Evaluation of the Intraparotid Branching Patterns of the Facial Nerve During Parotidectomy: A Clinical Study Based on the Davis and Alomar Classifications.

BACKGROUND: The branching patterns of the facial nerve (FN) vary among individuals, which may affect the risk of surgical complications. AIM: The aim of this study is to examine the relationship between FN branching patterns and clinical variables including facial paralysis in patients undergoing parotidectomy in our clinic and to reveal the region-specific anatomical distribution of the FN. METHODS: Data from 39 patients who underwent parotidectomy were analyzed. In all cases, the main trunk of the FN was located at the exit of the stylomastoid foramen and dissected. Intraoperative nerve mo

TREATMENT OF A PATIENT WITH SEVERE HIGH-VOLTAGE ELECTRICAL INJURY: A CLINICAL CASE.

A 32-year-old patient sustained a high-voltage electrical injury while touching live parts of railway equipment. Examination revealed electrical burns of mixed depth covering a total area of 42%, 25% of which were deep grade IIIB burns. The affected area included the face, neck, torso, and right extremities. The Frank burn index was 92 units, consistent with an extremely severe burn injury. Laboratory tests revealed a pronounced systemic inflammatory response, hypoproteinemia, signs of metabolic stress, and activated coagulation. Bilateral lower lobe pneumonia and retinal angiopathy were also

[Comparative efficacy of island latissimus dorsi myocutaneous flap versus lower trapezius myocutaneous flap for repair of posterior neck skin and soft tissue defects].

OBJECTIVES: Posterior neck skin and soft tissue defects often result from electrical burns, post-tumor resection, infections, or trauma. The region has complex anatomy, high mobility, and proximity to the cervical spine, major vessels, and critical nerves; thus, reconstruction must balance functional restoration and aesthetic outcomes. The island latissimus dorsi myocutaneous flap, with its reliable blood supply, large harvestable area, and long rotation arc, is widely used for trunk and limb reconstruction and shows particular advantage for large defects in the thoracic, dorsal, and posterior

Lightning Strike-Induced Cutaneous Burns and Airway Injury: A Case Report Highlighting Recurrent Extubation Failure in a Critically Ill Patient.

INTRODUCTION: Lightning strikes cause a unique spectrum of cutaneous burn injuries that differ substantially from conventional thermal or high-voltage electrical burns. Despite the well-documented systemic lethality of lightning injuries, the burn-specific sequelae-including airway involvement and its contribution to ventilatory failure-remain underreported in the burn literature. CASE REPORT: We present a 31-year-old woman who sustained multiple cutaneous burns following a lightning strike, including a circumferential necklace-pattern burn to the neck, a large midline torso burn, and burns t

Perforator-Aware Explainable Microsurgical Intelligence for Basilar Trunk Aneurysm Reconstruction: A Human-Centred Clinical AI Framework for Precision Neurovascular Surgery-A Retrospective Single-Centre Study.

Basilar trunk aneurysms (BTAs) are rare intracranial aneurysms. These aneurysms pose substantial clinical risk because they are located close to vital cranial structures and perfusion-sensitive perforating arteries. In addition, outcomes from BTA repair are dependent upon several factors, which include proximity to the brainstem, preservation of perforators, corridor access to the aneurysm, successful clipping of the aneurysm through reconstruction, the need for intraoperative rescue manoeuvres and the ability of the patient to recover from complications. Many previous studies have documented

Ultrasound-guided infraspinatus-teres minor plane block for acute post traumatic shoulder pain in the emergency department: A case series.

Acute traumatic shoulder injuries are often associated with severe pain that hinders examination, radiographic positioning, and timely orthopaedic management. Conventional systemic analgesia may be inadequate, while interscalene and superior trunk blocks can be limited by hemidiaphragmatic paresis. The ultrasound-guided infraspinatus teres minor (ITM) inter-fascial plane block is a recently described diaphragm-sparing technique that targets the suprascapular and axillary nerves through a single posterior injection and has shown promising anatomic and postoperative analgesic coverage in earlier

Pseudoaneurysm arteria carotis.

A 90-year-old man with type 2 diabetes mellitus, hypertension, and hyperlipidemia was admitted for vascular evaluation due to a progressively enlarging mass on the right side of the neck. Ultrasound examination revealed a pseudoaneurysm at the bifurcation of the right common carotid artery. Clinically, a pulsatile, painless mass measuring 6 × 4 cm was present without overlying skin erythema. CT angiography confirmed the diagnosis of a pseudoaneurysm with a thickened wall and a wide neck. Following a multidisciplinary discussion, the patient was indicated for an endovascular procedure. A stent

Long-segment cervicothoracic spinal cord injury occurring after cerebral angiography: a case report.

BACKGROUND: Spinal cord injury (CIS) is an exceedingly rare complication of cerebral angiography, with only nine cases since 1962. Pathogenesis remains poorly understood. CASE PRESENTATION: A 67-year-old diabetic woman presented with dizziness and normal neurological examination. During cerebral angiography, repeated injections of ioversol contrast medium (total 100 mL; approximately 20 min at a single site) were performed into the right subclavian artery for vertebral artery imaging due to vascular tortuosity. The angiography showed normal cerebrovascular morphology, without emboli, stenosis

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Brain and body: Early decision making and outcomes for multiply injured patients with penetrating brain injury.

INTRODUCTION: For patients presenting with gunshots to the head and injuries to the neck or torso, head computed tomography (CT) provides essential information for prognosticating and managing brain injury but may delay hemorrhage (HRH) control. We hypothesized that use of preoperative head CT would be common, and clinicians would prioritize neuroimaging over prompt HRH control in patients with multiple penetrating injuries at high risk for HRH. METHODS: Using a statewide trauma database to identify patients who sustained gunshot wounds (2017-2021) with both penetrating brain injury and penet

Association of Total Body Surface Area and Body Part With Mental Health Outcomes in Pediatric Burn Patients.

INTRODUCTION: Burn injuries in children are associated with negative impacts on psychological and emotional well-being, yet specific risk indicators are not well researched. We hypothesize that children with greater total body surface area (TBSA) burned and burns in highly visible and functionally significant body parts are more likely to experience negative mental health outcomes compared to their counterparts. MATERIALS AND METHODS: Database review of 178 pediatric burn patients (median age: 13 ± 2.99) utilizing the Burns Model System National Database, retrospectively collected data betwee

Anatomical variations of the recurrent laryngeal nerve and postoperative outcomes in thyroid surgeries conducted at a teaching hospital in Ethiopia.

Thyroid surgery requires a thorough knowledge of the neck anatomy and its anatomical variations. This is of utmost importance since it is well known that variations of the recurrent laryngeal nerve are prone to iatrogenic injuries. Injury to the recurrent laryngeal nerve is one of the most severe complications of thyroid surgery. Surgeons must comprehensively understand the anatomy of the recurrent laryngeal nerve during thyroid operation. To assess the anatomical variations of recurrent laryngeal nerves, with inferior approach using inferior thyroid artery as a consistent anatomical landmark,

Evidence for peripheral neuroinflammation after acute whiplash.

Whiplash injury is associated with high socioeconomic costs and poor prognosis. Most people are classified as having whiplash-associated disorder grade II (WADII), with neck complaints and musculoskeletal signs, in the absence of frank neurological signs. However, evidence suggests that there is a subgroup with underlying nerve involvement in WADII, such as peripheral neuroinflammation. This study aimed to investigate the presence of neuroinflammation in acute WADII using T2-weighted magnetic resonance imaging of the brachial plexus, dorsal root ganglia and median nerve, and clinical surrogate

Impact of buccal and marginal mandibular branch transection on neuromuscular junction integrity in a rodent model of facial nerve paralysis.

BACKGROUND: Facial nerve paralysis is a complex and devastating condition, with translational research primarily relying on animal models. Models include injuries to the main trunk or distal branches of the facial nerve, such as the buccal and marginal mandibular branches. While recovery is well described following main trunk injury, this study investigates the impact of distal branch injuries on facial nerve recovery and neuromuscular junction (NMJ) innervation. NEW METHOD: Adult rats underwent transection of the buccal branch (BB) or of the buccal and marginal mandibular branches (BB+MM) of

Emergency hybrid surgery for transection of pancreas at the head and neck after blunt abdominal trauma: A case report and review of the literature.

INTRODUCTION: A complete disruption of main pancreatic duct (MPD) presents a significant challenge to the surgeon. Historically, the standard surgical approach for addressing a complete disruption of the MPD involved distal pancreatic resection and pancreaticojejunostomy Roux-en-Y anastomosis. Nevertheless, there have been no reported cases of hybrid surgery being employed for the complete disruption of the MPD. PATIENT CONCERNS: A 63-year-old male patient presented with blunt trauma in the upper abdomen and was transferred to our trauma center 10 hours after injury. Upon arrival at the emerg

Repair of Radiation Ulcers After Breast Cancer Surgery With Simple Local Random Flaps.

BACKGROUND: Radiation ulcers that develop after breast cancer surgery are mainly repaired with pedicled flaps or free flaps rather than local random flaps or skin grafts due to large skin defects and poor wound healing. Complicated surgical techniques and donor site reconstruction increase the risk of failure. We report our experience of using the local random long neck reading man flap (LNRMF) to cover large chest wall radiation ulcers, achieving good outcomes. METHODS: Eight patients who developed chest radiation ulcers after breast cancer surgery and were treated between January 2024 and J
