Hybrid-Enveloped Pedicle of Distant Abdominal Flap for Reconstruction of Extensive Upper Extremity Defects.

BACKGROUND: Extensive upper extremity soft-tissue defects with exposed vital structures present a significant reconstructive challenge, particularly when microsurgical options are unavailable. Traditional distant pedicled abdominal flaps require a wide skin tube to protect the vascular pedicle, resulting in substantial donor-site morbidity and risks of pedicle compression. This study introduces a modified technique utilizing a hybrid combination of artificial dermis and local skin flap to create a hybrid-enveloped pedicle, aiming to minimize donor-site sacrifice and improve postoperative manag

Study of the Spread of Scarring along the Damaged Area of the Limb Nerve after Gunshot and Explosive Injuries.

BACKGROUND: Determining the true size or extent of the nerve defect after damage and excision of the posttraumatic neuroma is fundamental in the microsurgical treatment of the patient. It is assumed that the length of the scar in the nerve may depend on the nature of the combat wound of the limb, gunshot, or mine-explosive. Determining the extent of scarring in the nerve in cases of these two types of injury may provide an opportunity to establish a relationship with the type of injury or to disprove such a relationship. This should help in the surgical treatment tactics. METHODS: Fifty sampl

Improved Ulnar Plate Positioning Using the WHAM (Wrist Henry Approach Modification) for Volar Rim Fixation of Distal Radius Fractures.

INTRODUCTION: Fixation of small, very distal volar rim fragments of the distal radius is technically demanding because the classic Henry approach may provide limited ulnar exposure. The Wrist Henry Approach Modification (WHAM) combines a limited volar incision with a dual-window exposure. HYPOTHESIS: The WHAM approach would enable more ulnar positioning of volar rim-specific plates than the classic Henry approach without increasing approach-related complications. MATERIALS AND METHODS: This retrospective single-center comparative study included 39 patients with distal radius fractures treate

Prospective Study of Ultrasound-Guided Regional Anesthesia Blocks for Upper Extremity Procedures in a Community Emergency Medicine Hospital.

Painful upper extremity injuries frequently require procedures for which procedural sedation may be resource-intensive. This prospective study evaluated the educational impact and clinical feasibility of implementing a structured ultrasound-guided regional anesthesia curriculum as an alternative to procedural sedation for selected upper extremity procedures within a community emergency medicine residency. From January through December 2025, attending emergency physicians participated in a two-hour didactic session and hands-on workshop followed by longitudinal on-shift mentorship. Physician kn

Locking Plate Fixation versus Nonoperative Management in Patients Aged ≥65 Years with Proximal Humerus Fractures: Clinical and Functional Outcomes from a Retrospective Cohort Study.

OBJECTIVES: This study aimed to compare the clinical and functional outcomes of geriatric patients (≥65 years) with proximal humerus fractures treated with locking plate fixation or nonoperative management and to identify factors associated with the development of complications. We hypothesized that fracture pattern, rather than treatment modality alone, would be more strongly associated with functional outcomes and complication risk in this population. METHODS: Patients aged 65 years and older who were treated for proximal humerus fractures were included in this retrospective cohort study. P

Functional and Radiological Outcomes of Extra-articular Distal Radius Fractures Treated With Percutaneous K-wiring Using the Ulna as a Cantilever: A Single-Centre Prospective Observational Study.

BACKGROUND: Distal end radius fractures are among the most common orthopaedic injuries and can significantly impair wrist function if not managed appropriately. Various treatment modalities exist, but achieving stable fixation with minimal complications remains a challenge, especially in extra-articular fractures. AIM: The aim of this study was to evaluate the functional outcome of distal end radius fractures treated by K-wiring using the ulna as a cantilever. METHODS: This prospective observational study included 80 skeletally mature patients with extra-articular distal end radius fractures

What to do with K-wires in displaced upper extremity fractures in children - where to bury or not.

Kirschner wires (K-wires) are the standard fixation method for displaced pediatric upper extremity fractures, yet no consensus exists on whether wire ends should be buried subcutaneously or left exposed epicutaneously. While epicutaneous K-wires offer cost-effectiveness and avoid second anesthesia, concerns remain regarding infection risk and psychological distress. To systematically review and compare complication rates between buried and exposed K-wire fixation in pediatric upper extremity fractures, including infection, refracture, skin erosion, and secondary implant displacement. Near-Hanging Incidents and Suspected Spinal Cord Injuries: A 5-Year Analysis in a Developing Nation.

Survivors of hanging who reach hospital alive, commonly referred to as "near-hanging". The characteristics and outcomes of patients with suspected spinal cord injury (sSCI) following near-hanging remain poorly described in developing-country settings. The study aims to describe the demographic and clinical characteristics, temporal patterns, and outcomes of patients presenting after near-hanging with documented sSCI, while distinguishing neurological manifestations from associated neck soft-tissue and airway injuries. This retrospective multicenter observational study reviewed medical records

Bayesian re-analysis of ERA223 and PEACE-3 temporal dynamics and bone protection in radium-223-treated metastatic castration-resistant prostate cancer.

OBJECTIVE: To define the time course of fracture risk with radium-223 in combination with androgen receptor pathway inhibitor (ARPi) therapy and assess the clinical impact of concomitant bone-protective agent (BPA) use on mitigating this risk. METHODS: An exploratory person-time approximation approach was used to perform a Bayesian re-analysis of the landmark ERA223 and PEACE-3 clinical trials. In ERA223, we estimated interval-specific fracture incidence rate ratios (IRRs) during follow-up to characterize temporal trajectories. Interval-specific analyses were based on an exploratory person-ti

Preserving the Bone Envelope: A Prospective Observational Study of Closed Reduction and Ilizarov Circular Fixation for Open and Closed Distal Tibial Fractures.

Distal tibial fractures are challenging injuries because of limited soft-tissue coverage, frequent high-energy mechanisms and the potential for soft-tissue compromise. Extensive surgical exposure may further disturb the local soft-tissue and biological environment. Ilizarov circular external fixation permits stable fixation with limited exposure of the fracture site. This prospective observational study evaluated fracture union, bone and functional outcomes and complications following closed or indirect reduction and Ilizarov circular external fixation in patients with open and closed distal t

Nutritional Supplements in the Treatment of Endometriosis-Associated Symptoms: A Systematic Review.

Cancer is regarded as one of the leading causes of death worldwide, despite the progress of traditional therapies. Chemotherapy, radiotherapy, and surgery are often accompanied by significant side effects and the development of drug resistance contributes to making the fight against cancer even more challenging, which clearly highlights the urgent need to develop new therapeutic molecular approaches. In this context, natural peptides were introduced into the pharmaceutical market in the last decade and have replenished the ranks of effective anticancer agents due to their structural diversity,

Post-cycle therapy after androgen abuse: what are we seeking to achieve?

Despite limited evidence supporting its efficacy, the use of post-cycle therapy (PCT) is widespread amongst men following cessation of androgen abuse. The proposed rationale for PCT is to accelerate recovery of hypothalamic-pituitary-gonadal (HPG) axis function following androgen-induced suppression. In this commentary, we consider the distinction between pharmacological response and sustained recovery of endogenous HPG axis function, the relationship between biochemical and symptomatic recovery, and the potential role of PCT as a harm-reduction strategy.

Operative Treatment of Pediatric Supracondylar Humerus Fractures Using Mini C-Arm Fluoroscopy.

BACKGROUND: Supracondylar humerus (SCH) fractures comprise about 20% of pediatric fractures. Displaced SCH fractures require reduction and percutaneous pinning, most commonly making use of conventional C-arm (CCA) fluoroscopy. Many authors have proposed the use of mini C-arm (MCA) in the upper extremity, given multiple proposed benefits including decreased radiation. We investigated the utility of MCA in the operative treatment of pediatric SCH fractures. METHODS: Patients who underwent operative fixation of SCH fractures using MCA from 20202022 were reviewed. Demographic, surgical, and radio

Safe dosage and potential risks of chlorogenic acid: insights from in vitro and in vivo studies.

INTRODUCTION: As the economy grows, there is a growing emphasis on food safety. While the health benefits of chlorogenic acid (CGA) are recognized, safe dosages and potential liver cell damage from excessive CGA consumption are not well studied. This study aims to determine the safe and effective dose range of CGA and understand how it causes toxicity in hepatocyte at half-maximal inhibitory concentration (IC50). METHODS: This study assessed the impact of various CGA concentrations on liver cells, examining growth, viability, toxicity, energy metabolism, and colony formation using Real-Time C

Lower Extremity Compartment Syndrome Due to Capillary Leak Syndrome Following 60% Total Body Surface Area Burn Injury.

Lower extremity compartment syndrome (LECS) following burn injury in the absence of circumferential burns is rare. Capillary leak syndrome (CLS) is a condition characterized by systemic capillary hyperpermeability and can be triggered by the multisystem inflammatory response seen in extensive burns. The resulting intravascular fluid loss into the interstitial space can elevate compartment pressures and contribute to the development of compartment syndrome. In the case presented here, the patient developed LECS in the setting of severe burn injury with clinical and laboratory evidence consisten

Revascularize or Amputate? Underrated Arterial Damage in High-Voltage Electrocution: A Literature-Informed Clinical Perspective.

High-voltage electrocution injuries can result in extensive, multisystem tissue damage, including vascular injuries. The decision between revascularization and early amputation remains challenging, especially given the risks of hemorrhage, infection, and repair failure. A comprehensive literature review was performed using PubMed, Scopus, and Cochrane databases with predefined MeSH terms and keywords related to electrocution and arterial injury. Nineteen relevant human studies were selected after screening. In addition, we present a case of high-voltage upper limb injury in which an arterial i

Economic benefits of T cell-depleting induction in deceased donor kidney transplant recipients.

Thymoglobulin (TMG) induction improves long-term outcomes in high-immunologic-risk kidney transplant (KTx) by reducing acute cellular rejection. However, routine use of TMG in lower-immunologic-risk KTx is controversial, given concerns about posttransplant infectious complications and added health care expenditures. United States Renal Data System data were examined to compare real-world clinical and economic outcomes-including 3-year median Medicare expenditures, incidence of key adverse events, and allograft survival-in a cohort of Medicare-insured deceased donor KTx recipients (2006-2020) t
