Predictors of delayed union after surgical treatment in patients with osteoporotic vertebral compression fractures: a systematic review and meta-analysis.

PURPOSE: The factors that predict delayed union in patients undergoing surgical management of osteoporotic vertebral compression fractures (OVCFs) have not been definitively established. This review aimed to comprehensively identify and analyze these factors to inform clinical practice and improve patient outcomes. METHODS: Four English and three Chinese databases were systematically searched from inception to July 1, 2025. Stata 16.0 was utilized for data analysis. Predictors of delayed union reported in two or more studies were combined. Sensitivity analyses were performed to ensure stabili

Robotic-assisted versus fluoroscopy-guided sacroiliac screw fixation: A retrospective comparative study.

BACKGROUND: Percutaneous sacroiliac (SI) screw fixation is a widely used technique for stabilizing sacral fractures but is considered technically demanding due to complex pelvic anatomy and proximity to neurovascular structures. Conventional fluoroscopy-guided methods are associated with a relatively high risk of screw misplacement and considerable radiation exposure to patients and staff. Robotic-assisted navigation systems have been introduced to enhance screw accuracy and reduce radiation exposure. The aim of this study was to assess the efficiency, safety, and accuracy of sacroiliac screw

Anterior Odontoid Screw Fixation: A Surgical Technique Guide By the Cervical Spine Research Society.

Odontoid fractures comprise 10%-15% of all cervical fractures in adults, of which Anderson and D'Alonzo type II odontoid fractures are the most common subtype. Anterior screw fixation for type IIB odontoid fractures is a safe and effective treatment option in the properly selected patient. This less invasive technique preserves the C1-C2 motion while stabilizing the dens and yields a high union rate with minimum morbidity in appropriately indicated patients. Younger patients and nonosteoporotic patients with anterior superior to posterior inferior (type IIB) fracture patterns are ideal candida

Fracture risk in metastatic breast cancer patients treated with CDK 4/6 inhibitors and endocrine therapy.

PURPOSE: Investigate skeletal morbidity (SM) in metastatic breast cancer (MBC) patients undergoing CDK4/6 inhibitors (CDK4/6is) and endocrine therapy (ET). METHODS: In this retrospective study we evaluated skeletal morbidity - defined as the occurrence of skeletal-related events (SREs) in metastatic bone and fragility fractures in non-metastatic bone - in 214 MBC patients who had received ET and CDK4/6is. As secondary aim, we compared VF progression, defined as a new fracture or worsening of a pre-existing fracture at spine CT scan, between 121 patients receiving ET alone (cohort A) and 121 p

Bone phenotype in patients with kidney failure with and without type 2 diabetes.

Type 2 diabetes mellitus (T2DM) is common in patients with chronic kidney disease (CKD). Both conditions are associated with an increased fracture risk, with the proposed etiology including impaired bone quantity and quality. This cross-sectional study examined the bone phenotype in patients with T2DM and kidney failure and explored the role of glycemic control on the bone phenotype. To do so, laboratory parameters of mineral metabolism (including sclerostin) and bone turnover markers (BTM) (bone-specific alkaline phosphatase [BALP], trimeric pro-collagen type I N-terminal propeptide [intact P

Atypical femoral fracture with early PET/CT changes from denosumab for metastatic breast cancer and transition from denosumab to zoledronic acid.

Atypical femoral fractures (AFFs) are potential complications arising from long-term use of bone modifying agents such as denosumab. We describe a case of bilateral AFFs that was treated surgically in a middle-aged woman receiving oncologic doses of denosumab for treatment of metastatic breast cancer for seven years. In retrospect, early cortical change was present on scout computer tomography (CT) and positron emission tomography-computer tomography (PET-CT) prior to her left AFF. This case highlights the potential for earlier opportunistic AFF radiologic screening during cancer surveillance.

Diabetic Neuropathy Is Associated With Lower Bone Mineral Density and Higher Fall Risk in Young Elderly Adults With Type 2 Diabetes.

BACKGROUND AND AIM: Diabetic neuropathy (DN) is a recognised risk factor for fragility fractures. However, the mechanisms linking DN, bone health, and falling risk remain unclear. We aimed to assess bone health and risk of falls, with their contributing factors, in young elderly patients with type 2 diabetes (T2D) and mild-to-moderate DN. METHODS: We enrolled 144 subjects with T2D, excluding those with severe DN (neuropathy disability score -NDS- ≥ 9) or fracture history. Clinical and biochemical data were collected, including surrogate markers of insulin resistance, such as the triglycerides

Temporal trends in major in-hospital and immobility-related complications following traumatic spinal cord injury: a retrospective cohort study.

BACKGROUND: Traumatic spinal cord injury (SCI) is a catastrophic condition associated with significant morbidity and healthcare utilization. While existing literature has largely focused on long-term sequelae, data on temporal trends in early in-hospital and immobility-related complications are limited. This study aims to evaluate temporal trends in major in-hospital and immobility-related adverse events among patients with acute traumatic SCI. METHODS: We conducted a retrospective cohort study using data from the American College of Surgeons Trauma Quality Improvement Program (TQIP) from 201

Preoperative malnutrition is associated with increased postoperative complications following lumbar fusion: a propensity-matched analysis.

BACKGROUND: Lumbar fusion (LF) is a common surgical intervention for degenerative spinal conditions but carries significant short- and long-term risks. Preoperative malnutrition, characterized by low serum albumin or leukocyte counts, impairs wound healing and increases susceptibility to infection, yet its influence on LF outcomes remains underexplored. This study evaluates the association between preoperative malnutrition and long-term postoperative outcomes following LF. METHODS: Adult patients undergoing LF between 2003 and 2023 were identified in the TriNetX US Collaborative Network. Maln

Pedicle screw insertion technique into a previously cement-augmented vertebral body: a technical note with a case series.

As the population ages, the incidence of osteoporotic vertebral compression fractures (OVCF) continues to rise, leading to an increased use of cement augmentation procedures. Consequently, clinicians are more frequently encountering patients with cement-augmented vertebrae who require additional spinal instrumentation. However, pedicle screw insertion into previously cement-augmented vertebral bodies remains technically challenging. This study aimed to describe a simple and reproducible technique for a safe and effective insertion of a pedicle screw into cement-augmented vertebral bodies. Ten
