[Intraoperative facial nerve monitoring for parotid gland surgery].

INTRODUCTION: The route of the facial nerve in its extracranial part determines the technique of parotid gland surgery. Permanent facial nerve paralysis after parotidectomy is not rare. It is the most devastating complication for the patient and surgeon. Facial nerve monitoring by observing or palpating the face during the surgery is a long-standing practice. Using an EMG device is a standard procedure at present. AIM OF THE STUDY: Evaluation of the effectiveness of intraoperative facial nerve monitoring for parotid gland surgery. MATERIAL AND METHODS: Fifty three patients operated on due to

Lymphadenectomy along the left recurrent laryngeal nerve by a minimally invasive esophagectomy in the prone position for thoracic esophageal cancer.

BACKGROUND: A thoracoabdominal esophagectomy for esophageal cancer is a severely invasive procedure. A thoracoscopic esophagectomy may minimize injury to the chest wall and reduce surgical invasiveness. Conventional thoracoscopic procedures are performed in the left lateral-decubitus position. Recently, procedures performed in the prone position have received more attention because of improvements in operative exposure or surgeon ergonomics. However, the efficacy of the prone position in an aggressive thoracoscopic esophagectomy with an extensive lymphadenectomy has not been fully documented.

Clinical treatment of traumatic brain injury complicated by cranial nerve injury.

OBJECTIVE: To discuss the epidemiology, diagnosis and surgical treatment of cranial nerve injury following traumatic brain injury (TBI) for the sake of raising the clinical treatment of this special category of TBI. PATIENTS AND METHODS: A retrospective analysis was made of 312 patients with cranial nerve injury among 3417 TBI patients, who were admitted for treatment in this hospital. RESULTS: A total of 312 patients (9.1%) involving either a single nerve or multiple nerves among the 12 pairs of cranial nerves were observed. The extent of nerve injury varied and involved the olfactory nerve

Complications in retrosigmoid cranial nerve surgery.

CONCLUSIONS: Although microvascular decompression (MVD), facial nerve splitting (FNS) and neurectomy procedures were safe treatments for hemifacial spasm (HFS), trigeminal neuralgia (TN) and glossopharyngeal neuralgia (GPN) in retrosigmoid cranial nerve surgery, fatal and severe complications may occur. It is essential to pay great attention to the entire procedure to avoid these complications. OBJECTIVE: To report the complications of cranial nerve surgery via the retrosigmoid approach. PATIENTS AND METHODS: We reviewed 516 cases of cranial nerve surgery via the retrosigmoid approach for HF

[High exposure of the distal internal carotid artery].

High exposure of the internal carotid artery is a challenging procedure even for experienced surgeons. Access to the distal internal carotid artery is impeded by progressive encroachment of the mastoid process and the angle of the mandible and furthermore at this level the artery is intimately associated with the hypoglossal and glossopharyngeal nerves. If high exposure is needed we prefer preparation of the distal internal carotid artery by dissection of the venter posterior of the digastric muscle and the styloid process including the stylohyoid and stylopharyngeus muscles. This procedure ca

Trigeminal impingement syndrome: the relationship between atypical trigeminal symptoms and anteromedial disk displacement.

The purpose of this study was to determine if compression of the mandibular nerve and its branches could be caused by antero-medial disk displacement, resulting in atypical facial pain. Sixteen temporomandibular joints (TMJ) were dissected and injected with an autopolymerizing solution into the superior compartment, which produced an artificial capsular swelling that caused disk displacement. In all specimens, the TMJ capsule was close to the mandibular branch of the trigeminal nerve after the intracapsular injection. Thus, capsular distension or antero-medial disk displacement, as seen in var

Environmental triggers of thyroiditis: hepatitis C and interferon-α.

Autoimmune thyroid diseases (AITD) are postulated to develop as a result of a complex interplay between several genetic and environmental influences. The pathogenesis of AITD is still not clearly defined. However, among the implicated triggers (e.g. iodine, infections, medications), more recent data confirmed strong associations of AITD with the hepatitis C virus (HCV) infection and interferon-α (IFNα) therapy. Moreover, it is likely that HCV and IFN act in synergism to trigger AITD in patients. Indeed, approximately 40% of HCV patients develop either clinical or subclinical disease while rece

[Albers-Schönberg's disease with ocular manifestation].

Albers-Schönberg's disease is a rare disease (one case in 100,000 inhabitants), asymptomatic in the majority of cases. It belongs to the four clearly individualized forms of human osteopetrosis and has an autosomal dominant transmission. It induces generalized osteosclerosis, and most symptoms result from complications such as fractures following mild injury, compression of cranial nerves, especially the optic nerve, by stenosis of extracranial ostia, but also osteomyelitis of the lower maxilla. The treatment of Albers-Schönberg's disease is disappointing and only symptomatic, although the res

[Genuine adult unilateral glaucomas: true or false].

The diagnosis of unilateral glaucoma essentially requires an etiologic investigation primarily to make sure that the deterioration is truly glaucomatous, thus specifically ruling out other optical neuropathies and papillary abnormality, such as tilted disc or drusen, which can mimic glaucomatous perimetric defects. Nonetheless, some apparent unilateral glaucomas can become bilateral later, as with primary open-angle glaucoma, though more frequently with primary closed-angle glaucoma in the absence of prophylaxis, and with exfoliative glaucoma. Genuine unilateral glaucomas concern the pigmentar

[Herpes zoster oticus-associated jugular foramen syndrome].

We report a patient with herpes zoster oticus who presented with acute jugular foramen syndrome, and we present the review of 9 similar cases reported previously. Jugular foramen syndrome associated with varicella-zoster virus (VZV) infection is characterized by acute-onset dysphagia and dysphonia, usually accompanied or preceded by cranial, cervical, or pharyngeal pain. Herpetic eruptions on the skin or the mucosal surface may not occur, occur late after onset, or go undetected. Magnetic resonance imaging may reveal contrast enhancement around the jugular foramen, which implying inflammation

Unusual oral complications of herpes zoster infection: report of a case and review of literature.

A case of herpes zoster infection with unusual oral complications involving the mandibular division of the trigeminal nerve is presented. The post-herpetic complications of osteonecrosis, spontaneous exfoliation of teeth, and subsequent pathologic fracture of mandible in the absence of concurrent predisposing factors in a 65-year-old man are demonstrated. Forty-one cases with osteonecrosis and spontaneous exfoliation of teeth previously presented in the literature are reviewed. This is the first report of pathologic fracture after herpes zoster infection.

Facial nerve palsy: etiology and approach to diagnosis and treatment.

Facial nerve palsy has a broad differential diagnosis and possible psychological and anatomical consequences. A thorough investigation must be performed to determine the cause of the palsy and to direct treatment. If no cause can be found, therapy with prednisone with or without an antiviral medication can be considered and begun as early as possible after onset of symptoms. Resolution and time to recovery vary with etiology, but overall prognosis is good.

Malformation of the external ear and facial paralysis caused by megatemperature hydro-aluminum.

We present a case of right lateral auricle contracture malformation, auricular canal atresia, and complete facial paralysis (House-Brackmann grade VI) caused by a megatemperature hydro-aluminum injury at work. The diastrophic auricle and auricular canal atresia were reshaped. The complete facial paralysis was reanimated to House-Brackmann grade III after surgical hypoglossal-facial end-to-end anastomosis. These outcomes indicate that hypoglossal-facial end-to-end anastomosis is an effective surgical option for successful reanimation of complete facial paralysis.

[Identification of the recurrent laryngeal nerve and parathyroids in thyroid surgery].

Subtotal thyroid resection in the case of benign goiter carries a high risk of recurrence. The relatively high rate of postoperative cancer findings also leads frequently to secondary completion thyroidectomy. Internationally therefore the paradigm has shifted from subtotal resection to extended or total thyroidectomy, which shifts the risk to the vocal cords and parathyroids, where as a rule complications are much worse than those to the thyroid. The main surgical goal therefore is avoiding as far as possible recurrent laryngeal nerve palsy and disturbances to parathyroid function following e

Vulnerable structures during intraoral sagittal split ramus osteotomy.

The aim of this study was to elucidate the anatomical structures that are vulnerable to injury during sagittal split ramus osteotomy (SSRO). Twenty-nine hemifaces of 19 Korean adult cadavers (11 men and 8 women; age range, 50-91 years) were dissected, and the locations of the facial nerve, retromandibular vein (RMV), and external carotid artery (ECA) were measured on the base of the mandibular posterior border and occlusal plane. Sagittal split ramus osteotomy was performed on the cadaver heads at intervals of 10 mm, and the proximity of the facial nerve was observed. The buccal branch and man

[Sinonasal cavities: CT imaging features of anatomical variants and surgical risk].

Anatomical variants of the sinonasal cavities are common. About 15 major variants are described (nasal septal deviation is present in up to 62% of the population). Because the may lead to complications at the time of endovascular procedures or endoscopic sinonasal surgery (vascular, nervous, or osseous injury), there detection has medicolegal implications. Knowledge of anatomical variants by radiologists and ENT surgeons is thus required. We will describe these variants, their imaging features, frequency, implications and associated risk of potential complication.

Multiple cranial neuropathies.

Patients presenting with multiple cranial neuropathies are not uncommon in neurologic clinical practice. The evaluation of these patients can often be overwhelming due to the vast and complicated etiologies as well as the potential for devastating neurologic outcomes. Dysfunction of the cranial nerves can occur anywhere in their course from intrinsic brainstem dysfunction to their peripheral courses. The focus of this review will be on the extramedullary causes of multiple cranial neuropathies as discussion of the brainstem syndromes is more relevant when considering intrinsic disorders of the

Peduncular hallucinosis following microvascular decompression for trigeminal neuralgia without direct brainstem injury: case report.

We report a 68-year-old woman who experienced visual hallucinations diagnosed as peduncular hallucinosis (PH) following microvascular decompression for trigeminal neuralgia. Postoperative MRI showed only ischemic oedema of the left cerebellar hemisphere due to retraction. Our case first reveals that PH can be induced without direct brainstem injury, instead possibly by a cerebellar lesion.

Surgical risk factors and maxillary nerve function after Le Fort I osteotomy.

PURPOSE: Data on intraoperative risk factors for long-term postoperative complications after Le Fort I osteotomy (LFO) are limited. The aim of this study was to describe prospectively the overall postoperative changes in maxillary nerve function after LFO, and to correlate these changes with a number of possible intraoperative risk factors. PATIENTS AND METHODS: Twelve men and 13 women (mean +/- standard deviation: aged 25+/-10 years) participated in 4 sessions: 1 before LFO (baseline), and the rest at 3, 6, and 12 months after LFO. At each session, somatosensory sensitivity was assessed for

Atypical trigeminal neuralgia of the mental nerve: a case study.

Any division or branch of the trigeminal nerve can exhibit signs and symptoms of neuralgia. Those who treat patients suffering with craniofacial pain are frequently charged with determining the cause of illusive pain complaints and if objective signs are not discovered, the patient may not be treated and may be forced to seek help elsewhere. Trigeminal neuralgia-like pain is often seen in the mental nerve region of the mandible, but frequently, there is no radiographic evidence for the source of such pain. In the current case report, it was discovered that the patient's mental nerve pain was c
