[Tissue expansion in children: Indications and management of complications. A 10-year experience].

INTRODUCTION: Tissue expansion is a plastic surgery technique which enables optimal quality and skin surface reconstruction in case of soft tissue defect. Despite the good results obtained, both from a functional and aesthetic point of view, there is a high rate of complications whose management seems to be essential to ensure satisfactory results. PATIENTS AND METHODS: We retrospectively reviewed the medical files of 45 children treated in our department between 2002 and 2012: 73 expanders were placed during 51 protocols. RESULTS: The studied protocols gathered a large variety of situations

The search for the ideal thin skin flap: superficial circumflex iliac artery perforator flap--a review of 210 cases.

BACKGROUND: The superficial circumflex iliac artery perforator flap is a thin skin flap that can be harvested reliably and quickly from the groin. It is ideal for single-stage resurfacing of cutaneous defects. The donor site heals well and is easily concealed. The authors clarify the anatomy, simplify the flap harvest technique, and outline the modifications to expand the applications of this flap. METHODS: Between January of 2011 and January of 2014, 210 superficial circumflex iliac artery perforator flaps were performed at Asan Medical Center. The flaps were used for head and neck reconstru

Risk and injury severity of obese child passengers in motor vehicle crashes.

OBJECTIVE: To investigate the risk and injury severity on the regional body (head, neck, and chest) of obese children in frontal motor vehicle crashes (MVCs). METHODS: No physical surrogates (i.e., crash dummies) for obese children were available, and experiments on pediatric cadavers were generally not feasible. Therefore, computational models of obese children using medical imaging processing and state-of-the-art modeling techniques were developed. A hybrid modeling technique was used to integrate the finite element model for torso fat layer into the standard multibody model to represent va

[Towards an improvement of the quality of life after radiotherapy in children].

Pediatric radiotherapy did not differ technically from adult radiotherapy but its characteristics, its difficulties and challenges are specific. Thanks in part to this treatment, overall survival of French children with cancer is around 80%. It is therefore important to integrate in the elaboration of the treatment planning potential side long-term effects. Indeed, whatever the technique is, even the most sophisticated, it is usually inevitable that healthy nearby organs receive a dose of radiation. Dose limits on organs at risk come from adult data and are adapted for children. After the trea

Increased and safer detection of nonrecurrent inferior laryngeal nerve after preoperative ultrasonography.

OBJECTIVES/HYPOTHESIS: Right nonrecurrent inferior laryngeal nerve (NRLN) is an anatomical variant reported with a variable prevalence (0.3%-6%). It is associated with some arterial abnormalities (absence of the brachiocephalic trunk and presence of a right aberrant subclavian lusorian artery) that may be identified by preoperative ultrasonography (pUS). NRLN represents a major morbidity risk factor during neck surgery. The aim of this study was to verify pUS accuracy in predicting NRLN and to assess the impact of this technique on NRLN detection rate and laryngeal morbidity. STUDY DESIGN: Re

Smooth Pursuit Eye Movement Deficits in Patients With Whiplash and Neck Pain are Modulated by Target Predictability.

STUDY DESIGN: This is a cross-sectional study. OBJECTIVE: The purpose of this study is to support and extend previous observations on oculomotor disturbances in patients with neck pain and whiplash-associated disorders (WADs) by systematically investigating the effect of static neck torsion on smooth pursuit in response to both predictably and unpredictably moving targets using video-oculography. SUMMARY OF BACKGROUND DATA: Previous studies showed that in patients with neck complaints, for instance due to WAD, extreme static neck torsion deteriorates smooth pursuit eye movements in response

Automatic periodic stimulation of the vagus nerve during single-incision transaxillary robotic thyroidectomy: Feasibility, safety, and first cases.

BACKGROUND: Automatic periodic stimulation (APS) of the vagus nerve during thyroid surgery may provide the patient with increased safety to prevent impending thermal or stretch-related injury to the recurrent laryngeal nerve (RLN). METHODS: Vagal stimulation would be of particular interest in remote access robotic thyroid surgery, where the RLN may be exposed to more conducted heat because of the exclusive use of the Harmonic scalpel. Using APS, the console surgeon may adjust the dissection technique according to the signal changes to avoid impending thermal damage to the nerve. RESULTS: In

Division of the spinal accessory nerve in the anterior triangle: a prospective clinical study.

The anatomical relation between the spinal accessory nerve and internal jugular vein is well documented, but other variants of the nerve, including the contributions of the cervical plexus to supply motor fibres to the trapezius, are less well known. We have previously described an anatomical variant in which the spinal accessory nerve divided before entering the sternocleidomastoid, and the inferior trunk passed directly under it to supply the trapezius. We now present a prospective study of 133 neck dissections (excluding radical dissections) in which a meticulous search was made for the var

Unique case of pneumomediastinum due to penetrating injury to the mediastinum.

Pneumomediastinum is a rare condition, and mostly occurs following traumatic or iatrogenic tracheal and oesophageal perforation, but spontaneous pneumomediastinum has also been described. We report a case of a 17-year-old woman who presented with a penetrating neck wound after a fall down the stairs. She had an extensive pneumomediastinum without signs of tracheal or oesophageal laceration, rib fractures, pneumothorax or haematothorax. The contaminated wound was surgically explored and extensively lavaged. Prophylactic antibiotic treatment was given and the patient recovered without complicati

Interobserver agreement in retrospective chart reviews for factors associated with cervical spine injuries in children.

OBJECTIVES: The objective was to describe the interobserver agreement between trained chart reviewers and physician reviewers in a multicenter retrospective chart review study of children with cervical spine injuries (CSIs). METHODS: Medical records of children younger than 16 years old with cervical spine radiography from 17 Pediatric Emergency Care Applied Research Network (PECARN) hospitals from years 2000 through 2004 were abstracted by trained reviewers for a study aimed to identify predictors of CSIs in children. Independent physician-reviewers abstracted patient history and clinical fi

Age-related differences in factors associated with cervical spine injuries in children.

OBJECTIVES: The Pediatric Emergency Care Applied Research Network (PECARN) investigators previously identified risk factors associated with cervical spine injuries (CSIs) in children. Anatomic maturation and age-related variation in mechanisms of injury suggested the need to explore factors separately for younger versus older children. The purpose of this substudy was to investigate CSI risk factors in age subgroups within the PECARN study cohort. METHODS: This was an age-stratified case-control analysis of children younger than 16 years presenting to 17 PECARN hospitals following blunt traum

Seizure induced polytrauma; not just posterior dislocation of the shoulder.

A 61-year-old woman sustained multiple fractures secondary to the tonic clonic muscular contractions of a seizure. Her injuries included: bilateral proximal humerus posterior fracture dislocations, manubrium fracture, unstable sixth thoracic vertebrae crush fracture, bilateral acetabular fractures and a left femoral neck fracture. Seizures are a rare but recognised cause of fracture. Patients with osteoporosis are more susceptible to more serious fractures to long bones, spine and pelvis. The lack of history of high-energy trauma can lead to a delay in diagnosis. Rarely, the combination and ma

Cost-utility analysis of prehospital spine immobilization recommendations for penetrating trauma.

BACKGROUND: The American College of Surgeons' Committee on Trauma's recent prehospital trauma life support recommendations against prehospital spine immobilization (PHSI) after penetrating trauma are based on a low incidence of unstable spine injuries after penetrating injuries. However, given the chronic and costly nature of devastating spine injuries, the cost-utility of PHSI is unclear. Our hypothesis was that the cost-utility of PHSI in penetrating trauma precludes routine use of this prevention strategy. METHODS: A Markov model based cost-utility analysis was performed from a society per

Characteristic travelling patterns of non-recurrent laryngeal nerves.

BACKGROUND: The non-recurrent laryngeal nerve is subject to potential injury during thyroid surgery. Intra-operative identification and preservation of this nerve can be challenging. Its presence is associated with an aberrant subclavian artery and the developmental absence of the brachiocephalic trunk. This study aimed to evaluate the incidence of non-recurrent laryngeal nerves and present a new classification system for the course of these nerves. METHODS: Non-recurrent laryngeal nerves were identified on the right side in 15 patients who underwent thyroidectomy. The incidence of non-recurr

Nodular Fasciitis of the Breast: A Case and Literature Review.

Nodular fasciitis is a benign fibroblastic proliferation in soft tissue that is most commonly found in the upper extremities, trunk, head, and neck region. Its occurrence in the breast has been rarely reported. The most characteristic features are the sudden appearance and rapid growth of a palpable lesion. Nodular fasciitis can clinically, radiologically, and histopathologically mimic a breast carcinoma. We present a case of nodular fasciitis of the breast and a review of the relevant literature.

Tracheoinnominate fistula: a rare acute complication of penetrating neck injury.

Penetrating injuries in the base of the neck are considered to be the most dangerous due to the potential combination of vascular and intrathoracic lesions. We describe an extremely rare case of combined injury of the trachea and innominate artery, which resulted in formation of a traumatic acute tracheoinnominate fistula. Previously, these fistulas have been described as an iatrogenic complication of tracheostomy, presenting with massive peristomal bleed or hemoptysis. This case demonstrates that a combination of lesions to vital anatomical structures in the neck can change their clinical pre
