Ectopic orbital brain tissue with perineural cerebrospinal fluid leakage: a case report.

A newborn girl presented with perinatal left periorbital swelling and was treated with antibiotics for 7 days, without improvement. At 9-days-old, ophthalmic examination showed an inferomedial soft, palpable, mobile mass with prominent dilated overlying conjunctival vessels and a morning glory optic disc anomaly of the left eye. Magnetic resonance imaging (MRI) revealed a complex, multilobulated mass displacing the globe superolaterally. Subtotal excision revealed benign ectopic brain tissue. Subsequently, swelling and proptosis recurred. Computed tomography (CT) with cisternogram showed cereb

Navigating the Rare Medial Rectus Entrapment in Orbital Fractures: A 5-Year Series and Systematic Review of the Literature.

BACKGROUND: Inferior rectus muscle entrapment associated with orbital floor fractures is commonly described in the literature. Less commonly described, however, is the phenomenon of medial rectus entrapment found in association with medial orbital wall fractures. This study primarily aims to bring further attention to this rare occurrence through the presentation of three medial rectus entrapments encountered at a single institution. Additionally presented are a systematic review of the literature and a discussion of the authors' preferred surgical technique for management of acute medial rect

Factors associated with diplopia before and after orbital blowout fracture reconstruction.

BACKGROUND/AIMS: To ascertain factors associated with persistent diplopia after orbital fracture reconstruction. METHODS: In this retrospective study, we reviewed 798 cases with depressed fragment fracture at the Department of Kyoto Prefectural University of Medicine Hospital between January 2009 and November 2023. The restriction of extraocular movements and diplopia was assessed using the Hess area ratio (HAR). The factors for HAR% ≥85% before surgery and 6 months after surgery were assessed. RESULTS: Of the 798 cases in this study, 437 cases (54.8%) had an HAR% of ≥85%, and 361 cases (45.

Is Ophthalmology Consult Needed for Acute Orbital Fractures?

Failure to recognize concomitant ocular injury in orbital fracture patients is a feared consequence of evaluation in the acute setting. These injuries and their accompanying functional sequelae range in severity, and reconstructive surgeons need to be sensitive to the possibility of major ocular injuries that could potentially have serious outcomes. Indications for specialty-trained ophthalmology evaluation in patients with orbital fractures remain controversial, with some advocating for an immediate and comprehensive evaluation of every patient with an orbital fracture. In many institutions,

Esotropia Induced by Slipped Lateral Rectus Muscle After Orbital Fat Herniation Surgery: A Case Report.

BACKGROUND Strabismus and diplopia are well-documented complications following various ophthalmic surgeries; however, no prior reports have linked these complications to orbital fat herniation surgery. Here, we report a case of secondary esotropia resulting from lateral rectus muscle injury following orbital fat herniation surgery. CASE REPORT A man in his 60s presented to our hospital with diplopia during distance fixation, which he noticed a few days after undergoing orbital fat herniation surgery on his right eye. His visual acuity was 24/20 in both eyes. Alternate prism cover testing revea

Two Cases of Rapidly Progressive Vision Loss From Ischemic Reperfusion Injury and Rhabdomyolysis.

Orbital compartment syndrome due to rhabdomyolysis and subsequent ischemic reperfusion injury is an extremely rare event. Treatment consists of decreasing intraorbital pressure, which may include emergent surgical decompression and addressing the underlying cause, which in these cases was skeletal muscle injury in the setting of prolonged immobilization. Despite intervention, acute orbital compartment syndrome with visual acuity loss has a poor prognosis if effective therapy is not initiated promptly. To the authors' knowledge, there is only 1 previously reported case of orbital apex syndrome,

Balanced orbital decompression for thyroid-associated orbitopathy and the role of stereotactic navigation in infraorbital nerve preservation.

INTRODUCTION: Thyroid-associated orbitopathy (TAO) is the most common adult inflammatory eye disease. Proptosis is a TAO common clinical sign. Besides daily physical discomfort, TAO impacts negatively mental health. Proptosis is treated by orbital decompression surgery. Infraorbital nerve dysesthesia is a common complication. Stereotactic navigation is an imaging-guided surgery. This less invasive procedure could reduce postoperative complications and improve recovery. This study aims to evaluate the impact of navigation on patients' outcomes. METHODS: A retrospective study was conducted, inc

Successful management of a rare intraorbital wooden foreign body extending to the orbital apex in a paediatric patient.

Intraorbital foreign bodies (IOFBs) are a recognised complication of orbital trauma, but intraorbital wooden foreign bodies are rare and pose significant diagnostic and management challenges due to their radiolucency, high risk of infection and potential for severe inflammation. This case describes a rare presentation of a young boy who sustained orbital trauma after an 8-foot fall, resulting in a 5 cm × 3 cm wooden IOFB extending to the orbital apex with an associated sphenoid fracture. He presented with proptosis, an inferotemporal scleral indentation mimicking retinal detachment and a later
