Iatrogenic ruptures of the tracheobronchial tree.

We did a retrospective study in 12 patients with iatrogenic tracheal or tracheobronchial ruptures treated since 1975. Ten female subjects, one male subject, and one child (age range, 8 to 72 years), all of whom had undergone intratracheal intubation, were admitted to the hospital. Four patients had been intubated with a double-lumen catheter (two Carlens type with carinal spur, two Robertshaw without spur), and seven had had "high volume-low pressure" tubes, placed under emergency conditions in three of those seven cases. In one further case, an unsuccessful attempt of percutaneous tracheostom

[Anterior spinal fusion by thoracoscopy. A non-traumatic technique].

PURPOSE OF THE STUDY: Video assisted thoracic surgery (VATS) is a new modality which allows visualization of, and access to the intrathoracic organs without thoracotomy. Recently, this technique has been used for anterior thoracic spine approach to perform surgery which previously required standard postero-lateral thoracotomy. The authors report their initial experience of anterior spinal fusion using thoracoscopy and give a detailed description of their surgical procedure. MATERIAL AND METHODS: This technique, started on June 1993, was performed only in one level 1 in 10 patients who had tho

[Diagnosis and therapy of diaphragmatic rupture after blunt thoracic and abdominal trauma].

Diagnosis of diaphragmatic rupture is often missed after blunt thoracic and abdominal injuries because diaphragmatic injury does not play an important role beside severe injuries of intrabdominal and/or thoracic organs. Between 1976 and 1993, 141 patients were treated for traumatic injury of the diaphragm. In 42 patients with penetrating injuries following stab or shot wounds diaphragmatic lesions were diagnosed by the emergent surgical therapy. 99 had diaphragmatic tears from blunt thoracic or abdominal trauma by accidents. 14 of 99 patients sustained isolated diaphragmatic rupture, in 85 the

[A case of giant bronchogenic cyst complicated with postoperative esophageal perforation].

A 42-year-old man with giant bronchogenic cyst occupied from the middle and posterior mediastinum was treated surgically. The tumor was stoutly adhered to anterior aspect of the esophagus and membranous portion of the right bronchus. In dissecting the tumor, muscular coat of the esophagus was partly severed, which was repaired interruptedly with absorbable suture material. However, esophageal perforation was detected by barium swallow on the 3rd postoperative day. Conservative therapy included intrathoracic drainage and antibiotics was successfully undertaken. It is imperative to dissect the t

Traumatic rupture of diaphragm.

Traumatic diaphragmatic rupture remains a diagnostic challenge, and associated injuries determine the outcome in those diagnosed early, whereas that of latent cases is dependent on the consequence of the diaphragmatic rupture: namely, the diaphragmatic hernia. To analyze the clinical and radiologic features and the therapeutic implications, we reviewed 980 patients reported in the English-language literature. This injury affects predominantly males (male:female = 4:1) in the third decade of life, and is often caused by blunt trauma (75%). There were 1,000 injuries, of which 685 (68.5%) were le

Acute diaphragmatic rupture after blunt trauma.

In a retrospective analysis of 20 patients operated on for acute diaphragmatic rupture following blunt trauma, 85% of the patients presented with shortness of breath, and 85% had signs of peritoneal irritation or abdominal distension on physical examination. Preoperatively, a diaphragmatic rupture was diagnosed or suspected in 80%, and was based on chest X-ray examination alone or in combination with other radiological studies in all but one case. The diaphragmatic rupture was left-sided in 70%, right-sided in 20%, and bilateral in two cases. Intrathoracic herniation of abdominal organs occurr

[The characteristics of the intrathoracic complications of suppurating echinococcal liver cysts].

Using the material from 48 patients with intrathoracic complicated festered liver echinococcus cysts, the variations of break and the localization of connections between festered liver echinococcus cysts and thoracic organs were characterized. The pathological processes in the organs of the pleural cavity were described. A classification of intrathoracic complications proposed by the authors permits one to avoid diagnostical and tactical mistakes and to decrease human mortality.

Determinants of outcome in lesions of the thoracic aorta in patients with multiorgan system trauma.

Of all patients presenting at our level 1 trauma center with multiorgan system injuries, 33 have been identified with acute lesions of the thoracic aorta. Mean severity injury score was 24 +/- 3. Four patients underwent resuscitative thoracotomy upon arrival in the emergency department. One survived and fully recovered. The rest underwent diagnostic procedures and repair of aortic lesions in conjunction with surgical treatment of other injured organ systems. The overall survival rate was 82 percent. Survivors arrived significantly faster to the ED and had lesser degree of multiorgan system inj

[Multiple organ failure. Reflection of generalized cell damage of all organs following severe trauma].

Multiple organ failure (MOF) is presently recognized as the most severe, and often lethal, complication after multiple trauma. Causal factors and pathomechanisms remain unclear, however. Generalized inflammatory cell tissue injury with a subsequent increase in permeability in all organs has been suggested. For this reason, 38 polytraumatized patients were examined in a prospective study. Organ function was analyzed, and specific clinical and histological studies were performed to check for generalized cell tissue damage and increased respiratory permeability. In all organs we found signs of ti

Management of empyema thoracis.

Over a 5-year period, 65 adult patients with empyema thoracis were treated. The cause of the empyema was postpneumonic in 52%, postresectional in 24%, a complication of minor surgical procedures in 14%, posttraumatic in 5%, and the result of miscellaneous causes in 5%. In the postpneumonic group, infection resulted from a single aerobic organism in 41%, multiple aerobic organisms in 9%, single anaerobic organisms in 12%, and mixed aerobic-anaerobic organisms in 18%. Peptostreptococcus, Streptococcus viridans, Staphylococcus epidermidis, Peptococcus, Staphylococcus aureus, and diphtheroids were

[Dangers and errors in pleural puncture and placing suction drainage].

Mistakes during puncture and drainage of the pleural space can result in serious complications. A few case reports are presented. Injuries of intrathoracic and intraabdominal organs can occur-, the latter mainly due to underestimating the elevated level of the diaphragm. Trauma of the liver and spleen are mainly observed in such cases. To avoid such life-threatening complications "blind" puncture and drainage should be avoided and visual aids such as fluoroscopy, CT and ultrasonography should be used more frequently.

[Clinical features of traumatic diaphragmatic injuries with or without herniated visceral organs].

The difficulty of diagnosis and selection of operative approach in traumatic diaphragmatic injuries (T. D.I.) have been debated for a long time. The reason why these problems remain unsolved is possibly due to the inaccuracy in the definition of T.D.I. We propose to classify the T.D.I. into two categories; diaphragmatic rupture without herniated visceral organs (D.R.) and diaphragmatic hernia with herniated visceral organs (D.H.). Clinical features in these two groups (D.R. in 9 cases and D.H. in 11 cases) were compared and discussed as follows: 1) Intrathoracic injuries were associated in all

Organ procurement for pulmonary transplantation.

Selection of suitable donors is critical to the success of clinical pulmonary transplantation. Requirements for lung donors, management before explantation, and methods of preservation were reviewed for the 70 heart-lung, eight double-lung, and two single-lung transplantations performed at the University of Pittsburgh since 1982. Careful observation of trends of hyperoxygenation studies, chest roentgenograms, and Gram stain and culture results of tracheal secretions, as well as findings on bronchoscopy, can help identify which lungs not only have adequate function but are acceptable for transp

[Intrathoracic hemorrhage of the intra-abdominal organs following diaphragmatic rupture. A rare cause of hemothorax].

After posttraumatic diaphragm ruptures in some cases life threatening haemorrhages from intraabdominal organs (especially from the spleen) into the thorax were found. In general, the diagnosis cannot be established by peritoneal lavage or abdominal sonography. And in case of emergency, there is often not enough time to obtain a computed tomography. It is therefore extremely important for the patient's prognosis that the surgeon initially performing treatment be aware of this cause of bleeding. Correct interpretation of the chest x-ray, for instance in the case of stomach air in a left basal sh

Resection of advanced stage neuroblastoma with the cavitron ultrasonic surgical aspirator.

Current protocols for the treatment of neuroblastoma emphasize total or near total resection of tumor to improve survival. This is preferentially performed as a primary procedure, or is attempted at a second-look operation. Unfortunately, this tumor often grows to large size with invasion of the spinal canal, or encasement of major vascular or other retroperitoneal structures. A primary attempt at complete removal may result in difficult-to-control hemorrhage or injury to, or loss of, vital organs. A second-look procedure carries other intrinsic risks. It often must be performed during a perio

Management of traumatic pneumothorax.

In road accident trauma, pneumothorax can be 'open' (associated with a penetrating chest wound) or 'closed'; either can be associated with fractured ribs or development of intrapleural tension. Tension and open pneumothorax are recognised easily and their correction at the scene of the accident can be lifesaving. Any accompanying major intrathoracic and subcostal organ damage should be assessed.

Prevention of central nervous system infections in patients at risk.

Infections of the central nervous system in patients at risk are relatively uncommon when compared with pneumonia, bacteremia, and soft tissue infection. However, they carry serious morbidity and are frequently fatal. Each of the diverse conditions that can place a patient at risk for central nervous system infection is associated with a fairly predictable spectrum of etiologic organisms. Various forms of trauma (including blunt and penetrating injuries and neurosurgery, especially when a cerebrospinal fluid shunt is implanted) predispose to infection with common pathogenic bacteria. Defects o

[Traumatic lesions of the diaphragm (personal experience)].

From 1971 to 1982, 126 blunt thoraco-abdominal trauma were seen: in 9 cases (7,1%), diaphragmatic rupture was present. Automobile accidents were the most frequent cause of diaphragmatic rupture and, in only one case, the injury was single. Diaphragmatic rupture was suspected preoperatively in two patients, who had positive physical and radiologic chest findings. The lesion was seen intraoperatively in the other patients, who underwent surgical operation because of severe intra-abdominal injuries. In the author's experience, roentgenologic examinations and adjuntive studies were not important f

Unusual causes of spontaneous pneumoperitoneum.

Spontaneous pneumoperitoneum may pose a diagnostic dilemma, especially when evaluating a patient who has no or minimal abdominal or constitutional findings accompanying its presence. The free intraperitoneal air may be the result of a perforation of a hollow viscus in such instances. Under these circumstances, the air is most often from another source. The more common sites of origin are intrathoracic sites, air in the wall of the intestinal tract and the genital organs in women. Diagnostic and therapeutic procedures in or adjacent to the abdomen may result in an iatrogenic pneumoperitoneum. I

[Early thoracotomy and chest wall stabilization with elastic rib clamps (author's transl)].

In patients with chest injuries, serial rib fractures and intrathoracic lacerations the indication for primary thoracotomy is usually limited to life-threatening organ ruptures. In our own experience with 18 patients primary thoracotomy offers the chance of immediate closure of lung lacerations, suture of pleural and pericardial tears, ligature of torn intercostal vessels and stabilization of the chest wall by elastic rib clamps "Essener Model". Postoperative respiration, freedom of pain, is restored immediately and therefore complications and duration of high-risk respirator treatment can be
