OBJECTIVE: Elevated intracranial pressure (ICP) is an important complication of craniosynostosis, and early identification may help optimize surgical planning and treatment. Although invasive ICP monitoring remains the reference standard, its routine use in infants and young children is limited by procedural risks. This study aimed to evaluate whether quantitative computed tomography (CT) parameters could serve as noninvasive imaging markers associated with a high risk of elevated ICP in children under 2 years of age with craniosynostosis, compared with age- and sex-matched controls.

Hyperpolarization-activated cyclic nucleotide-gated (HCN) channels have recently emerged as promising targets for the treatment of neuropathic pain. This study investigated the potential involvement of HCN2 channels in the development of trigeminal neuropathic pain following peripheral nerve injury. Infraorbital nerve chronic constriction injury (ION-CCI) model was adopted to rats, and head withdrawal thresholds (HWT) to mechanical stimulation were assessed pre- and postoperatively, as well as after pharmacological intervention. In the trigeminal ganglion (TG), intracellular cyclic adenosine m

BACKGROUND: Postoperative recurrent laryngeal nerve paralysis (RLNP) is a major complication following radical esophagectomy with superior mediastinal lymph node dissection for esophageal cancer. We evaluated whether intraoperative nerve integrity monitoring (NIM) reduces the incidence of RLNP. To our knowledge, this is one of the largest studies reported to date.
METHODS: We retrospectively reviewed patients with esophageal cancer who underwent minimally invasive or robot-assisted esophagectomy at our institution between January 2013 and December 2024. NIM was introduced in January 2018 and

Peripheral facial nerve defects resulting from oncological resection necessitate early repair, and underscore the need for accurate intraoperative assessment of their extent. This study proposes a span-based classification system for peripheral facial nerve defects, based on the length and location of the defect. The system delineates specific defect characteristics and provides practical guidance for intraoperative decision-making and personalised repair plans.

BACKGROUND: Intracranial hypertension is a life-threatening complication of acute brain injuries such as traumatic brain injury (TBI), subarachnoid hemorrhage (SAH), or intracerebral hemorrhage (ICH). In low-income and middle-income countries (LMICs), limited resources can delay timely neurocritical interventions. Smartphone-based quantitative pupillometry offers a scalable solution for early detection of elevated intracranial pressure (ICP). Here, we assessed its ability to (1) detect raised optic nerve sheath diameter (ONSD), a noninvasive surrogate for elevated ICP, and (2) classify severe

Superior orbital fissure syndrome (SOFS) is a rare, vision-threatening condition caused by injury or compression of cranial nerves III, IV, and VI. We report a 16-year-old male who sustained extensive craniofacial trauma, including a displaced left sphenoid wing fracture, after a motorized scooter accident. Initially, he had limited extraocular motility, but by hospital day 5, he developed complete ophthalmoplegia due to progressive nerve compression. Urgent surgical decompression of the superior orbital fissure and reduction of the fracture were performed via a pterional craniotomy on day 7.

Investigating TCM for preventing and treating respiratory virus-induced nerve injury urgently requires highly biomimetic models that can replicate the complex structure and physiological microenvironment of the body. Conventional two-dimensional cell models and animal experiments present significant limitations, hindering a comprehensive understanding of the complicated interactions between drug components and their targets. As cutting-edge technologies, organoids and organ-on-a-chip systems address these shortcomings by respectively modeling three-dimensional cellular architecture, inter-orga

Neurogenic cough is a diagnosis of exclusion characterized by laryngeal hypersensitivity. More common etiologies of chronic cough should be systematically excluded prior to diagnosis. Although the pathophysiology of neurogenic cough is not fully understood, proposed etiologies typically involve injury to the vagus nerve. Treatment options include neuromodulating medications, behavioral cough suppression therapy, and procedures such as superior laryngeal nerve block. Newer targeted treatments, including P2X3 antagonists, may offer effective treatment alternatives moving forward.

OBJECTIVE: Recurrent laryngeal nerve (RLN) paralysis is a significant complication of thyroid surgery. Even with meticulous nerve preservation, postoperative RLN paralysis can still occur. We investigate risk factors for postoperative RLN paralysis among the patients with thyroid carcinoma.
METHODS: Between 2017 and 2023, 216 patients underwent surgery for thyroid carcinoma at Kumamoto University Hospital. Among these, 26 patients displayed preoperative vocal cord paralysis. Records for the remaining 190 patients were reviewed retrospectively to identify risk factors for postoperative RLN par

BACKGROUND: Inferior alveolar nerve (IAN) and lingual nerve (LN) injury remain among the most significant complications of mandibular third molar surgery. Earlier studies relied primarily on panoramic predictors such as interruption of the white line (IoWL), often without three dimensional validation. This multicentre retrospective analytical study aimed to identify independent predictors of neurosensory injury and to develop and internally validate a composite risk prediction model. METHODS: Records of 2,850 third molar extractions performed between January 2019 and December 2024 across three

Inferior alveolar nerve (IAN) injuries are common complications of orthognathic surgery, with incidences reported from 0% to 85% due to inconsistent definitions, assessment methods and follow-up protocols. These limitations hinder comparisons, emphasising the need for standardised evaluation. This 10-year retrospective cohort study investigates IAN injury rates following mandibular osteotomies, focusing on medium-long term outcomes and risk factors. A retrospective cohort study was conducted at a tertiary orthognathic centre (2014-2024) on sagittal split osteotomies, performed either as stand-

Sagittal split ramus osteotomy (SSRO) is the most commonly performed mandibular procedure in orthognathic surgery. Although generally safe, neurosensory disturbance (NSD) of the inferior alveolar nerve (IAN) remains a frequent complication that can impair oral function and quality of life. This retrospective study included 463 patients who underwent SSRO, comprising 873 rami, at our department between 2014 and 2024. Patient- and ramus-level variables were extracted from electronic medical records, with postoperative NSD of the IAN defined as the primary outcome. We investigated the presence or

BACKGROUND: Recurrent laryngeal nerve (RLN) dysfunction represents one of the most feared complications of thyroid surgery, with significant impacts on patients' quality of life due to the potential paralysis of the vocal cords. Although nerve transection is rare, functional loss - non-structural and invisible to the human eye - is the primary cause of this complication. Intraoperative nerve monitoring (IONM), introduced more than 50 years ago and significantly improved over the past decade, represents a crucial tool for preventing this outcome.
METHODS: This retrospective observational study

PURPOSE: Tracheal branch (TB) and esophageal branch (EB) injuries of the recurrent laryngeal nerve (RLN) during thyroid surgery are underexplored. In this study, we examined postoperative clinical manifestations in these injuries. METHODS: Using a head-mounted 2.5× magnifying loupe, TBs and EBs were isolated to assess branching patterns and postoperative outcomes. RESULTS: TBs comprised 1–3 branches, most commonly two, whereas EBs comprised 1–2 branches, usually one. Clinical symptoms were more frequent after TB injury than in controls, with 81.6% of patients showing chest tightness or dyspnea

BACKGROUND: Vagal paragangliomas (VPGs) are rare neuroendocrine tumors, typically benign but with unpredictable growth and local invasiveness. Their management is debated, particularly in asymptomatic or elderly patients, due to the lack of clear predictive indicators for progression and treatment response. This systematic review aimed to assess the clinical features and treatment outcomes of VPGs.
METHODS: We conducted a systematic review of the literature, screening studies at multiple levels. A total of 41 articles were included, comprising data from 501 patients. Treatments were categoriz

Cerebrospinal fluid (CSF) leakage can be difficult to localize and may result in unexplained intracranial hypotension with intractable positional headache. Loss of CSF cushioning can cause brain sagging, leading to compression and traction of cranial nerves within the brain cisterns and mimicking Chiari malformation. This case report describes a patient presenting with trigeminal facial pain and positional headache who was ultimately diagnosed with a thoracic spinal CSF-venous fistula. Both headache and facial pain resolved following surgical intervention. This report highlights the importance

PURPOSE: This study aimed to compare the effects of proximal versus distal dissection of the recurrent laryngeal nerve (RLN) on the preservation of parathyroid function in patients undergoing total thyroidectomy for malignant thyroid tumors. METHODS: In this retrospective study, 209 patients were categorized into two groups based on the RLN exposure technique: a proximal approach (identification within 1 cm of the laryngeal entry point, n = 59) and a distal approach (identification within the tracheoesophageal groove, n = 150). Serum parathyroid hormone (PTH) and calcium levels were measured p
