Sex-related differences in end-of-life decision-making in patients with traumatic brain injury: a systematic review and meta-analysis.

BACKGROUND: Families and medical professionals caring for traumatic brain injury (TBI) patients are often faced with decisions regarding the limitation of life-sustaining treatment (LLST). Considering difficulty of outcome prognostication and the resulting lack of objectivity, these decisions remain challenging. Although it is recognized that sex influences end-of-life care, research on sex differences in palliative care (PC) is scarce. To better understand the decision-making process at the end of life in TBI, we investigated potential sex-differences in the frequency of LLST and PC involveme

Tmem110 regulates the conformation of TRPML1 to maintain endolysosomal homeostasis and prevent mitochondrial DNA leakage and pathological self-DNA processing.

The mechanisms by which phagocytes handle large quantities of internalized organelles, such as mitochondria released during tissue injury, remain unclear. Here we show that the endoplasmic reticulum transmembrane regulator TMEM110 is a key determinant of disease severity in traumatic brain injury-associated multiple organ dysfunction. Loss of TMEM110 impairs the clearance of mitochondria aberrantly released into the circulation, leading to heightened autoimmune-mediated tissue injury and mortality. TMEM110 maintains lysosomal function by controlling the conformational transition of the lysosom

Active subperiosteal vs. passive subdural 24-h drainage following single burr hole evacuation of chronic subdural hematoma: statistical analysis plan for the multicenter, randomized, non-inferiority clinical trial SUPERDURA.

BACKGROUND: In Denmark, the current treatment of patients with symptomatic chronic subdural hematoma (CSDH) is single burr hole hematoma evacuation followed by 24-h passive subdural drainage. However, recent studies indicate that 24-h active subperiosteal drainage may be safer and have fewer recurrences. The upcoming SUPERDURA trial will investigate 24-h active subperiosteal drainage versus 24-h passive subdural drainage following single burr-hole evacuation of symptomatic CSDH. This study presents the statistical analysis plan for the SUPERDURA trial. METHODS: SUPERDURA is a national multi-c

Long-term health outcomes of adolescent mTBI: a scoping review.

INTRODUCTION: Adolescent mild traumatic brain injury (mTBI) carries potential long-term effects. This scoping review aimed to map and explore recent literature on the long-term outcomes of mTBI in adolescents aged 13-18, defining long-term as a minimum of 12 months post-injury. METHODS: Following the JBI framework and PRISMA-ScR guidelines, we reviewed 37 English articles published in peer-reviewed academic journals between 2015 and 2024. Narrative synthesis and concept analysis were used to identify themes and patterns related to long-term outcomes. FINDINGS: The findings reveal a complex p

Association between hemostatic changes and contusion volume in traumatic brain injury: an observational cohort study.

PURPOSE: Contusion expansion is a key determinant of outcome after traumatic brain injury (TBI). Because many patients develop acute coagulopathy, it has been proposed that hemostatic changes may drive this expansion, but the link remains uncertain. METHODS: In this retrospective single-center cohort, we included adults with isolated moderate-to-severe TBI and no pre-injury antithrombotic therapy. The hemostatic markers activated partial thromboplastin time (APTT), prothrombin time (PT, reported as INR), platelet count (PLT), and fibrinogen were measured on admission and during the first 72 h

Exercise ameliorates microglial activation and cognitive impairment induced by GFAT1 knockdown after traumatic brain injury.

BACKGROUND: The potential of exercise to prevent traumatic brain injury (TBI) has been extensively studied. Microglia play a critical role in TBI pathogenesis. Glutamine-fructose-6-phosphate transaminase 1 (GFAT1), the key enzyme regulating the hexosamine biosynthetic pathway (HBP), not only controls glucose influx but also plays an important role in the neuroinflammatory process. However, the relationship between GFAT1 expression and microglial metabolic activity during treadmill exercise in TBI mice remains unclear. METHODS: A mouse TBI model was established via needle puncture, with mice r

Anti-Inflammatory Potential of Stearidonic Acid Ethanolamide in a Model of Mild Traumatic Brain Injury in Mice.

Chronic neuroinflammation is recognized as a pivotal mechanism responsible for secondary damage following mild traumatic brain injury (mTBI), underscoring the critical need for therapeutic strategies capable of mitigating this pathological process. This study evaluated the anti-inflammatory properties of stearidonic acid ethanolamide (SDEA, C20H33NO2). The findings indicate that mTBI triggers persistent neuroinflammation, which is correlated with cognitive deficits. A ten-day treatment regimen with SDEA at 10 mg/kg/day facilitated the restoration of cognitive abilities and suppressed the neuro

The dual role of TRPC channel-mediated dysregulation of calcium signaling in regulating mitophagy in early brain injury after subarachnoid hemorrhage.

Subarachnoid hemorrhage (SAH) induces early brain injury (EBI) through mechanisms involving mitochondrial dysfunction and dysregulated calcium signaling. Transient receptor potential canonical (TRPC) channels are critical mediators of calcium homeostasis and have emerged as key players in SAH pathophysiology. This review explores the interconnected roles of TRPC channel-mediated calcium dyshomeostasis and mitophagy in EBI. We summarize how mitochondrial damage post-SAH triggers mitophagy via ubiquitin-dependent and -independent pathways, a process with dual effects on neuronal survival. We the
