BACKGROUND: Periprosthetic fractures of the glenoid and acromion are rare but clinically relevant complications following primary shoulder arthroplasty. Due to the increasing use of reverse shoulder arthroplasty, the incidence of these fractures is expected to increase.
OBJECTIVE: This review aims to summarize current evidence on the pathomechanisms, diagnosis, treatment strategies, and prognostic factors of periprosthetic glenoid and acromial fractures from a trauma and orthopedic surgery perspective.
MATERIALS AND METHODS: A narrative review of current clinical and biomechanical literature

STUDY DESIGN: We developed a novel posterior atlantoaxial screw-rod (PASR) fixation system to manage atlantoaxial dislocation (AAD) and compared it with a conventional PASR fixation system. The novel system was designed to simplify the fixation procedure and achieve direct reduction of irreducible AAD without the need for surgical release.
PURPOSE: To perform a comparative assessment of clinical and radiological outcomes between our novel PASR fixation system and a conventional PASR fixation system.
OVERVIEW OF LITERATURE: The current literature provides limited evidence on the efficacy of t

BACKGROUND: Tibial spine avulsion fractures (TSFs) are the pediatric equivalent of anterior cruciate ligament (ACL) injuries. The optimal management of type II fractures remains debated, and both conservative and surgical strategies have been described. This systematic review compares outcomes of nonsurgical and surgical treatments in type II TSFs and evaluates different surgical fixation techniques.
METHODS: A PRISMA-guided search identified studies reporting clinical and radiographic outcomes of pediatric TSFs. Data were pooled for patient demographics, treatment, complications, and functio

CASE: A 6-year-old boy with Hereditary Sensory and Autonomic Neuropathy Type IV (HSAN IV) presented after repetitive self-inflicted head trauma with new-onset headache and neck discomfort. He had a history of self-injurious behavior and was neurologically intact on exam. Imaging revealed a displaced odontoid synchondrosis fracture with C1 to C2 subluxation. Halo immobilization failed due to pin-site complications and behavior. He was successfully managed with staged nonoperative treatment including Minerva casting and cervicothoracic lumbar orthosis bracing.
CONCLUSION: This case highlights t

BACKGROUND: While sex-specific disparities in trauma outcomes are well established; the biological contribution of sex versus anthropometric variables like obesity in determining initial anatomical injury severity remains poorly understood. This study aimed to independently isolate the effect of biological sex on serious damage, while controlling for energy exposure and body factors.
METHODS: A retrospective cohort study of 2,128 consecutive adult trauma bay patients (males: n = 1570; females: n = 558) was conducted. Univariate comparisons and multivariate binary logistic regression models we

BACKGROUND: In the Netherlands, approximately 20,000 older adults sustain a hip fracture every year. Most of these patients are frail, institutionalised, have limited postoperative mobility and a high mortality rate. When surgery is not feasible or preferred, optimizing quality of life and effective pain management becomes paramount. However, current analgesic strategies often fail to provide sustained relief and timely discharge. Timely discharge is essential to prevent hospital-related complications in frail patients. BACKGROUND: Percutaneous pedicle screw fixation is widely used for the minimally invasive treatment of single-level thoracolumbar compression fractures. However, the conventional fluoroscopy-guided technique requires repeated imaging, resulting in increased radiation exposure and prolonged operative time. This study aimed to evaluate the clinical efficacy, safety, and efficiency of a posture-sensing method combined with blind guidewire probing for percutaneous pedicle screw placement.
METHODS: This single-center retrospective comparative study included 143 patients with single-level thoracolu

STUDY DESIGN: Case series.
OBJECTIVE: This study aims to analyze the characteristics of spinal and other injuries in survivors of suicide attempts who jump from heights.
SUMMARY OF BACKGROUND DATA: Suicide attempts by jumping often result in severe injuries, particularly affecting the spine. The severity and pattern of these injuries are strongly influenced by the height of the fall. However, there is a lack of comprehensive studies directly correlating jump height with injury outcomes.
METHODS: Patient data from 100 survivors of suicide attempts were retrospectively analyzed and categorize

INTRODUCTION: The rapid expansion of glucagon-like peptide-1 receptor agonist (GLP-1 RA) use for weight management and diabetes has raised perioperative questions, particularly regarding delayed gastric emptying and infection risk. Emerging real-world data suggest a more nuanced perioperative profile. This scoping review synthesizes evidence on preoperative GLP-1 RA exposure and postoperative outcomes across orthopaedic procedures, with emphasis on soft-tissue sports medicine, and is intended to map associative perioperative outcomes rather than establish causality.
METHODS: Following Arksey-

Urinary tract infections are one of the most frequent healthcare-associated infections representing a relevant postoperative complication in orthopedics patients. These infections are associated with increased morbidity, prolonged hospitalizations, and higher rates of readmission. In selected settings, they may coexist with or precede surgical site infections, including periprosthetic joint infections. This review aims to summarize and evaluate recent evidence from the last five years in regards to epidemiology, risk factors, clinical consequences, and preventative strategies for patients unde

Hip fractures in the elderly population are associated with considerable complications such as morbidity, mortality, and enduring functional deterioration. Thus, they render perioperative management an essential element of strategies aimed at fostering healthy aging. Regional anesthesia (RA) and general anesthesia (GA) constitute the two predominant methods for the surgical intervention of hip fractures; however, the most effective technique remains a subject of ongoing scholarly debate. This research review seeks to systematically assess and compare regional anesthesia to general anesthesia i

BACKGROUND: This study aimed to establish the median effective dose (ED50) and 95% confidence interval (CI) of 0.25% ropivacaine for ultrasound-guided pericapsular nerve group (PENG) block for facilitating spinal anesthesia positioning in elderly patients with intertrochanteric femoral fractures. METHODS: The elderly patients received elective proximal femoral nail antirotation (PFNA) surgery under spinal anesthesia. By adjusting the needle bevel orientation toward the superior pubic ramus, an ultrasound-guided PENG block was administered five minutes before positioning. The dose of 0.25% ropi

PURPOSE: Kirschner wires (K-wires) are commonly used for fixation of acromio-clavicular joint injuries. Although migration of broken K-wires has been described, migration of a broken K-wire into the spinal canal with spinal cord impalement is exceedingly rare. METHODS: We report the case of a 30-year-old man who underwent tension band wiring with K-wires for a displaced distal clavicle fracture. Fourteen months later, he presented with neck pain and headache without neurological deficit. Imaging revealed breakage of one of the K-wires with migration through the C7–T1 neural foramen into the sp

BACKGROUND: Halo-vest immobilisation remains an established non-operative treatment option for selected cervical spine injuries, particularly in patients with reducible fractures or high surgical risk. However, contemporary evidence evaluating functional recovery and patient-reported outcomes following halo treatment remains limited.
METHODS: Prospectively maintained institutional data of patients treated with halo-vest immobilisation between January, 2021 and December 2024 were retrospectively analysed. Forty-seven consecutive patients (mean age 43.1 ± 3.6 years) with odontoid Type II/III or

BACKGROUND: Hip fractures and vertebral fractures are significant public health concerns, causing substantial morbidity, mortality, and economic costs. Hip fractures often lead to mobility loss, while vertebral fractures, frequently underdiagnosed, can result in spinal deformities and recurrent fractures or subsequent injuries.
METHODS: This study identified hip and vertebral fractures using ICD-9 and ICD-10 codes in Taiwan's National Health Insurance Research Database (NHIRD). It applied Kaplan-Meier and Monte Carlo methods to estimate survival functions, comparing outcomes with a reference

BACKGROUND: Cement location within the vertebral body may influence pain relief after unipedicular vertebroplasty. We tested a simple four-zone classification of dominant intravertebral cement distribution in osteoporotic vertebral compression fractures.
METHODS: We retrospectively analyzed 425 patients treated from 2021 to 2024. On postoperative imaging, the vertebral body was divided into four equal zones (Zones 1-4) and the zone with the greatest cement accumulation was recorded. Pain was measured with the visual analog scale (VAS) before and after the procedure; change in VAS was the prim

BACKGROUND: Early surgical site infection (SSI) is a serious complication following proximal femoral fracture surgery in elderly patients. Perioperative blood transfusion has been suggested as a potential contributor to postoperative infection through immunomodulatory mechanisms, but evidence in geriatric trauma populations remains limited. This study evaluated the association between perioperative blood transfusion and early SSI in elderly patients undergoing surgery for proximal femoral fracture.
HYPOTHESIS: Perioperative transfusion of labile blood products is associated with an increased

UNLABELLED: Hypertension is associated with increased fracture risk. However, evidence on its association with BMD is conflicting. Here, we demonstrate that hypertensive patients have lower trabecular bone scores compared to normotensive subjects, despite similar BMD. This degradation of bone microarchitecture may help explain the increased skeletal fragility observed in hypertensive patients.
PURPOSE: Hypertension is associated with an increased fracture risk. However, evidence on its association with bone mineral density (BMD) is conflicting, and data on bone microarchitectural quality are

BACKGROUND: Iatrogenic vertebro-vertebral arteriovenous fistula (VVAVF) is an uncommon and potentially serious postoperative complication of spinal surgery. This case report highlights a rare occurrence of bilateral VVAVF after upper cervical spine fixation and the challenges in its diagnosis and management. The novelty of this case lies in its detailed description of the vascular anatomy involved, the surgical and endovascular interventions used and the successful treatment outcome.
CASE PRESENTATION: A 64-year-old woman presented with bilateral VVAVF following upper cervical spine fixation
