OBJECTIVE: The objective of this study was to investigate whether the resistance of the bone surrounding the mandibular canal had sufficient density and thickness to avoid perforation by drills when preparing the bed of the implant. BACKGROUND: Damage to the inferior alveolar nerve (IAN) is more common than expected. This injury may lead to serious complications ranging from mild paresthesia to total anaesthesia of the lower jaw. MATERIALS AND METHODS: The CT images of 99 patients, whose ages ranged between 20 and 79years, and who applied for an implant application to the posterior aspect of

Classification of potential risk factors for trigeminocardiac reflex in craniomaxillofacial surgery.

PURPOSE: Trigeminocardiac reflex (TCR) in craniomaxillofacial surgery can lead to severely life-threatening situations. At least mild forms are probably much more common than the existing surgical literature suggests. Therefore, the aim of this presentation of cases and literature review was to evaluate the predisposing factors leading to a classification of risk factors for potential TCR and to give information concerning preventive measures and management procedures. PATIENTS AND METHODS: All surgery reports from the Department of Cranio-Maxillofacial and Oral Surgery in the University Hosp

A new vagal anchor electrode for real-time monitoring of the recurrent laryngeal nerve.

BACKGROUND: Despite conventional neuromonitoring, the recurrent laryngeal nerve (RLN) is still at risk for damage during thyroid surgery. The feasibility of continuous RLN monitoring by vagal nerve (VN) stimulation with a new anchor electrode should be shown, and electromyographic signal alterations of stressed RLN were analyzed to be alerted to imminent nerve failure whereby the nerve damage becomes reversible. METHODS: VN stimulation was achieved in 23 pigs. Sensed signals were analyzed and stored as real-time audio/video feedback EMG system. RLN was stressed by mechanical and thermal injur

A real-time monitoring system for the facial nerve.

OBJECTIVE: Damage to the facial nerve during surgery in the cerebellopontine angle is indicated by A-trains, a specific electromyogram pattern. These A-trains can be quantified by the parameter "traintime," which is reliably correlated with postoperative functional outcome. The system presented was designed to monitor traintime in real-time. METHODS: A dedicated hardware and software platform for automated continuous analysis of the intraoperative facial nerve electromyogram was specifically designed. The automatic detection of A-trains is performed by a software algorithm for real-time analy
