Endoscopic transorbital surgery: a transformative approach to pediatric orbital trauma with functional and structural complications.

The endoscopic transorbital approach (ETA) has emerged as a promising technique in minimally invasive skull base surgery. However, there is limited data on its safety and efficacy in pediatric patients. We operated on pediatric patients with (Cerebrospinal Fluid) CSF leakage, compression of orbital structures and orbital subperiosteal hematoma due to trauma. Patients were operated to eliminate limitation of eye movements, to prevent the persistence of diplopia, to prevent CSF leakage and to reduce proptosis. This study aim to evaluate the feasibility and outcomes of ETA in the surgical treatme

Toxic Orbital Cellulitis and Optic Neuropathy from High-Pressure Diesel Fuel Injury: A Case Report and Literature Review.

PURPOSE: High-pressure injuries to the orbit are rare, yet they can cause severe damage due to mechanical destruction and the toxicity of the injected substance. Diesel fuel toxicity can lead to rapid necrosis and inflammation, complicating the prognosis even with prompt medical intervention. METHODS: A 39-year-old male presented with a right orbital injury from a diesel fuel explosion. Initial symptoms included mild periorbital swelling and a laceration on the upper eyelid. Despite early management with antibiotics and surgical repair, the patient's condition rapidly deteriorated, developing

Granulomatosis with polyangiitis presenting as axial proptosis and corneal perforation-a case report.

BACKGROUND: Granulomatosis with polyangiitis (GPA) is a rare, aggressive, and rapidly progressive multisystem disease that primarily affects the respiratory tract, kidneys, and vascular system. Although less common, GPA can affect the eye through corneal involvement, scleral inflammation, and orbital mass formation. This report highlights a case in which sudden axial proptosis and corneal perforation revealed underlying GPA. CASE PRESENTATION: A 32-year-old female presented with a 20-day history of painful left eye proptosis, purulent discharge, photophobia, and progressive vision loss, which

Orbital textiloma: Two case reports and a literature review.

PurposeThis report presents two cases of orbital textiloma resulting from retained surgical gauze.Case descriptionBoth patients presented with postoperative orbital inflammation unresponsive to medical treatment: one eight weeks after excision of an orbital cavernous hemangioma, and the other six months following surgical repair of an orbital floor fracture. CT scans of the orbit revealed well-defined lesions with a heterogeneous center. An exploratory orbitotomy retrieved a large piece of gauze that was firmly adhered to the globe and the adjacent extraocular muscle. Both patients experienced

Endovascular treatment of carotid-cavernous fistulas in a medium-sized Scandinavian neurovascular center.

PURPOSE: Carotid-cavernous fistulas (CCFs) are treated almost exclusively by endovascular techniques, but the frequency of treatments is limited in smaller centers. We analyzed all CCFs treated in our hospital to determine if high-quality treatment of CCFs can be provided in a medium-volume neurovascular center. METHODS: Retrospective quality-control cohort study. RESULTS: From 2005 to 2021, 28 CCF-treatments were performed in our hospital, all endovascular. Fourteen were direct and fourteen were indirect CCFs. All direct CCFs were treated using transarterial coil embolization, and in seven

Retrospective evaluation of ocular injuries in fractures of the zygomaticoorbital complex in a level I trauma center: is primary specialized ophthalmologic examination always necessary?

PURPOSE: The necessity of a specialized ophthalmological assessment following fractures of the zygomaticoorbital (ZMO) complex in an emergency setting is still debated in resource-limited health systems. The primary aim of this study was to determine the incidence and types of ocular and periocular injuries (OPIs) associated with different fracture patterns of the ZMO complex. The secondary aim was to investigate the association between patient- and trauma-specific variables with the different types of OPI and identify high-risk patients for severe OPI. METHODS: A retrospective cohort study w

Penetrating orbital injury by a nine-centimetre lead sinker.

A male in his early 40s presented with a penetrating orbital injury after a nine-centimetre lead sinker was propelled into his right orbit while fishing. He reported pain, ptosis and visual acuity reduced to light perception. X-rays, preferred over CT due to metallic artefacts, revealed the sinker in the inferior orbital fissure with an intact eyeball. Initial surgical extraction attempts triggered a severe vagal response, necessitating endoscopic navigation for safe removal. Postoperatively, despite an intact globe, he developed vitreous haemorrhage and retinal detachment, requiring vitrectom

A case of panophthalmitis with orbital infection of polymicrobial origin.

BACKGROUND: Infectious endophthalmitis following penetrating ocular trauma represents an acute and often blinding disease with a generally poor prognosis. The causative pathogens involved are diverse, among which bacterial organisms are the most prevalent. In contrast, infections caused by fungi of the order Mucorales are rare, constituting acute and fulminant opportunistic mycoses. These fungal infections are characterized by an insidious onset and are associated with an exceedingly high mortality rate, particularly in patients with comorbid diabetes mellitus. CASE PRESENTATION: A 60-year-old

From cabin pressure to orbital pressure: subperiosteal hematoma unmasking a frontal sinus hematoma.

We describe the first case of a subperiosteal orbital hematoma complicating acute barotraumatic frontal sinusitis during a plane landing. A 38-year-old woman presented with a sudden-onset and intense left periorbital pain during a plane landing. Few hours after a commercial flight landing, she presented a delayed left periorbital ecchymosis and left subperiosteal orbital hematoma. An exclusively endoscopic approach effectively drained both the frontal submucosal and orbital subperiosteal hematomas. This approach not only resolved the sinus pathology but also restored ocular function, with sign

Eyelid Malpositions and Ocular Surface Disease: Clinical Correlations and Management Strategies.

Eyelid retraction, cicatricial entropion, and deformities associated with facial nerve palsy are among the eyelid malpositions most detrimental to the ocular surface, as they cause exposure, tear film instability, inflammation, and potentially significant visual impairment. These conditions present major functional and esthetic challenges, underscoring the need for a clear understanding of their mechanisms and management. A narrative review was conducted using PubMed, MEDLINE, Embase, and Google Scholar to identify English and non-English studies (with English abstracts) addressing eyelid malp

Advances in materials research related to orbital reconstruction: A review.

Orbital injuries or defects caused by various reasons are quite common, such as violent trauma or tumors. If the damaged orbits are not treated in a timely manner and the normal orbital structure cannot be restored, it may lead to ocular nerve injury, embedding or protrusion of orbital contents, and complications such as enophthalmos, diplopia, and eye movement disorders. Therefore, it is particularly important to repair orbital injuries or defects and reconstruct the normal structure of the orbit. Currently, there are various types of implants applied to reconstruct the orbit, which can be ca

PEEK-on-PEEK orbital implantation: a secondary augmentation approach for delayed enophthalmos.

This case report discusses the long-term management of a patient's left zygomatic fracture with an associated orbital floor injury. The initial treatment involved open reduction and internal fixation (ORIF), followed by the placement of a polyetheretherketone (PEEK) implant for globe dystopia. Despite this intervention, the patient developed progressive secondary enophthalmos and globe dystopia. To correct this, a second PEEK implant was used to improve the position of the eye. The aim of this report is to present a novel technique of PEEK-on-PEEK implants to restore globe alignment. This case
