Spontaneous regression of post-traumatic syringomyelia: longitudinal mr imaging demonstrating a dynamic disease course.

INTRODUCTION: Posttraumatic syringomyelia is a recognized complication of spinal cord injury, often associated with progressive neurologic decline and surgical management. Its natural history remains incompletely understood, and spontaneous resolution is rare. CASE PRESENTATION: We performed a literature review identifying six previously reported cases of spontaneous radiologic resolution of posttraumatic syringomyelia and report a 69-year-old patient with posttraumatic cervicothoracic syringomyelia showing a dynamic course characterized by initial mild enlargement followed by spontaneous rad

[Arachnoid web presenting as tetraparesis].

Arachnoid webs are rare intradural lesions which disrupt cerebrospinal fluid flow and may cause spinal cord compression. This is a case report of a previously healthy 19-year-old woman who presented with acute back pain and rapidly progressive tetraparesis. MRI revealed cervical spinal cord ischaemia and an arachnoid web at the thoracic level. Surgical removal led to partial neurological recovery. Early recognition is essential, as timely surgery may prevent irreversible injury.

Case Report: Beyond chronicity: sustained multidimensional response to spinal cord stimulation three decades after traumatic spinal cord injury.

INTRODUCTION: Spinal cord stimulation (SCS) is an established treatment for refractory neuropathic pain conditions, yet evidence for its effectiveness in spinal cord injury (SCI)-related neuropathic pain, particularly after decades of persistent symptoms, remains limited. It remains unclear whether unusually prolonged pain chronicity and established neurological deficits affect the potential for a meaningful response to neuromodulation. CASE PRESENTATION: We report a 60-year-old man with chronic incomplete SCI, bilateral spastic paraparesis markedly more pronounced on the left, and severe chr

Chronic post-traumatic stress disorder-related behavioral traits develop in female rats exposed to repetitive low-level blast injury and are reversible with (2R,6R)-hydroxynorketamine.

INTRODUCTION: Many military Veterans who experienced blast-related injuries during military service suffer from chronic cognitive and mental health problems. Women constitute more than 15% of active-duty U. S. military personnel and are the fastest-growing population of U. S. Veterans. For more than a decade, we have studied a rat model of repetitive low-level blast exposure designed to mimic the type of blast-related exposure that is common in military settings. Male rats develop chronic cognitive and post-traumatic stress disorder-related traits that appear in a delayed fashion and persist f

Neurosurgical manifestations and management of mucopolysaccharidoses: A systematic review.

Mucopolysaccharidoses (MPS) represent a group of rare inherited lysosomal storage disorders, of which neurosurgical issues arise during childhood and adolescence. The literature however remains fragmented in terms of these specific outcomes. Correspondingly, we sought to synthesize a primer for neurosurgeons in regard to MPS, summarizing the neurosurgical manifestations and management reported in the contemporary literature. A systematic interrogation of the literature was performed according to PRISMA guidelines. Electronic databases were searched from inception through June 2026. Studies rep

Individualizing Diagnosis and Management of Stress Urinary Incontinence in Neurogenic Lower Urinary Tract Dysfunction.

PURPOSE OF REVIEW: Stress urinary incontinence (SUI), defined as involuntary leakage with effort, exertion, sneezing, or coughing, remains a significant cause of morbidity. While well characterized in the general population, SUI in the setting of neurogenic lower urinary tract dysfunction (NLUTD) poses unique diagnostic and management challenges. This review outlines sex-specific epidemiology, diagnostic strategies, and evolving management approaches for SUI in NLUTD, emphasizing individualized and mechanism-based care. RECENT FINDINGS: Incontinence prevalence varies by neurologic diagnosis,

Application of cervical epidural blood patch for spontaneous cerebrospinal fluid leaks: Two case reports.

Spontaneous cerebrospinal fluid (CSF) leaks are uncommon but significant causes of orthostatic headaches, often resulting from dural injuries. Although lumbar epidural blood patches (EBPs) are commonly used to treat such leaks, cervical EBPs are rarely performed due to the higher risk of complications, including spinal cord injury. This case series presents two patients with spontaneous cervical CSF leaks successfully treated with cervical EBPs under fluoroscopic guidance. The first case involved a 24-year-old female who presented with a two-month history of severe orthostatic headaches and ti

Association of Glymphatic Imaging Markers With Clinical Outcomes and Cerebral Small Vessel Disease Burden After Intracerebral Hemorrhage.

OBJECTIVE: The glymphatic system plays a critical role in cerebral metabolite clearance and white matter injury. However, its clinical relevance in intracerebral hemorrhage (ICH) remains poorly understood. This study aimed to investigate imaging-based glymphatic markers in relation to functional outcomes and cerebral small vessel disease (CSVD) burden in patients with ICH. METHODS: We included 70 patients with acute supratentorial ICH who underwent diffusion tensor imaging (DTI). Glymphatic-related imaging parameters were derived using magnetic resonance imaging (MRI), including DTI-ALPS, fre

Painless Spontaneous Spinal Epidural Hematoma Mimicking Acute Ischemic Stroke: A Case Report.

AIM: Spontaneous spinal epidural hematoma is a rare neurological emergency. Its painless variant is particularly uncommon and can easily be misdiagnosed as acute ischemic stroke, especially in patients receiving anticoagulant therapy. This study aims to enrich the clinical recognition of painless spontaneous spinal epidural hematoma and emphasize its differential diagnosis from acute stroke in high-risk medicated patients. CASE PRESENTATION: We report a rare case of painless spontaneous spinal epidural hematoma mimicking acute ischemic stroke in a woman in her early 60s receiving anticoagulat

Abdominal Clonus as a Rare Visceromotor Manifestation in Spinal Cord Injury: A Case Report.

Spinal cord injury can lead to reorganization of spinal reflex circuits and loss of supraspinal modulation of autonomic and somatic functions. Although bladder distension is a well-recognized trigger of autonomic dysreflexia, associated somatic motor manifestations are rarely described. We report the case of a 69-year-old man with a long-standing traumatic complete C6 spinal cord injury and chronic neurogenic bladder. Neurological classification according to the International Standards for Neurological Classification of Spinal Cord Injury (ISNCSCI) was American Spinal Injury Association (ASIA)

Surgery to treat autonomic dysreflexia due to cubital tunnel syndrome in cervical spinal cord injury: a case report.

INTRODUCTION: Autonomic dysreflexia (AD) is a potentially fatal syndrome in people with spinal cord injury (SCI) marked by episodic hypertension with flushing, diaphoresis, and bradycardia. Common etiologies include bladder distension and bowel impaction. Compressive neuropathies lack attention as a potential precipitant of AD. This report describes an individual with cervical SCI and with recurrent AD treated with ulnar nerve decompression surgery. CASE PRESENTATION: A 74-year-old woman with a remote history of C6 SCI (American Spinal Injury Association Impairment Scale Grade B) presented to

A Novel Case Report of Closed-Loop Spinal Cord Stimulation for Neuropathic Pain in Cervical Spinal Cord Injury.

BACKGROUND: Neuropathic pain after spinal cord injury (SCI) is common and often refractory. Closed-loop spinal cord stimulation (CL-SCS) is a novel intervention that adjusts stimulation based on evoked compound action potentials in real time, but its use for pain in complete SCI is not well described. CASE REPORT: A 45-year-old man with complete C6-C7 SCI developed severe, diffuse neuropathic pain below the injury level refractory to multiple medications, an intrathecal fentanyl pump trial, and ketamine infusions. With extensive epidural scarring, a staged buried CL-SCS trial with a combined

Reframing traumatic brain injury and spinal cord injury as chronic multisystem disorders: toward integrated, long-term care.

Traumatic brain injury (TBI) and spinal cord injury (SCI) affect more than 220 million people worldwide and are associated with cardiometabolic, endocrine, neurological, and psychiatric sequelae for years to decades after an acute injury. However, most health systems still treat neurotrauma (TBI and SCI, collectively) as a time-limited event, leaving long-term surveillance of these patients fragmented or absent, which leads to preventable mortality and avoidable readmissions. TBI and SCI in adults should be reclassified as acute-onset multisystem chronic-risk conditions. Existing cardiovascula

Treadmill Exercise Can Modulate the Inflammatory Microenvironment and Promote Regeneration After Compressive Spinal Cord Injury in Female Mice.

Traumatic spinal cord injury (SCI) leads to severe sensorimotor impairments and reduced quality of life. Physical rehabilitation has emerged as a key therapeutic strategy to improve function and mitigate complications such as muscle atrophy and joint contractures, partly through modulation of the inflammatory microenvironment. This study investigated the effects of a structured exercise protocol on inflammation, neuroplasticity and sensorimotor recovery following compressive SCI in mice. Female C57BL/6 mice (8-12 weeks) were subjected to extradural spinal cord compression and divided into thre

Thoracic and retroperitoneal lymphatic injuries following spine surgery: illustrative cases.

BACKGROUND: Postoperative lymphatic leakage is a rare complication of spine surgery caused by injury to the thoracic duct, cisterna chyli, or retroperitoneal lymphatic channels. Because of its rarity, diagnosis is often delayed and optimal management remains poorly defined. The authors present 3 illustrative cases and review current diagnostic and therapeutic strategies. OBSERVATIONS: A 66-year-old man developed a right chylothorax after posterior T2-3 corpectomy with costotransversectomy and cervicothoracic fusion for metastatic spinal cord compression. Following failed thoracic duct emboliz
