Spinal 7-dehydrocholesterol reductase dependent cholesterol overload regulates astrocyte activation and mGluR5/ERK cascades via monocyte chemotactic protein 3 in tibial fracture-induced chronic pain in mice.

Patients with bone fractures commonly experience an exasperating and long-lasting pain following orthopedic procedures. Spinal monocyte chemotactic protein 3 (MCP-3)-encoding astrogliosis and mGluR5-mediated excitatory synaptic plasticity are essential for central nociceptive transmission. Recent studies highlight the requirement of 7-dehydrocholesterol reductase (DHCR7) for cholesterol overload and neuroinflammation. This present study investigates the potential associations among DHCR7, MCP-3 and mGluR5 in chronic pain using a mouse model of tibial fracture with intramedullary pinning. Here,

Risk Factors for Vertebral Compression Fracture Following Spine Stereotactic Body Radiation Therapy.

PURPOSE: Vertebral compression fractures (VCFs) are potential complications following spine stereotactic body radiation therapy (SBRT). However, limited data exist on prognostic factors underlying VCF development following SBRT. The objective of this clinical cohort investigation was to evaluate risk factors for VCF development, VCF type (de novo or progressive), and VCF severity, following SBRT for spinal metastases. METHODS AND MATERIALS: A retrospective analysis was performed on a prospectively maintained database of 600 SBRT treatments (779 vertebral segments) for spinal metastases from 2

CT for Cervical Spine Clearance in the Obtunded Adult Blunt Trauma Patient is Appropriate in a Resource-Constrained Environment.

PURPOSE: Cervical spinal injury (CSI) following blunt trauma to the neck can have devastating consequences. There is a current controversy surrounding how CSIs are excluded in obtunded patients. This study aimed to evaluate the utility of CT in cervical spine clearance in a high-volume trauma center in a developing world setting. METHODS: Longitudinal 11-year data (2012-2022) were collected from the Hybrid Electronic Medical Registry (HEMR), based at the Pietermaritzburg Metropolitan Trauma Service, to identify all obtunded patients (GCS < 15) who underwent a cervical CT scan. The accuracy of

Duration of external neck stabilisation in the management of older and frail patients with a new odontoid fracture: the DENS RCT.

BACKGROUND: Fractures of the cervical spine's odontoid process can result from low-impact falls in frail or older adults, and are usually managed in hard collars to immobilise the fracture and promote bony healing. However, bony healing often does not occur in older/frail adults and is not necessary for good outcomes. Further, hard collars can cause complications, such as skin pressure ulcers, and difficulties with swallowing or personal care. OBJECTIVE: To examine whether management of odontoid process fractures without hard collar immobilisation is associated with improved quality of life i

C1 translaminar screw as a salvage technique for a failed lateral mass screw: A case report and literature review.

The placement of C1 lateral mass screws is a common procedure in posterior atlantoaxial fixation, but variations in local anatomy, including the C1 lateral mass and vertebral artery, can complicate screw placement, increasing the risk of complications. This case report presents the use of a C1 translaminar screw as a salvage technique following the failure of a C1 lateral mass screw during the treatment of a type II odontoid fracture. A 16-year-old male involved in a motor vehicle accident was scheduled for posterior atlantoaxial fixation with C1 lateral mass screws and C2 pedicle screws. Howe

Effect of Regional Anesthesia vs. General Anesthesia Technique on Perioperative Outcomes in Geriatric Patients With Hip Fracture: Narrative Review.

The rising incidence of hip fractures in aging populations necessitates optimized perioperative management to mitigate complications. This narrative review evaluates the impact of regional anesthesia (RA) and analgesia techniques compared to general anesthesia (GA) on perioperative outcomes in geriatric patients undergoing hip fracture surgery. Current evidence reveals no consistent mortality benefit for RA over GA. Multicenter trials such as REGAIN and RAGA found no significant difference in 30- to 60-day survival. Similarly, spinal anesthesia does not show a clear superior advantage over GA

Feiyanning formula inhibits metastasis and differentiation of osteoclasts by targeting miR-328/NF-κB signaling axis via exosomal HOTAIR in lung cancer cells.

As one of the most lethal malignant tumors worldwide, the highly invasive and metastatic nature of lung cancer leads to a generally poor prognosis for patients. Among the complications associated with lung cancer, skeletal-related events (SREs) resulting from bone metastases, including bone pain, pathological fractures, hypercalcemia, and spinal cord compression, significantly impair quality of life and reduce survival rates. The long non-coding RNA HOTAIR, which has been linked to various cancers, plays an unclear role in lung cancer bone metastasis and the regulation of the immune microenvir

Temporary Fusionless Occipitocervical Fixation of Bilateral Occipital Condylar Avulsion Fractures in an Adult.

INTRODUCTION: Occipital condyle fractures (OCFs) are rare injuries presenting with neurologic sequelae that may be debilitating or even fatal. Treatment options for unstable OCFs include fusion or halo placement. Fusion of the occipitocervical junction is associated with a significant loss of motion. CASE REPORT: This study presents a case of bilateral occipital condylar fractures with C1-C2 rotatory subluxation treated with temporary fusionless occipitocervical fixation. CONCLUSION: This is a rare case of bilateral OCFs with atlantoaxial instability that underwent fusionless instrumentation

Perioperative Anesthetic Challenges in the Management of an Early Periprosthetic Femoral Fracture Following Total Knee Arthroplasty in a High-Risk Patient: A Case Report.

Early periprosthetic distal femoral fractures following total knee arthroplasty (TKA) are uncommon but clinically significant complications requiring urgent surgical intervention. These cases pose substantial anesthetic challenges, particularly in obese patients and during early reoperation. We report the anesthetic management of a 47-year-old female patient (BMI 39 kg/m²) who underwent bilateral TKA followed by surgical fixation of a periprosthetic femoral fracture 25 days later. Both procedures were performed under combined spinal-epidural anesthesia with ultrasound-guided femoral nerve bloc

The Effect of Technological Innovations in Reverse Total Shoulder Arthroplasty on the Rate of Periprosthetic Acromion and Scapular Spine Fractures.

INTRODUCTION: Innovations in reverse total shoulder arthroplasty (rTSA) have improved outcomes and mitigated complications through improvements in implant design, surgical techniques, and enabling technologies. The purpose of this study was to evaluate the association between technologic evolutions in rTSA and the incidence of acromion and scapular spine fracture (ASF). The authors hypothesize that ASF incidence has decreased over time and that this decrease is temporally associated with successive changes in implant design, surgical technique, and enabling technology within a single-design pl

Pre-operative proton pump inhibitor use and risk of adverse outcomes after total elbow arthroplasty: a propensity-matched analysis.

BACKGROUND: Proton pump inhibitors (PPIs) are widely prescribed and commonly used in orthopedic patients, yet increasing evidence links chronic PPI exposure to infectious, skeletal, and thromboembolic complications. While associations have been reported following hip, knee, and spine surgery, the impact of PPI use on outcomes after total elbow arthroplasty (TEA) remains poorly characterized. METHODS: We performed a retrospective propensity-matched cohort study using the TriNetX Global Collaborative Network. Adult patients undergoing primary TEA were stratified by pre-operative PPI exposure wi

Robot-assisted percutaneous pelvic channel screw placement for unstable pelvic ring injuries: a systematic review and meta-analysis with narrative technical comparison of multi-platform differences.

Percutaneous pelvic channel screw fixation is a key technique for the minimally invasive treatment of pelvic ring injuries. Robotic-assisted systems aim to improve accuracy and reduce radiation, but direct comparative studies across platforms are lacking. This systematic review and meta-analysis primarily compares robot-assisted versus conventional fluoroscopy-guided pelvic channel screw placement, with a secondary narrative comparison of technical differences across TiRobot, Mazor X, and ROSA platforms. We systematically searched PubMed, Web of Science, Embase, Cochrane Library, CNKI, Wanfang

Outcome of arthroscopic ACL avulsion fixation using adjustable Endobutton: a case series report of new surgical technique.

INTRODUCTION: Anterior cruciate ligament (ACL) avulsion fractures, also referred to as tibial spine fractures, are increasingly encountered in both pediatric and adult populations. Although multiple fixation techniques have been described, the optimal surgical approach - particularly for displaced and comminuted fractures - remains controversial. Arthroscopic fixation using an adjustable-loop cortical Endobutton has emerged as a minimally invasive and potentially reliable option. METHODS: This case series includes five consecutive male patients (aged 15-31) with displaced ACL avulsion fractur
