Impacted mandibular first molar with root encircling the inferior alveolar nerve and an odontogenic cyst formation.

BACKGROUND: Dentigerous cysts are among the most common benign lesions encountered in oral and maxillofacial surgery. While these cysts typically develop in association with impacted mandibular third molars, their occurrence in relation to mandibular first molars represents an exceptionally rare clinical entity. This study aims to systematically describe the surgical management, standardized postoperative care protocols, and long-term follow-up strategies for this rare presentation. CASE PRESENTATION: A 35-year-old female patient presented to our institution with a 12-month history of persist

The Positional Relationship Between the Mandibular Canal and the Lower Third Molar Determined on Cone-Beam Computed Tomography.

Background and Objectives: The extraction of mandibular third molars poses challenges due to their proximity to the mandibular canal and risk of inferior alveolar nerve (IAN) injury. Accurate preoperative evaluation is essential to minimize complications. This study assessed the three-dimensional positional relationship between the mandibular canal and lower third molars using cone-beam computed tomography (CBCT), aiming to identify anatomical positions associated with increased surgical risk. Materials and Methods: This retrospective study analyzed 253 CBCT scans of fully developed lower thir

Noninvasive Intracranial Pressure Prediction Using a Multimodal Ultrasound-Based Hemispheric Modeling Strategy: A Prospective Dual-Center Study.

BACKGROUND: Accurate intracranial pressure (ICP) surveillance is a cornerstone of neurocritical care management, yet invasive monitoring still depends on neurosurgical expertise, specialized hardware, and continuous bedside resources-factors that restrict universal use even though insertion-related complications are relatively uncommon. Contemporary noninvasive ultrasound methods have limited predictive accuracy and seldom incorporate affected side information. We therefore preliminarily validated a multimodal, ultrasound-based hemispheric modeling approach that blends hemodynamic and structur

[Chorda tympani in the practice of an otosurgeon. Literature review].

The chorda tympani is one of the most traumatic anatomical structures during surgical interventions on the middle ear. The problem of intraoperative damage to the chorda tympani remains underestimated by otosurgeons due to its insufficient coverage in the medical literature. A search and analysis of modern scientific publications from publicly available information resources was conducted, including review articles, original research, monographs, observations of clinical cases, and textbooks from domestic and foreign authors. The anatomical and topographic features that contribute to the injur

Impacted Mandibular Third Molar: Approaches and Current Perspectives in Surgical Therapy.

Mandibular third molar surgery is a procedure that can be accompanied by several complications, one of the most important being neurological ones, resulting from transient or permanent injury to the lingual nerve (LNI) or inferior alveolar nerve (IANI). For this reason, a conservative technique, coronectomy, has been proposed. Despite the fact that this procedure seems to have important advantages, such as IANI reduction, it turns out that it is currently a non-standardized procedure, showing several limitations and requiring a precise and well-documented informed consent process because its u

Migration of a Separated Endodontic File Into the Mandibular Canal: An 8-Year Follow-up Case Report.

Separated endodontic instruments in mandibular molars may migrate into the mandibular canal, potentially causing neurological complications. The management of asymptomatic cases remains controversial. This case report presents the long-term conservative management of a separated stainless steel file that migrated into the mandibular canal. A 47-year-old male patient with chronic myeloid leukemia presented with a separated endodontic file that had migrated from the distal root of his mandibular right second molar into the mandibular canal over a 7-year period. Clinical examination, panoramic ra

Clinical features, management, and prognosis factors of traumatic optic neuropathy.

We provide a comprehensive clarification of the demographics, clinical features, management, prognostic factors, and complications of traumatic optic neuropathy (TON). About 6068 patients from 109 studies were included. The pooling results indicated that TON was male-dominated (84 %) with road traffic accidents as the leading cause (67 %). After trauma, 46 % of cases had no light perception (NLP), and the most frequent associated injury was craniofacial fractures (70 %). Optic canal fractures (OCF) were detected in 34 % of cases via computed tomography and in 47 % intraoperatively. The general

Resolution of Iatrogenic Spinal Accessory Neuropathy Following Delayed Surgical Intervention.

We present an interesting case of a 74-year-old female who presented with spinal accessory neuropathy following an ipsilateral right-sided carotid endarterectomy 6 months after the initial injury. Subsequent surgical exploration revealed a surgical clip that had been placed directly across the spinal accessory nerve. Clip removal and neurolysis was subsequently performed, leaving the nerve intact. After surgical intervention, she immediately demonstrated increased bilateral shoulder shrug motion and continues to improve her function with physical therapy. This case demonstrates a fascinating c

A Comparison of End-to-End Versus Reverse End-to-Side Nerve Transfer Techniques in Treating Brachial Plexus Birth Injury-Associated Glenohumeral Dysplasia.

BACKGROUND: Glenohumeral dysplasia (GHD) commonly occurs following brachial plexus birth injuries and, if not addressed appropriately, can result in shoulder dysfunction. Despite its severity, there are no clearly defined surgical criteria for correcting GHD. This study compares the outcomes of end-to-end and reverse end-to-side spinal accessory to suprascapular nerve transfers for correction of GHD. METHODS: All patients presented with shoulder functional limitations, as indicated by Active Movement Scale (AMS) scores in shoulder abduction, shoulder flexion, and external rotation, and eviden

Diagnostic Neuromuscular Ultrasonography of Spinal Accessory Nerve Injury After Cosmetic Rhytidectomy.

Iatrogenic spinal accessory nerve (SAN) injuries can occur as complications of posterior triangle of the neck surgery, resulting in lateral scapular winging and a shoulder droop. Rarely has this complication been described after a cosmetic rhytidectomy (face-lift procedure). Here, we present two cases of iatrogenic SAN injuries that occurred after cosmetic rhytidectomies. The use of electrodiagnostic and ultrasound examinations provides invaluable information to assess peripheral nerve injuries. Electrodiagnostic testing can confirm the presence and severity of the injury, while ultrasound can

Apelin-13 attenuates optic nerve damage in glaucomatous mice by regulating glucose metabolism.

BACKGROUND: In patients with advanced glaucoma, lesions frequently extend beyond the eye and affect other organs. However, early distal consequences of elevated intraocular pressure (IOP) remain unclear. This study aimed to observe glucose uptake in the optic cortex during the early stages of elevated IOP and to investigate the mechanism by which Apelin13 exerts neuroprotective effects. METHODS: This study used a single anterior chamber injection of polystyrene microbeads and triblock copolymer hydrogel in 6- to 8-week-old male C57BL/6J mice and observed glucose uptake in the optic cortex dur
