Immune checkpoint inhibition in bone metastases: lesion response heterogeneity, the osteoimmune niche, and microenvironment-directed therapy.

Bone metastases are common in advanced solid tumors and are clinically important not only because they cause skeletal-related events, including bone pain, pathological fractures, and spinal cord compression, but also because they may influence responses to immune checkpoint inhibitors (ICIs). Clinical evidence has consistently associated bone metastases with poorer survival after ICI treatment. Although some studies suggest that the relative benefit of ICIs may be reduced in patients with bone metastases, most available evidence is retrospective and lacks adequate treatment-interaction testing

Fully percutaneous, navigation-assisted cervical pedicle screw instrumentation: technical note and a report of safety and accuracy.

INTRODUCTION: Cervical pedicle screws provide superior biomechanical stability compared with lateral mass screws, resulting in stronger and more durable constructs. Furthermore, most published reports consist of small retrospective series or feasibility studies, and prospective evidence regarding fully percutaneous navigation-assisted cervical pedicle screw fixation remains scarce. We present a technical note on fully percutaneous cervical pedicle screw instrumentation and report the preliminary radiological and clinical outcomes of our initial case series. MATERIALS AND METHODS: Data from th

Effects of an Adaptive Exercise Program on Inflammation After Spinal Cord Injury.

BACKGROUND: Chronic inflammation contributes to cardiovascular and respiratory diseases, the leading causes of death in individuals with spinal cord injury (SCI). People with SCI tend to have higher inflammation levels than able-bodied individuals and are often physically inactive despite the anti-inflammatory benefits of exercise. OBJECTIVES: This study evaluated whether an adaptive exercise program could reduce inflammation in individuals with SCI. METHODS: Twenty-four individuals with SCI were randomized to either an immediate start (intervention) or delayed start (control) group. The 12-

Association of coexisting type 2 diabetes and DXA-defined low muscle mass with prevalent radiographic vertebral fracture in community-dwelling older adults: a cross-sectional study.

BACKGROUND: Prevalent radiographic vertebral fracture is frequently clinically unrecognized in older adults, and areal bone mineral density does not fully characterize its distribution. Whether coexisting type 2 diabetes mellitus (T2DM) and objectively measured low muscle mass identify a subgroup in which this burden is concentrated remains unclear. METHODS: This community-based cross-sectional analysis included 1,642 adults aged 65 years or older recruited through 16 community health service centers covering 36 administrative communities in Changchun, China, between July 2021 and March 2023.

Transient Paraparesis from Post-traumatic Muscle Herniation Through Laminectomy Defect: Rare Sequela of Thoracolumbar Kyphosis Correction: A Case Report.

CASE: An 8-year-old girl with congenital thoracolumbar kyphosis underwent posterior instrumented correction with a closing-opening wedge osteotomy. Seven years later, minor trauma caused acute paraparesis. Imaging showed intact implants and solid fusion, but muscle herniation into the laminectomy defect had impinged the spinal cord. Steroids and physiotherapy led to full recovery within 1 month, confirmed at 1-year and 3-year postinjury follow-up. CONCLUSION: This case highlights transient paraparesis from soft-tissue impingement years after correction, underscoring the exposed apical cord's
