Complications and Instrumentation Failure After Total En Bloc Spondylectomy: A Systematic Review and Meta-analysis.

STUDY DESIGN: Systematic review and meta-analysis. OBJECTIVE: To generate pooled estimates of instrumentation failure (IF), complication, mortality, and local recurrence rates after total en bloc spondylectomy (TES), and to quantify the influence of follow-up duration on reported IF rates. SUMMARY OF BACKGROUND DATA: TES offers margin-free resection for primary spinal malignancies and selected solitary metastases but carries substantial perioperative hazards and a delayed risk of IF. How the duration of postoperative observation shapes reported IF rates has not been formally quantified. MET

Surgical versus Nonsurgical Management of Traumatic Fracture in Geriatric Patients with Diffuse Idiopathic Skeletal Hyperostosis.

OBJECTIVES: Patients with Diffuse Idiopathic Skeletal Hyperostosis (DISH) are at increased risk for fracture with low-impact trauma. DISH is more often seen in the elderly, who may be at greater risk for postoperative complications. There remains a paucity of literature guiding surgical management for this patient population, which this study seeks to address. MATERIALS AND METHODS: This was a retrospective review of geriatric (≥65) patients admitted to the urban level 1 trauma center for a large healthcare system from January 1, 2018, to December 31, 2022, with radiographic evidence of an ac

Baseline history of primary fragility fracture is associated with medical complications and revision through 1 year in adult spinal deformity patients undergoing thoracolumbar spinal fusion.

PURPOSE: Fragility fractures (FFx) can indicate osteoporosis following posterior spinal fusion (PSF) in adult spinal deformity (ASD) patients. We sought to evaluate the impact of a history of FFx on adverse postoperative outcomes following ≥ 3-level PSFs. METHODS: This retrospective cohort study queried the Mariner10 PearlDiver database to identify patients aged ≥ 50 years undergoing ≥ 3-level PSF for ASD from 2010 to 2022. All trauma, infection, and tumor indications were excluded. Patients with and without a history of FFx (hip, proximal humerus, distal radius, vertebral body, or pelvis fra

Arthroscopic Posterior Cruciate Ligament Tibial Spine Avulsion Fixation Using a Suture Pull-Through Single-Tunnel Technique.

INTRODUCTION: Posterior cruciate ligament (PCL) tibial avulsion fractures are uncommon intra-articular injuries that may cause persistent posterior instability, loss of motion, and functional impairment if displaced fractures are not anatomically reduced. Arthroscopic suture pull-through fixation offers stable transosseous fixation while avoiding complications associated with screw fixation. AIM: The aim of this study was to evaluate the functional and radiological outcomes of arthroscopic PCL tibial avulsion fixation using a suture pull-through single-tunnel technique. MATERIALS AND METHODS

Stereotactic body radiotherapy versus conventional external beam radiotherapy for bone metastases: a meta-analysis.

Stereotactic body radiotherapy (SBRT) and conventional external beam radiotherapy (cEBRT) are widely used treatment options for the management of painful bone metastases. Randomized phase III trials have provided direct comparative evidence for SBRT and cEBRT; however, their results vary according to treatment regimen and clinical endpoint, supporting a broader quantitative synthesis of pain response, local control, retreatment, and safety outcomes. A systematic search was conducted in PubMed, Scopus, the Cochrane Library, and Google Scholar from database inception through January 2026. Compar

The impact of lesion size on outcomes following stereotactic body radiotherapy for spinal metastases.

OBJECTIVE: Stereotactic body radiotherapy (SBRT) for metastatic spinal disease provides effective local control (LC). However, there are limited data evaluating how the pretreatment volume or number of affected vertebral levels impacts treatment effect. Therefore, this study compared the efficacy of spine SBRT treatment on LC, distant progression (DP), and overall survival (OS) between patients with limited spinal disease (≤ 2 vertebral segments or < 50 cm3 pretreatment volume) to those with more widespread spine metastases. METHODS: All patients with metastatic spinal disease treated with SB

Interventional radiology in bone metastases: Current concepts and perspectives.

The skeleton is a frequent site of metastatic spread, leading to skeletal-related events (SREs) such as pathological fractures, spinal cord compression, and refractory pain that profoundly impact patient survival and quality of life. Traditionally managed by a triad of radiotherapy, surgery, and systemic treatments, the landscape of bone metastasis care has undergone a major paradigm shift. Interventional radiology (IR) has emerged as the "fourth pillar" of oncology, evolving from a palliative last-resort tool into a proactive specialty integrated into precision medicine and multidisciplinary

Association between thoracolumbar kyphosis and fracture risk assessment tool in Korean postmenopausal women: A retrospective study.

Thoracolumbar kyphosis (TLK) has been suggested as a potential risk factor for vertebral fractures in postmenopausal women; however, its association with bone mineral density (BMD), trabecular bone score (TBS), and fracture risk assessment tools, such as the Fracture Risk Assessment Tool (FRAX), remain undefined. In this study, we investigated the association between the TLK level and fracture risk in postmenopausal women. We retrospectively analyzed the associations between the thoracolumbar Cobb (TLCobb) angle, baseline compression fractures, BMD, TBS, and volumetric BMD measured using dual-

Type II odontoid fracture with complete posterior dens displacement: Manual reduction and anterior screw fixation.

BACKGROUND: Type II odontoid fractures with significant dens displacement are frequently treated with surgical fixation, most commonly through posterior cervical approaches. However, extensive posterior soft tissue injury may limit the feasibility of posterior fixation and require alternative anterior odontoid fixation. CASE DESCRIPTION: A 61-year-old male sustained a type IIA odontoid fracture with complete posterior dens displacement following an electrical burn injury and fall. Extensive burns involving the posterior neck/back precluded posterior cervical fixation (i.e., high risk of infec
