Machine learning for discovery of clinical pain biomarkers following spinal cord injury.

Chronic pain is highly prevalent in patients with spinal cord injury (SCI) and further degrades the quality of life in individuals already struggling with somatic, motor, and autonomic deficits. The management of SCI pain is challenging, mainly due to the lack of reliable, FDA-approved diagnostics, effective therapies, and incomplete understanding of the underlying mechanisms. These limitations have led to increased efforts dedicated to the identification of objective pain biomarkers. However, the FDA has yet to approve a physiologically relevant biomarker for the assessment of pain in populat

Retained intravesical catheters in a patient with spinal cord injury performing clean intermittent self-catheterization: A case report.

CONTEXT: Clean intermittent self-catheterization (CISC) is a first-line bladder management strategy for patients with spinal cord injury (SCI). However, complications such as urinary tract infections and stone formation are relatively common. The retention of the catheter as an intravesical foreign body is an exceedingly rare event and can present with symptoms similar to infection or radiolucent calculi. FINDINGS: A 36-year-old male with a history of chronic spinal cord injury (T12, AIS A) utilizing clean intermittent self-catheterization (CISC) presented with urinary frequency and the prese

Fatal hyperthermia triggered by drug-induced fever in a patient with high-level spinal cord injury: a case report.

We report a rare fatal hyperthermic crisis likely precipitated by drug-induced fever in a muscular, obese patient with chronic high-level spinal cord injury (SCI). Shortly after antibiotic therapy was changed to meropenem, the patient developed severe agitation and hyperthermia exceeding 41 °C, accompanied by autonomic instability. Despite intensive management, including active cooling, intravenous fluids, and vasoactive support, the core temperature remained above 41 °C. Computed tomography revealed a low-density brainstem lesion consistent with infarction. The patient experienced respiratory

Intravesical instillation of pirfenidone/polydopamine nanoparticles for managing neurogenic bladder post-spinal cord injury by alleviating bladder inflammation and fibrosis.

Neurogenic bladder (NB) secondary to spinal cord injury (SCI) progresses from early-stage bladder inflammation to late-stage fibrosis, ultimately resulting in renal dysfunction. However, effective therapeutic options for NB remain limited, and patients currently rely exclusively on catheterization-based management to prevent upper urinary tract deterioration. Polydopamine nanoparticles (PDA NPs), a mussel-inspired polymer formed through the oxidative self-polymerization of dopamine, exhibits inherent bioadhesive properties and notable anti-inflammatory activity. Herein, we develop an intravesi

Spinal augmentation for vertebral body fractures in the elderly population.

Osteoporotic vertebral body fractures (OVBFs) are a highly prevalent pathology in the United States elderly population and can result in height loss and kyphotic deformity. While minimally invasive surgical techniques such as percutaneous kyphoplasty (PKP) and vertebroplasty (PVP) are commonly used, the efficacy of these techniques in older individuals is unknown. By examining the current body of evidence, we aim to assess the efficacy of these procedures within the context of appropriate grading of levels of evidence. This narrative review was conducted by searching multiple databases, includ

In Vivo Telemetry to Record Long-Term Cardiovascular Parameters, Temperature, and Activity in Spinal Cord Injury Rat Models.

Higher-level spinal cord injury (SCI) above thoracic level 6 (T6) disrupts autonomic function and contributes to secondary complications, including fluctuations in blood pressure, heart rate, and temperature. In rats, the placement of telemetric implants in the descending aorta offers a robust methodology for assessing various cardiovascular parameters, such as systolic and diastolic pressure, mean arterial pressure, and heart rate. Core body temperature and animal activity can also be recorded following telemetric implant placement. A rat with a telemetric implant is kept on the receiver plat

Electroacupuncture alleviates depression-like behavior after spinal cord injury via SIRT1/HMGB1 signaling axis mediated ferroptosis.

BACKGROUND: Spinal cord injury (SCI) induces neuroinflammation and brain alterations that contribute to depression-like behaviors. Ferroptosis plays a critical role in neuronal injury following SCI. HMGB1 regulates ferroptosis, while SIRT1 inhibits HMGB1 activity. Electroacupuncture (EA) has demonstrated neuroprotective and antidepressant effects, but its mechanisms in SCI remain unclear. This study investigated whether EA alleviates post-SCI depression by activating SIRT1, retaining HMGB1 in the nucleus, and limiting ferroptosis in the prefrontal cortex (PFC). METHODS: A moderate SCI model w

Silencing of Ifi27l2a Attenuates Inflammation After Spinal Cord Injury by Regulating Microglial Polarization via JAK2/STAT3 Signaling.

Microglial polarization toward M1/M2 phenotypes is crucial in modulating neuroinflammation following spinal cord injury (SCI). This study aimed to investigate the role of interferon alpha-inducible protein 27-like 2A (Ifi27l2a) in regulating microglial polarization in SCI. The expression of Ifi27l2a were analyzed using single-cell RNA sequencing. C57BL/6 mice that underwent SCI were pretreated with adeno-associated virus (AAV) carrying sh-Ifi27l2a. In vitro, BV-2 cells were transfected with si-Ifi27l2a and stimulated with lipopolysaccharide (LPS). The effects of Ifi27l2a silencing were assesse

Sex differences in pain-related behaviour and the endocannabinoid system following hind limb ischemia-reperfusion injury.

Ischemia-reperfusion (I/R) injury causes tissue damage in organ transplantation, stroke and myocardial infarction, and underlies chronic wound formation. The endocannabinoid system (ECS) is involved in pain modulation, maintenance of skin homeostasis, and ischemic preconditioning, an endogenous protective mechanism against I/R-mediated damage. The rodent hind limb I/R model is a model of chronic post-ischemia pain, complex regional pain syndrome-type 1 (CRPS-1), and a potential model of wound-related pain. Research into sex differences in pain-related behaviour and the ECS following hind limb

Exosomes-mediated delivery of miR-27a-3p antagomir alleviates white matter injury by regulating PPARγ/PRDX1/JNK pathway after subarachnoid hemorrhage in rats.

White matter injury (WMI) is a critical factor contributing to poor neurological outcomes following subarachnoid hemorrhage (SAH). MicroRNAs (miRNAs) are key regulators of WMI-related pathology and can be delivered via exosomes, yet their mechanisms and therapeutic potential remain largely unexplored. In this study, miRNA sequencing revealed a significant upregulation of miR-27a-3p in peripheral blood exosomes after SAH, which was further confirmed in white matter tissue. BV2 cell-derived exosomes loaded with miR-27a-3p antagomir were administered intranasally and effectively targeted oligoden

Development and clinical validation of a stroke-specific Gait Deviation Index.

Stroke affects over 101 million individuals worldwide, leaving many to cope with long-term consequences. These often include physical and cognitive impairments such as muscle weakness, loss of coordination, and challenges in performing daily activities. Several quantitative indices have been developed to objectively quantify gait deviations with a focus on ensuring ease of use and accessibility within clinical settings. Among those, the most cited one is the Gait Deviation Index (GDI), originally developed with data from a population of children with cerebral palsy. This characteristic probabl
