Consensus-based criteria for actionable hemorrhage in pediatric trauma: A Delphi study.

BACKGROUND: Blood transfusion and hemorrhage control procedures can be unreliable surrogates for bleeding requiring intervention in children. Some receive unnecessary blood transfusions or lack intraoperative findings for hemorrhage, while others die before an intervention occurs. Standardized criteria for adjudicating the presence of actionable hemorrhage are needed. We aimed to define expert consensus criteria for retrospectively identifying actionable hemorrhage within 6 hours of emergency department (ED) arrival. METHODS: Experts from six specialties involved in pediatric trauma care part

Obstructive Sleep Apnea: Epidemiology, Pathophysiology, Complications, Diagnosis, Management, and Emerging Fibrosis-Linked Remodeling.

Obstructive sleep apnea (OSA) is a highly prevalent yet substantially underdiagnosed sleep-related breathing disorder increasingly recognized as a heterogeneous systemic disease rather than a purely mechanical consequence of recurrent upper-airway collapse. This review provides an updated and integrated overview of OSA, spanning epidemiology, multifactorial pathophysiology, heterogeneous clinical presentation, major cardiometabolic and end-organ consequences, diagnostic principles, and evolving personalized management, while also emphasizing the limitations of conventional severity classificat

Delayed Right Hemidiaphragm Rupture After Blunt Trauma Repaired With Prolene Mesh: A Case Report.

Traumatic diaphragmatic rupture (TDR) is a rare but serious injury. It most commonly results from high-impact blunt trauma. Right-sided ruptures are particularly uncommon due to the protective effect of the liver. Delayed presentation can cause diagnostic challenges, as the injury may remain clinically silent for years before complications such as organ herniation. We report a case of a 65-year-old male who presented with progressive dyspnea, right-sided chest pain, and right upper quadrant abdominal discomfort 4 years after a severe motor vehicle accident. Chest radiography revealed a heterog

Beyond the Lungs: Extrapulmonary Effects of Non-Invasive and Invasive Ventilation Strategies.

Background/Objectives: Non-invasive respiratory support and invasive mechanical ventilation are critical interventions that can induce significant changes not only in the lungs but also in extra-pulmonary organs, which are often overlooked. Understanding the extra-pulmonary effects of non-invasive respiratory support and invasive mechanical ventilation is crucial since it can help prevent or mitigate complications and improve outcomes. This narrative review explores these consequences in detail and highlights areas that require further research. Main Text: Non-invasive respiratory support and

Research on the correlation between rib fracture characteristics and the risk of intrathoracic and intra-abdominal injuries.

PURPOSE: Intrathoracic and intra-abdominal injuries in patients with rib fractures are often overlooked, leading to delayed and ineffective treatment. However, the relationship between rib fractures and organ damage has been rarely studied. The purpose of this study was to analyze the risk factors associated with intrathoracic and intra-abdominal injuries in patients with rib fractures. METHODS: This retrospective observational study included 1269 patients diagnosed with rib fractures from September 2020 to April 2023. Patient data were collected, including gender, age, body mass index, systo

[Treatment strategy for severe implosion injuries of the lateral chest wall].

In the majority of cases implosion injuries to the thoracic wall are caused by blunt, massive force acting on the thorax. Basically, different regions and directions of the acting energy have to be taken into account. In common usage, the term implosion injury has become established, especially for the sequelae of lateral energy impact. Particular attention should be paid to the stability of the shoulder girdle, the underlying hemithorax and its intrathoracic organs.

Combined Rupture of the Diaphragm and Urinary Bladder in Blunt Trauma Abdomen.

The authors examined a 25-year-old man with a combined rupture of the diaphragm and urinary bladder following blunt trauma to the abdomen. The presence of hematuria, suprapubic tenderness, and elevated serum urea and creatinine levels in this patient raised suspicion of urinary bladder rupture. Documentation of bowel gas shadows on the chest x-ray suggested underlying diaphragm injury. A computed tomogram of the thorax and abdomen confirmed the tear in the left hemidiaphragm with intrathoracic herniation of abdominal contents; however, it failed to detect the intraperitoneal urinary bladder ru

Transdiaphragmatic intercostal herniation in the setting of trauma.

Transdiaphragmatic intercostal herniation is a rare injury that can be associated with blunt trauma. Since its first documentation within the literature in 1946, there have been less than 50 cases reported. We present a case involving a 56-year old female who presented to our Trauma Center with transdiaphragmatic intercostal herniation caused by blunt trauma from a high-velocity T-bone vehicular collision. Upon presentation, she exhibited bilateral breath sounds; however, with labored breathing, chest pain, and hypoxia. The initial chest radiograph interpretation indicated the presence of "lef

The brain of fetuses with congenital diaphragmatic hernia shows signs of hypoxic injury with loss of progenitor cells, neurons, and oligodendrocytes.

Congenital diaphragmatic hernia (CDH) is a birth defect characterized by incomplete closure of the diaphragm, herniation of abdominal organs into the chest, and compression of the lungs and the heart. Besides complications related to pulmonary hypoplasia, 1 in 4 survivors develop neurodevelopmental impairment, whose etiology remains unclear. Using a fetal rat model of CDH, we demonstrated that the compression exerted by herniated organs on the mediastinal structures results in decreased brain perfusion on ultrafast ultrasound, cerebral hypoxia with compensatory angiogenesis, mature neuron and

Final Lifelines: The Implications and Outcomes of Thoracic Damage Control Surgeries.

INTRODUCTION: There is a lack of data on the outcomes of thoracic damage control surgery (TDCS). This study aimed to describe the characteristics and outcomes of patients undergoing TDCS. METHODS: This is a retrospective analysis of the American College of Surgeons-Trauma Quality Improvement Program database (2017-2021). All trauma patients who underwent emergency thoracotomy and packing with temporary closure were included. Patients were stratified based on the age groups (pediatric [<18 y], adults [18-64 y], and older adults [≥65 y]). Our primary outcome measures included 6-h, 24-h, and in-

[Outcomes of Surgical Treatment for Chest Trauma].

BACKGROUND: Intra-thoracic organ bleeding and chest wall injury following chest trauma can easily lead to life-threatening emergencies and a delay in treatment may lead to fatal outcomes. Interestingly, the optimal timing, indications, and surgical techniques have not been standardized. METHOD: We retrospectively analyzed 35 patients who underwent surgical treatment for chest trauma. RESULTS: All patients with penetrating trauma (n=4) underwent emergency surgery for a hemothorax. There were no postoperative complications or hospital deaths. All patients with blunt trauma( n= 31) had multiple

Needle Decompression Complicated by Cardiac Injury in a Prehospital Environment.

Needle decompression is a mainstay intervention for tension pneumothorax in trauma medicine. It is used in combat and prehospital medicine when definitive measures are often not available or ideal. It can temporarily relieve increased intrathoracic pressure and treat a collapsed lung or great vessel obstruction. However, when done incorrectly, it can result in underlying visceral organ and vessel trauma. This is a case of an adult male who presented to the emergency department after sustaining multiple stab wounds during an altercation. On arrival, the patient had a 14-gauge angiocatheter inse

[Not Available].

In this case report, a 19-year-old woman suffered a high-energy trauma to her left side with multiple left-sided fractures, including ribs 8-12, with no pneumothorax on the left side, but a large right-sided pneumothorax. Contralateral pneumothorax is extremely rare in primarily unilateral trauma, especially when no contralateral rib fractures are present. A possible explanation is that of a barotrauma due to increased intrathoracic pressure with external compression. Careful examination of both sides of the body is necessary when evaluating patients with unilateral trauma, as serious organ in

[Delayed Massive Hemothorax due to Non-absorbable Sutures Tips Used in Diaphragm Reconstruction for Thymoma Dissemination].

A 57-year-old man underwent partial resection of the right diaphragm with invasive thymoma dissemination. Fifteen days after surgery, he suddenly developed right-sided chest pain with dyspnea and was raced to the hospital. Chest computed tomography (CT) showed a massive right hemothorax, and emergency surgery was performed due to hemodynamic shock. Thoracotomy revealed intercostal artery bleeding from a pinhole injury caused by a non-absorbable monofilament suture tip of the diaphragm. Hemothorax after thoracic surgery is a serious complication, and surgical materials with sharp edges may caus

Intrathoracic Needle Migration from the Mediastinum into the Thoracic Cavity.

Intrathoracic needles are rarely used in clinical practice. They can migrate within the body, injure large blood vessels and other organs, and cause severe complications. We report an interesting case of intrathoracic needle removal using video-assisted thoracoscopic surgery. The needle was inserted under the left clavicle, penetrated the mediastinum, and migrated into the right thoracic cavity. Although pneumothorax developed during the disease course, no severe complications were observed. This rare case illustrates the course of needle migration from the mediastinum into the thoracic cavity

ISOLATED SCAPULA FRACTURE WITH INTRATHORACIC DISLOCATION: A CASE REPORT AND LITERATURE REVIEW.

Scapula fractures are very rare, resulting from high-energy trauma and are mostly associated with other fractures of the locomotor system and injuries to the head, chest or abdominal organs. Most often, they are treated conservatively and the indications for surgical treatment are not yet clearly agreed. One of the conditions requiring surgical treatment is a scapula fracture with fragment migration into the thoracic cavity. In this paper, we are presenting an extremely rare, isolated scapula fracture, without injury to other parts of the locomotor system, with an intrathoracic fragment migrat

Analysis of factors associated with operative difficulty in thoracoscopic esophageal cancer surgery in the left-decubitus position: a single-center retrospective study.

BACKGROUND: The degree of difficulty in the overall procedure and forceps handling encountered by surgeons is greatly influenced by the positional relationship of intrathoracic organs in minimally invasive esophagectomy. This study aimed to identify the anatomical factors associated with the difficulty of minimally invasive esophagectomy assessed by intraoperative injuries and postoperative outcomes. METHODS: Minimally invasive esophagectomy in the left-decubitus position was performed in 258 patients. We defined α (mm) as the anteroposterior distance between the front of the vertebral body a
