Subdeltoid ultrasound-guided brachial plexus block: A technical description with anatomical correlation and a preliminary case series.

Regional anaesthesia in the emergency department requires reliable techniques with minimal complications. We describe a subdeltoid ultrasound-guided brachial plexus approach targeting a potential "nerve funnel" at the surgical neck of the humerus, bridging infraclavicular and axillary approaches. Cadaveric dissection explored its anatomical basis and correlated it with clinical sonographic findings. This retrospective case series included six adult patients with traumatic upper-limb injuries. Cadaveric observations suggested terminal nerve clustering and proximity of the musculocutaneous and i

Chilling the nerve, easing the pain?: A randomized clinical trial evaluating surgeon-administered bedside percutaneous cryoneurolysis for rib fracture pain.

BACKGROUND: A cornerstone of rib fracture management is multimodal pain control, which includes scheduled nonopioid analgesics, as-needed opioids, regional or neuraxial blockade, and surgical stabilization of rib fractures. However, adverse effect profiles and individual factors limit their use in all patients. Surgeon-administered, ultrasound-guided percutaneous cryoneurolysis performed at the patient bedside is a promising analgesic adjunct. METHODS: We performed a prospective, randomized clinical trial assessing benefit and safety of surgeon-administered percutaneous cryoneurolysis to our

Free fillet upper extremity flap reconstruction after forequarter amputation: A systematic review and meta-analysis.

BACKGROUND: Forequarter amputation and related shoulder-girdle ablative procedures create extensive defects, often in previously irradiated fields. "Spare-parts" reconstruction using tissue from the amputated limb, most commonly the free fillet upper-extremity flap (FFUE), offers donor-site-sparing coverage. The reliability and complication profile of FFUE reconstruction remain uncertain because evidence is limited to small series and case reports. METHODS: A PRISMA 2020-compliant systematic review and meta-analysis was conducted. PubMed, Scopus, and Embase were searched on 25 Nov 2025 using

Anterior Interosseous Artery as a Recipient Vessel in Superficial Circumflex Iliac Artery Perforator Flap Transplantation to the Forearm.

BACKGROUND: When transplanting a superficial circumflex iliac artery perforator (SCIP) flap to the forearm, the small diameter of the superficial circumflex iliac artery (SCIA) complicates end-to-end anastomosis with the radial or ulnar artery. The small diameter of the anterior interosseous artery (AIA) may make it suitable for end-to-end anastomosis with the SCIA; however, no such cases have been reported to date. We assessed AIA diameter using computed tomography angiography (CTA) and analyzed six cases of SCIP flap transfer to the forearm using the AIA as the recipient vessel. METHODS: In

Closed Reduction Internal Fixation with Titanium Elastic Nailing System for a Proximal Humerus Shaft Comminuted and Segmental Fracture.

Segmental fractures of the proximal humeral shaft are rare and challenging to manage, particularly in elderly osteoporotic patients with multiple co-morbidities. Traditional open fixation methods carry risks of soft tissue complications, neurovascular injury, and delayed recovery. Minimally invasive techniques such as Closed Reduction and Internal Fixation (CRIF) with Titanium Elastic Nailing System (TENS) offer an alternative, though their use in segmental proximal humeral fractures remains underreported. We report the case of a 72-year-old female with diabetes mellitus, hypertension, and cor

Comparative Study of Functional and Radiological Outcomes of Antegrade Versus Retrograde Titanium Elastic Nailing for Pediatric Ulnar Diaphyseal Fractures.

Background Pediatric forearm fractures account for approximately 40% of childhood skeletal injuries, with ulnar fractures being particularly prevalent. Titanium Elastic Nailing (TEN) offers an effective minimally invasive option for these injuries; however, the optimal nail entry point - antegrade (olecranon) versus retrograde (distal ulna) - remains debated. This study compared functional and radiological outcomes, complication profiles, and implant-related morbidity between the two approaches. Methods A retrospective comparative study was conducted from May 2023 to May 2025 at the R.L. Jalap

Short commentaries on data published by Yono et al. on avoiding breast cancer-related arm lymphedema with the lymphatic microsurgical preventive healing approach.

Lymphatic microsurgical preventive healing approach (LyMPHA) is being adopted to prevent arm lymphedema after axillary lymph node dissection (ALND), but open questions remain over selection bias, non-standardized endpoints and variable surgical protocols before robust evidence is available.

Cerebral arterial oxygen embolism as a complication of hyperbaric oxygen treatment: a case report.

Cerebral arterial gas embolism (CAGE) is a recognised complication of diving-related barotrauma and of medical procedures whereby gas enters the vascular system. CAGE is a principal indication for hyperbaric oxygen treatment (HBOT). In contrast, CAGE resulting from HBOT itself is exceedingly rare. We describe the case of a 73-year-old man undergoing his first session of HBOT for a chronic lower limb wound, who developed acute focal neurological deficits during decompression. He had no known respiratory disease or smoking history. Immediate assessment demonstrated stable physiology. Initial com

Surgical stabilization of rib fractures in adults without flail chest: a scoping review of pain, quality of life, and clinical outcomes.

BACKGROUND: Rib fractures are common after blunt thoracic trauma and cause severe pain, impaired ventilation, and pulmonary complications. Surgical stabilization of rib fractures (SSRF) is established for flail chest, but its role in non-flail patients remains uncertain, with heterogeneous patient selection, timing, and outcomes. This scoping review aimed to map the extent, nature, and gaps of the evidence on SSRF in adults with acute non-flail rib fractures, focusing on pain, quality of life, complications, and hospital resource use. METHODS: Following Joanna Briggs Institute methodology and

Intraoperative Monitoring of the Diaphragm Function During Cervical Dorsal Root Entry Zone-Lesioning Illustration With a Case Report.

BACKGROUND AND IMPORTANCE: Microsurgical DREZotomy (MDT) involves lesioning the dorsal root entry zone at the cervical spinal level, carrying a potential risk of iatrogenic spinal cord injury. Misidentifying the C4 root can lead to impaired diaphragmatic contractions and respiratory compromise. This report is the first MDT procedure in our Central Asian region and introduces the simultaneous analysis of 3 ventilator waveforms-capnography, pressure time, and flow time-as a novel, rapid-response intraoperative neurophysiological monitoring for definitive C4 root identification. CLINICAL PRESENT

Chemical Assault Burns Leading to Multi-organ Complications and End-Organ Damage: A Case Series.

This report presents two cases of extensive chemical burn injuries following acute exposure. The first involved a 24-year-old previously healthy female who sustained deep dermal and full-thickness burns over 44% of total body surface area, including the face, neck, chest, and upper limbs. Her injuries were complicated by circumferential limb burns, ocular involvement with corneal damage, and rhabdomyolysis leading to early acute kidney injury. Management required airway protection, aggressive fluid resuscitation, serial debridements, staged skin grafting, and reconstructive surgery. After seve

Insights into Cervical Phrenic Nerve Variations Based on Cadaveric Dissection and Intraoperative Electromyography.

BACKGROUND: In complex cervical nerve surgeries, ensuring the integrity of the phrenic nerve (PN) adjacent to the anterior scalene muscle is critical. However, we have encountered a clinical paradox: Postoperative diaphragmatic dysfunction occurred even when the main PN trunk was preserved intraoperatively. This suggests the potential involvement of PN variations (PNVs), such as the accessory PN, which merges with the main trunk, and separate branches that maintain an independent course. METHODS: Using contralateral C7 nerve transfer (CC7) as a surgical model, this study aimed to characterize

Efficacy and Safety of Percutaneous Pedicle Screw Fixation for Neurologically Intact Thoracolumbar Burst Fractures: A Prospective Interventional Study.

BACKGROUND: Open posterior short-segment pedicle instrumentation is an established treatment for unstable thoracolumbar fractures but carries approach-related morbidity from paraspinal muscle stripping. Percutaneous pedicle screw fixation (PPSF) aims to provide comparable stabilization with reduced soft-tissue trauma. We evaluated the perioperative profile, radiographic correction, and screw-placement accuracy of PPSF performed under single conventional fluoroscopy in neurologically intact patients. METHODS: In this prospective, single-arm interventional study, 46 consecutive patients aged 18

Iatrogenic Nerve Injuries Affecting Upper Limb Function: Medicolegal Implications.

PURPOSE: Nerve injuries are among the commonest iatrogenic complications and most frequently are the result of direct intraoperative damage causing serious distress and long-term morbidity. Additionally, they can add a considerable financial burden to health care systems, while they constitute a well-known source of medicolegal implications. There are few published articles addressing the medicolegal consequences of the iatrogenic nerve injuries affecting upper limb function. Our aim was to identify malpractice claims related to iatrogenic nerve injuries affecting upper limb function and to de

Paediatric Hand Compartment Syndrome Secondary to Intravenous Contrast Extravasation: A Case Report.

Iodinated contrast extravasation is usually self-limited; however, in confined regions such as the dorsum of the hand - especially in paediatric patients - it can progress to acute compartment syndrome. We report a 9-year-old girl who developed hand compartment syndrome following extravasation of 50 mL iohexol (350 mgI/mL) via a 22G catheter during contrast-enhanced abdominal computed tomography. Despite standard extravasation management, symptoms progressed rapidly. Compartment syndrome was diagnosed clinically based on pain disproportionate to injury, refractory to analgesics, exacerbated by
