Diseases of the esophagus : official journal of the International Society for Diseases of the Esophagus2025eng
Recurrent laryngeal nerve palsy (RLNP) is a significant complication following upper mediastinal lymphadenectomy during esophagectomy. In this retrospective study, we investigated the association between recurrent laryngeal nerve (RLN) diameter and the risk of RLNP following robot-assisted minimally invasive esophagectomy (RAMIE). We analyzed consecutive patients with esophageal cancer who underwent RAMIE between January 2015 and March 2024. The left RLN diameter was measured from intraoperative digital recordings using validated image processing software. Patients were categorized into thick

Preservation of facial nerve function is extremely important in vestibular schwannoma surgery. Intraoperative transcranial facial motor evoked potential (Tc-fMEP) monitoring was used, and its accuracy and the correlations of Tc-fMEP results with postoperative facial nerve function at various time points were investigated. Factors associated with postoperative deterioration of facial nerve function were also examined. Forty-five consecutive cases of vestibular schwannoma that underwent surgery at our hospital from January 2013 to July 2022 were retrospectively reviewed. The correlation between

BACKGROUND: For immediate facial nerve reconstruction during head and neck tumor resection, a great auricular nerve (GAN) graft can be potentially harvested in the same surgical field during tumor resection. However, it is often avoided because a GAN graft is only approximately 5 cm long without any branches, and a sural nerve graft is recommended for larger defects. We investigated the length of the GAN that can be harvested in 18 patients, along with the evaluation of postoperative facial nerve palsy.
METHODS: We retrospectively analyzed 18 cases of immediate facial nerve reconstruction usi

BACKGROUND: Selecting the correct imaging technique for critical anatomical structures is essential in descriptive studies and for supporting clinical applications. Facial canal dehiscence poses a significant risk for iatrogenic facial nerve injuries during middle ear surgeries. Accurate imaging is critical for surgical planning and minimizing complications. Detection of facial canal openings in the clinic is performed using imaging methods such as high-resolution computed tomography (HRCT). Studies have shown that the sensitivity of this method is approximately 66%. A high-resolution, 3D imag

OBJECTIVE: This study examined patterns of cellular and neural degeneration in the peripheral vestibular system following head trauma, comparing cases with and without skull/temporal bone fractures.
METHODS: We analyzed 26 temporal bones (17 cases) with head trauma with fractures (six cases, nine ears) and ones without fractures (11 cases, 17 ears). Appropriate age-matched control groups comprising temporal bones without temporal bone pathology were included for comparative purposes. Histopathological analyses included counts of Scarpa's ganglion cells (ScGCs) in the superior and inferior ves

Substance P (SP) expressed by corneal nerves, is an 11-amino acid long neuropeptide from the tachykinin family, encoded by the Tac1 gene, and binds to neurokinin receptors. SP overexpression is associated with various pathological responses in the cornea including vasodilation, pain, inflammation, and angiogenesis in the normally avascular tissue. This study investigates the role of neurokinin-1 receptor (NK-1R) mediated signaling in nociception, nerve regeneration, and neuronal activation following mechanical corneal injury in mice. Corneal injuries were induced in age- and sex-matched C57BL/

BACKGROUND AND OBJECTIVES: Preservation of the frontotemporal branches of the facial nerve (FTB of CN VII) is essential to prevent frontalis muscle dysfunction after anterolateral skull base surgery. Although anatomical integrity of the FTB of CN VII is often preserved, patients may still experience frontalis palsy, suggesting that nontransectional mechanisms may contribute to neuropraxia. In this study, we describe the use of intraoperative neurophysiological monitoring combined with refined, tension-conscious retraction techniques to mitigate iatrogenic injury to the FTB.

Vestibular schwannoma surgery demands much of the surgeon, both in preparation and execution of a surgical plan. We review preoperative aspects, including decision-making and setting up the necessary equipment and surgical environment. The ergonomics of microsurgery and strategies to deal with the surgical burden are reviewed. The steps in tumor resection are described in detail, with the authors' perspective on how each of these stages is best achieved. This incorporates techniques for effective tumor removal while identifying and preserving key structures. We expand upon methods to dissect b

The growing use of injectable cosmetic fillers has been accompanied by an increase in reports of acute vision loss, typically caused by inadvertent intravascular injection and subsequent embolization of the ophthalmic artery, resulting in ocular ischemia. Effective management of this complication remains challenging, as clear clinical guidelines for retrobulbar hyaluronidase injection are lacking. This ultrasound-based observational anatomical guideline provides practical recommendations for retrobulbar administration of hyaluronidase. Real-time ultrasonography allows for direct visualisation

Lower cranial nerve palsy is a rare condition, most commonly caused by malignant skull base lesions or internal carotid artery dissection, and only rarely resulting from trauma. We report two cases of road traffic accident victims presenting with lower cranial nerve palsy involving cranial nerves IX, X, and XI. Both patients had similar clinical complaints. The most important findings were a dislocated occipital condyle fracture and suspected associated soft tissue injury. Both were treated conservatively with a rigid cervical collar. A nasogastric tube was required for the first patient durin

While orbital apex syndrome is considered a rare condition, it has significant neuro-ophthalmic implications for diagnosis and management. It is characterized by the involvement of the following cranial nerves: the optic nerve, the oculomotor nerve, the trochlear nerve, the abducens nerve, and the first division of the trigeminal nerve, manifesting as clinical symptoms upon their impairment. Although various factors can be causal, trauma can also lead to the development of orbital apex syndrome. Regardless of the cause, treatment often involves inducing decompression of the orbital space using

Upper gastrointestinal dysfunction is one of the most common comorbidities of spinal cord injury (SCI) and significantly impairs overall health and quality of life. Despite the need for targeted treatment options, the causal mechanisms underlying upper gastrointestinal dysfunction after injury remains unknown. Previous studies have demonstrated gastric vagal afferents are less sensitive to stimuli after SCI, which may be due to changes in voltage-gated Ca2+ (CaV) channels in gastric-projecting nodose ganglia (NG) neurons, as they contribute to action potential initiation along vagal afferents

The unprecedented technical and technological evolution in thyroid surgery has labelled it as an extremely safe and efficient procedure, and indeed "typifies perhaps better than any other operation the supreme triumph of the surgeon's art."-William Halsted, 1852-1922. Surgeon's experience reflected by annual case load is the most important denominator in thyroid surgery. Nevertheless, even high-volume thyroid surgeons in high-volume centres are not immune to its complications. Despite the advances in surgical technology and techniques, recurrent laryngeal nerve (RLN) injury is still a wellknow

BACKGROUND: The aim of this study is to compare the traditional triple innervation technique with a new version, designed to reduce eye-mouth synkinesis. To reduce unpleasant synkinesis, the authors proposed changing the connection between the masseteric nerve and the injured facial nerve, analyzing the outcome in terms of reduction of synkinesis and power of contraction of the mimic muscle. The traditional technique conveys the quantitative stimuli from the masseteric nerve to the temporofacial branch and from the hypoglossal nerve (30%) to the cervicofacial branch. Traditionally, the stimulu

Temporal bone cholesteatoma is a common invasive lesion affecting the temporal bone, leading to a series of extracranial and intracranial complications, with facial nerve paralysis being one. The treatment of temporal bone cholesteatoma typically requires surgical resection to prevent its complications. The article discusses the preservation of the facial nerve during temporal bone cholesteatoma surgery, the management of intraoperative facial nerve injury, and the treatment of cholesteatoma-associated facial paralysis.
