C/EBPβ Drives Ischemia-Induced Microglial Inflammatory Reprogramming Through Transcriptional Activation of ATF3 and Exacerbates Brain Injury.

Microglial activation contributes substantially to secondary brain injury after ischemic stroke, but the transcriptional mechanisms governing this response remain poorly defined. Here, we identified CCAAT/enhancer-binding protein β (C/EBPβ) as a key regulator of ischemia-induced microglial inflammation. RNA sequencing of microglia isolated from mice subjected to transient middle cerebral artery occlusion (tMCAO) revealed extensive transcriptional reprogramming, characterized by activation of inflammatory pathways, including chemokine, TNF, IL-17, and efferocytosis signaling, as well as cell-cy

Noninvasive ultrashort wave neuromodulation enhances glymphatic clearance to alleviate traumatic brain edema and Alzheimer's pathology.

The glymphatic system clears cerebral waste, yet strategies to enhance its function remain elusive. Here, we show that 40.68 MHz ultrashort wave (USW) rapidly enhances glymphatic bulk flow in healthy mice within 10 minutes. USW potentiates both glymphatic influx and efflux, concomitant with increased Delta wave neuronal synchronization, as confirmed by local field potentials and electroencephalograms. Chemogenetic silencing confirms that USW-driven glymphatic enhancement depends on neuronal activity, while pharmacological blockade of aquaporin-4 (AQP4) abolishes the effects, revealing dual rel

A Standardized Workflow for Multimodal Assessment of Right-to-Left Shunt-Related Ischemic Brain Injury.

Right-to-left shunt (RLS) has been associated with cryptogenic stroke, migraine, and other neurological conditions in which ischemic brain injury may be clinically relevant. In routine practice, however, assessment is often fragmented. Shunt presence may be documented, but shunt burden, provoked versus non-provoked patterns, anatomical complexity, and MRI-defined brain injury are not always integrated into a reproducible workflow. Here, we describe a standardized protocol for multimodal assessment of RLS-related ischemic brain injury in a clinically selected cohort. The workflow includes patie

Recommendations for the Management of Spontaneous Intracranial Hypotension With Spinal Longitudinal Epidural CSF Collection: A Delphi Study.

BACKGROUND AND OBJECTIVES: Spontaneous intracranial hypotension (SIH) results from spinal CSF leakage. Epidural blood patch (EBP) is an important first-line treatment. However, pre-EBP, intra-EBP, and post-EBP practice varies widely owing to limited consensus guidance. We aimed to develop expert consensus statements to support clinical decision-making in SIH management, including peri-EBP practices, focusing on patients with spinal longitudinal epidural CSF collection (SLEC) before definitive leak type classification. METHODS: An international multidisciplinary Delphi process was conducted be

Mild Traumatic Brain Injury and Psychiatric Disorders in Year 1 of the Adolescent Brain Cognitive Development (ABCD) Study.

OBJECTIVE: Pediatric mild traumatic brain injury (mTBI) is a major public health concern. New psychiatric disorders (NPDs) can arise after mTBI, yet postinjury NPDs in nonclinical samples remain poorly understood. METHODS: NPDs were prospectively characterized for youths ages 9-11 years who experienced an mTBI in their first year of participation in the Adolescent Brain Cognitive Development study, compared with control groups of orthopedically injured and noninjured individuals; a broad set of preinjury factors was accounted for in analyses. RESULTS: Between baseline (ages 9-10) and year 1

Early hematoma expansion provides incremental prognostic information beyond admission characteristics for 6-month functional outcome after adult traumatic brain injury.

BACKGROUND: Early hematoma expansion (HE) reflects interval progression of traumatic intracranial hemorrhage, but its association with long-term outcome and its prognostic contribution beyond admission characteristics remain incompletely defined. We evaluated whether HE was independently associated with 6-month functional outcome and provided incremental prognostic information beyond admission characteristics. METHODS: This single-center retrospective study included 640 adults with traumatic brain injury, including 285 with HE. HE was determined from the first two in-hospital head computed to

Reframing traumatic brain injury and spinal cord injury as chronic multisystem disorders: toward integrated, long-term care.

Traumatic brain injury (TBI) and spinal cord injury (SCI) affect more than 220 million people worldwide and are associated with cardiometabolic, endocrine, neurological, and psychiatric sequelae for years to decades after an acute injury. However, most health systems still treat neurotrauma (TBI and SCI, collectively) as a time-limited event, leaving long-term surveillance of these patients fragmented or absent, which leads to preventable mortality and avoidable readmissions. TBI and SCI in adults should be reclassified as acute-onset multisystem chronic-risk conditions. Existing cardiovascula

Post-traumatic dementia: Advances in understanding neurodegenerative mechanisms.

Traumatic brain injury can trigger long-term neurodegenerative diseases beyond its devastating acute effects. This narrative review summarizes current understanding of the neurodegenerative mechanisms underlying post-traumatic dementia. Core mechanisms involve multiple pathophysiological processes: long-term dysfunction of the blood-brain barrier, which impairs the clearance of toxic proteins; diffuse axonal injury, leading topersistent disruption of axonal transport, dysregulation of calcium homeostasis, and Wallerian-like degeneration; and the abnormal deposition of key pathological proteins

IMRCs Alleviate Post-resuscitation Cerebral Injury Possibly by Inhibiting Ferroptosis in a Porcine Model of Cardiac Arrest.

Cerebral injury following cardiac arrest (CA) is a primary contributor to patients' mortality and poor neurological outcome. However, there are currently no definitive and effective therapeutic approaches. Emerging evidence has indicated that mesenchymal stem cell (MSC) pretreatment exerted protective effects against cardiac and cerebral injuries in the porcine model of CA. However, given the unpredictable nature of CA, we administered immunity-and-matrix-regulatory cells (IMRCs) derived from human embryonic stem cells (hESCs) following CA to investigate whether IMRCs could alleviate cerebral

Artificial Intelligence in Maxillofacial Trauma.

Maxillofacial trauma presents significant diagnostic challenges due to the highly complex bony anatomy of the facial skeleton. The misdiagnosis or delayed detection of these fractures can lead to severe complications affecting a patient's vision and also their facial and masticatory functions. This review explores the integration of artificial intelligence (AI) into clinical workflows for the diagnosis, triage, and decision-making regarding the management of maxillofacial fractures. Serving as an indispensable third eye, AI not only reduces dangerous diagnostic errors and accelerates emergency

Selenium-Enriched Lacticaseibacillus rhamnosus Alleviates Hypobaric Hypoxia-Induced Brain Injury via Regulation of Gut Microbiota and Neurotransmitter Homeostasis.

Brain injury induced by hypobaric hypoxia is a concern for individuals exposed to high-altitude environments. This study evaluated the protective potential of prophylactic selenium-enriched Lacticaseibacillus rhamnosus (SeLRS) administration in C57BL/6 mice exposed to hypobaric hypoxia. Results showed that just 1 day of hypoxic exposure induced significant anxiety-like, depressive-like, and cognitive impairments, accompanied by hippocampal-amygdala structural damage, a decreased glutamate/gamma-aminobutyric acid ratio, and elevated oxidative stress and neuroinflammation. Critically, injury rel

Extracting the Unexpected: A 35-mm Curved Nail as an Intraocular Foreign Body.

BACKGROUND Management of posterior segment intraocular foreign bodies is challenging, requiring the preservation of ocular integrity and restoring vision. Traumatic endophthalmitis occurs in 7% to 13% of penetrating injuries, particularly with coexisting intraocular foreign bodies. Managing posterior segment intraocular foreign bodies poses significant challenges due to their complex clinical presentation and the need to achieve multiple goals during treatment. This unusual case of a large metallic intraocular foreign body associated with traumatic cataract, retinal detachment, and endophthalm

The emerging role of brain-derived microparticles in trauma-induced coagulopathy after traumatic brain injury.

Traumatic brain injury (TBI) is frequently complicated by trauma-induced coagulopathy (TIC), which contributes substantially to secondary brain injury and poor clinical outcomes. The mechanisms underlying traumatic brain injury induced coagulopathy (TBI-IC) are multifactorial and involve extensive cellular alterations in endothelial cells, neutrophils, and platelets. Brain-derived microparticles (BDMPs), a subgroup of extracellular vesicles derived from brain-associated cells, attracted increasing attention due to their potential involvement in coagulation regulation after TBI. This review sum
