Cervicothoracic arterial injuries: recommendations for diagnosis and management.

Management of arterial injuries at the thoracic outlet and neck presents a major challenge to the trauma surgeon: hemorrhagic shock, neurologic deficit, and limb loss are the serious sequelae. Over a 13-year period, 118 patients with injuries to the innominate, carotid, subclavian, and axillary arteries were evaluated. Most injuries were penetrating (78%). Half of the patients were diagnosed by physical examination and half by angiography. Patients were treated by either primary repair (35%), interposition graft (31%), ligation (8%), or anticoagulation (26%). Two patients required amputations

Effect of continuous versus intermittent turning on nursing and non-nursing care time for acute spinal cord injuries.

The present study was conducted to determine whether automated, continuous turning beds would reduce the nursing care time for spinal cord injured (SCI) patients by freeing hospital staff from manual turning of patients every 2 hours. Seventeen patients were randomly assigned to continuous or intermittent turning and were observed during the 8 hour shift for 1 to 18 days following injury. Trained observers recorded the time taken for patient contact activities performed by the nursing staff (direct nursing care) and other hospital staff. The mean direct nursing care time per dayshift per patie

Brachial plexus traction injuries.

Brachial plexus traction injuries most frequently occur following acute flexion or extension of the neck. The symptomatology following this injury may be defined clearly into the anatomic patterns of upper trunk, lower trunk, posterior cord, medial cord, and lateral cord radiations. This injury is the most common cause of the neurogenic form of thoracic outlet syndrome and is frequently seen in conjunction with cervical spine disease as well as peripheral entrapment syndromes of the ulnar, radial, and medial nerves.

Cervicothoracic vascular injuries. Presentation, management, and outcome.

One hundred consecutive patients with cervicothoracic vascular trauma were analyzed. The injury severity score, mechanism of injury, age, initial findings, management, and results were tabulated. There were 48 arterial and 61 venous injuries in the stable Group A patients, 11 arterial and 12 venous injuries in the unstable Group B patients, and three arterial and five venous injuries in the morbid Group C patients. Treatment included primary repair, resection with end-to-end anastomosis, or ligation. Twenty-three patients developed postoperative complications, the most common being respiratory

Iatrogenic vascular lesions. Pathogenesis and treatment: an 18 year review.

The records of 304 patients operated on for vascular injuries were reviewed; 76 (25%) of these presented an iatrogenic vascular injury: 13 involved the upper limbs, 49 the lower limbs and 14 the neck and trunk. The lesions were due to a vascular catheterism in 40 patients, to orthopaedic treatment in 27 patients, to general surgery procedure in 3 patients, to urologic surgery in 3 and to otorhinolaryngologic surgery in 3. In vascular catheterism the prevalent complications were thromboses and embolisms; 3 patients needed an amputation after the surgical repair. In orthopaedic surgery, vascular

Life-threatening fluid extravasation of central venous catheters.

The majority of life-threatening injuries secondary to the placement of central venous catheters, such as bleeding and pneumothorax, occur at the time of initial insertion. When a catheter extravasates in the neck, edema of the neck wall or chest is usually seen, and the pump indicates occlusion. We present four cases in which an uneventful, successful placement of four central lines (three superior vena cava, one inferior vena cava) were followed at greater than 48 hours by either hydrothorax or hydroperitoneum, which resulted in either cardiorespiratory collapse or intraabdominal sepsis. Cough in tetraplegic subjects: an active process.

STUDY OBJECTIVE: To determine the role of the clavicular portion of the pectoralis major during cough in tetraplegic subjects. PATIENTS: Eight patients with longstanding traumatic transection of the lower cervical cord. METHODS: The electromyographic activity of the pectoralis major and abdominal external oblique muscles and the changes in the anteroposterior diameter of the lower rib cage, the upper rib cage, and the abdomen were measured during voluntary coughing efforts in the seated posture. RESULTS: When coughing, all subjects showed a large amount of electrical activity in the pectora

[War injuries of the thorax. Aggressors and wound balistics].

War chest wounds are very common, but wound balistic notions are not known of physicians. Different threats are being listed, and we might retain the important rate of shrapnels wounds. Concerning the bullet behavior in soft tissue, one can discern full jacketed war bullets which may tumble after a variable "neck", and non jacketed missiles which cause wound through "mushrooming" and/or fragmentation effect. Buckshot wounds obey the rule of "all or none". Body reactions, particularly the clash with a hard material like bone, may overturn everything described in soft tissues. Every kind of poss

Serum alkaline phosphatase and inorganic phosphorus values in spinal cord injury patients with heterotopic ossification.

The blood chemistry was studied in 140 spinal cord injury (SCI) patients (acute injury ward), including 18 patients who developed heterotopic ossification (HO). Comparisons between the HO and non-HO groups were made to determine if the alkaline phosphatase (AP), inorganic phosphorus (P), or calcium (Ca) levels were of diagnostic value. The results showed that AP, P, and Ca by themselves were of little help in the diagnosis of HO. However, the combination of elevated AP and P was significant, especially if both were consistently elevated. There were no significant differences between the HO and

Endovascular occlusive intervention in the management of trauma.

This report summarizes a 10-year experience (1978-1987) in a metropolitan hospital with 102 patients sustaining a variety of complex or inaccessible vascular injuries. Management included the application of occlusive interventional arteriographic techniques. Regional injuries included head and neck (56%), trunk (13%), and extremity (32%). Techniques of vascular occlusion were often performed in conjunction with the initial arteriographic evaluation and were comprised of particulate embolization (42%), placement of mechanical devices (36%), or tissue adhesives (1%), or a combination (21%). Pretransfer tissue expansion of a scalp free flap for burn alopecia reconstruction in a child: a case report.

A nine-year-old boy who sustained severe grease-fire burns over his right scalp, face, neck, and trunk 14 months previously, presented with extensive alopecia over the right frontotemporoparietal scalp. Near complete coverage of the scalp defect was obtained with pretransfer expansion of the left temporoparietal scalp, followed by free-flap transfer of the expanded tissue to the defect. Pretransfer tissue expansion has the advantages of increasing flap surface area and minimizing donor site deformity. This case illustrates the applicability of pretransfer tissue expansion of free flaps, and de

Why is the lower torso protected in traumatic asphyxia? A new hypothesis.

Traumatic asphyxia secondary to a crush injury of the chest is characterized by craniocervical cyanosis, subconjunctival hemorrhage, and severe vascular engorgement of the head and neck. These signs are believed to be due to high venous pressures causing stasis and capillary rupture. A "fear response" that produces a strong Valsalva maneuver is thought to be necessary for their development. The lower torso seems to be protected, and previously this was thought to be due to its superior system of valves. We present here ultrasonographic evidence that the inferior vena cava is compressed or obli

[Diaphragmatic pacing by electrical stimulation of the phrenic nerve--a case report].

A four-year-old boy had traffic accident and was transferred to our hospital with cardiorespiratory arrest, on March 24, 1986. After resuscitation, quadriplegia and apnea were sustained. Computed tomography (CT) scan on the day of admission only demonstrated subarachnoid hemorrhage and plain roentgenograms of the skull and cervical spine showed no abnormalities. He opened his eyes to verbal order one week later, but apnea and quadriplegia sustained. CT scan obtained on that day demonstrated no abnormalities but roentgenogram of the cervical spine showed atlanto-axial dislocation. On May 26, 19

The clinical features of spasms in patients with a cervical cord injury.

A study concerning subjective symptoms of spasms which occur during 24 hours was carried out in 13 quadriplegic patients. All had spasms. Tonic and clonic spasms occurred involving their limbs and trunk. Extensor spasms occurred more often in the lower limbs than in the upper limbs. The duration of spasms was 8 seconds on average and the frequency was 15 times on average per day. The spasms which were severe tended to last longer in duration and were more frequent in occurrence. No spasms occurred without a trigger. The activities in daily living were interfered with by spasms in most patients

Blunt arterial injuries associated with multiple trauma.

In this retrospective study, we reviewed the records of 62 patients who were treated at a tertiary care community teaching hospital between 1977 and 1987 for major arterial injury caused by blunt trauma. Nearly half (45%, 28/62) of the injuries were to the thoracic aorta, 24% (15/62) were to arteries of the head and neck, 21% (13/62) were to the arteries of the extremities, and 10% (6/62) were to abdominal arteries. Twenty-eight thoracic aortic transections were repaired, 25 with Dacron grafts and three by primary repair. One patient developed an adventitial hematoma in the thoracic aorta, whi

Trauma in the Afghan guerrilla war: effects of lack of access to care.

Little or no acute medical care or evacuation capability was available to resistance forces in the Afghanistan War. We examined the effects of these constraints through a review of 1373 patients admitted to a Pakistani border hospital from 1985 to 1987. Most wounds were to the extremities (92%). Serious complications from injuries were found in 41% of cases, of which the leading causes were soft-tissue infections (36%), chronic osteomyelitis (25%), and marked restriction of joint movement (18%). For 201 patients with known cause of injury, the distribution of responsible weapons was similar to

Otologic complications following temporomandibular joint arthroscopy.

The recent application of arthroscopic surgical techniques to the temporomandibular joint (TMJ) has facilitated the diagnosis and treatment of TMJ disorders. However, as TMJ arthroscopy is performed more frequently, new complications are being recognized. We report three patients who developed severe otologic complications following TMJ arthroscopy. Two sustained complete or severe sensorineural hearing loss and severe vertigo from trauma to the ipsilateral ear. The third patient had complete facial paralysis from trauma to the facial nerve in the middle ear and a conductive hearing loss from
