An analysis of lifetime fractures in women with epilepsy.

OBJECTIVE: To clarify the relationship between fractures and antiepileptic drug (AED) use. METHODS: Menopausal women with epilepsy were interviewed at two clinics regarding site, year and circumstances of any fracture, duration of AED use and menopause. Fracture sites were analyzed according to AED use. RESULTS: Twenty-nine fractures occurred in 20 of the 50 interviewed subjects (mean age 54). Nine occurred prior to AEDs; seven attributed to accident and two to clumsiness. Twenty occurred on AEDs; 10 attributed to clumsiness (most in the leg and foot), eight to seizure (most in the arm or ha

Entrapment of the flexor digitorum superficials in the radius fracture site.

Entrapment of the flexor tendon after fracture of both forearm bones is very rare. This report describes entrapment of the middle finger flexor digitorum superficialis (FDS) tendon alone in the radius fracture site. A 13-year-old boy fractured both forearms and had closed reduction and a long-arm cast. After the cast was removed he had full middle finger motion with the wrist flexed but was unable to extend the proximal interphalangeal or metacarpophalangeal joints and could extend only the distal interphalangeal joint of the middle finger with the wrist in the neutral or extension position. Thromboembolic disease prophylaxis in patients with hip fracture: a multimodal approach.

OBJECTIVES: To assess if pneumatic compression in conjunction with chemoprophylaxis is an effective way to reduce the incidence of deep vein thrombosis in orthopedic trauma patients sustaining fragility hip fractures. DESIGN: Two hundred patients admitted to the authors' institution between May 1998 and June 2002 for fractures of the hip were prospectively studied. All patients were treated operatively and received the VenaFlow calf compression device on both lower extremities immediately following surgery. Chemical prophylaxis of either aspirin (n = 67) or warfarin (n = 133) was administered

Missed posterior fracture-dislocation of the humeral head following an electrocution injury to the arm.

Posterior dislocation of the shoulder is a rare and commonly missed injury. While prompt diagnosis and treatment is important to prevent untoward sequelae, it is often diagnosed and treated too late. The first reported case of a posterior fracture-dislocation of the humeral head following a domestic electrocution accident in Singapore is described in a 52-year-old man. The injury was missed by several doctors before a humeral head replacement was done. The diagnostic pitfalls and management of this injury and ways to avoid a missed or delayed diagnosis are discussed.

Dystonia after a bone fracture of the arm in a patient with a history of striato-pallidal ischemic stroke: a case report.

We report on a female with segmental dystonia of the upper limb after an anatomically related ischemic stroke. Dystonia developed almost 3 years after the onset of the stroke and immediately following the removal of the cast because of a bone fracture in the same limb. We discuss the case considering issues such as: delay-onset, lesion topography and pathophysiology, peripheral input and their possible contribution to the development of secondary dystonia.

Muscle strength is a determinant of bone mineral content in the hemiparetic upper extremity: implications for stroke rehabilitation.

Individuals with stroke have a high incidence of bone fractures and approximately 30% of these fractures occur in the upper extremity. The high risk of falls and the decline in bone and muscle health make the chronic stroke population particularly prone to upper extremity fractures. This was the first study to investigate the bone mineral content (BMC), bone mineral density (BMD), and soft tissue composition of the upper extremities and their relationship to stroke-related impairments in ambulatory individuals with chronic stroke (onset >1 year). Dual-energy X-ray absorptiometry (DXA) was used

Suprascapular neuropathy complicating a Neer type I distal clavicular fracture: a case report.

We report the case of a 43-year-old male patient who sustained a Neer type I distal clavicular fracture after a fall onto his lateral shoulder. He was initially treated nonoperatively, and the associated suprascapular nerve injury was not diagnosed until 3 months after the accident, when the obvious hypotrophic changes in the scapular muscles were noticed. After an additional 6 weeks of observation, surgery was performed due to the absence of any clinical and electrodiagnostic evidence of recovery. At the time of exploration, through a posterior approach, the suprascapular nerve was found to b

Initial experience of percutaneous vertebroplasty using single-plane C-arm fluoroscopy for guidance.

PURPOSE: The purpose of this study was to evaluate the feasibility and safety of percutaneous vertebroplasty (PVP) using single-plane C-arm fluoroscopy. MATERIALS AND METHODS: Lesions in the present study all comprised compression fractures with intact pedicles. A polymethylmethacrylate (PMMA) mixture was injected by the transpedicular approach under C-arm fluoroscopy using a lateral projection. Before and after PVP, the pain score was determined using visual analog scale methods. RESULTS: Between June 2002 and May 2004, we performed 31 PVPs in 17 patients during 22 treatment sessions. Unreduced chronic dislocation of the humeral head with ipsilateral humeral shaft fracture: a case report.

The purpose of reporting this case is to illustrate a treatment plan for a chronically anteriorly dislocated shoulder associated with an ipsilateral humerus fracture, a condition heretofore not addressed in the literature to our knowledge. An 18-year-old female, left hand dominant, injured her left upper extremity and liver in a motor vehicle accident. X-rays at time of injury revealed a diaphyseal facture of her left humerus. No x-rays of the shoulder were taken at time of injury. Treatment consisted of a plaster cast application and discharge at 1 week. The patient was seen again 4 weeks pos

A patient with an odontoid fracture and atrophy of the tongue: a case report and systematic review of the literature.

BACKGROUND: Traumatic hypoglossal nerve palsy is a rare entity and has rarely been described in association with an odontoid fracture. CASE DESCRIPTION: We present a patient with a posttraumatic odontoid fracture who developed selective weakness of his arms and a unilateral hypoglossal nerve palsy. A systematic review of the literature is presented, and hypothetical causes for the injury are discussed. CONCLUSION: Bell's cruciate paralysis and central cord syndrome are probably expressions of the same mechanism rather than 2 separate entities based on a preferential damage of pyramidal cross

Brachial artery transection associated with closed and open dislocation of the elbow.

Acute dislocations of the elbow complicated by brachial artery involvement are rare but serious injuries. Rapid assessment and a high index of suspicion are essential to facilitate prompt treatment, as delay in diagnosis is associated with a poorer outcome. We present two cases of brachial artery transection after closed and open dislocation of the elbow, and discuss the appropriate management of such patients.

Pediatric fractures of the forearm.

Forearm fractures are common injuries in childhood. There are a number of important principles that should be followed to achieve the ideal goal of fracture healing without deformity or dysfunction. I will review the general principles, classifications, diagnosis, treatment, and complications of pediatric forearm fractures, including some specific injuries such as Monteggia fractures, Galeazzi injuries, and open fractures. The basic principle is to accurately align the fracture fragments and to maintain this position until the fracture is united. Forearm fractures in children can be treated di

[Forearm fractures in children].

Fractures of the forearm in children are quite frequent injuries, treated in emergency rooms. Most of them occur during leisure or sport activities. Most cases are monotraumatic. The diagnosis can be made by a conventional x-ray examination of the forearm in two planes. It is possible to differentiate between stable fractures (greenstick fractures) and unstable (dislocated) fractures. The first-line therapy for stable fractures or fractures which can be reduced anatomically correctly, is the conservative procedure using a long arm cast. Axial dislocation up to 20 degrees (age <5 years) or up t

Outcome after surgery of distal radius fractures: no differences between external fixation and ORIF.

The purpose of this study was to determine the most appropriate surgical treatment from three procedures used for fractures of the distal radius. In a retrospective comparative study, 166 out of 237 patients who underwent surgery for AO/ASIF A3 or C2 distal radius fractures were evaluated. Surgical procedures were external fixation or open reduction and internal fixation with either palmar or dorsal plates. Radiological outcomes were palmar tilt, radial angle, radial length and ulnar variance. Functional outcomes were range of motion and grip strength. In addition, outcomes according to Gartla

A completely displaced Salter-Harris I fracture of the distal radial epiphysis with intact ulna: a case report and review of the literature.

We present a 12-year-old boy with a displaced fracture of the distal radial epiphysis type I according to the Salter-Harris classification. The distal radial epiphysis was completely dorsally displaced, while the distal ulna remained intact. The injury was result of a fall on the patient's outstretched hand. Closed reduction was applied and the forearm was immobilized with a long arm cast. In the radiological re-examination a week later, a complete re-displacement of the fracture was detected. Open reduction followed and interposed flexor tendons (flexor pollicis longus and flexor carpi radial

[Functional results after proximal row carpectomy (PRC) in patients with SNAC-/SLAC-wrist stage II].

The proximal row carpectomy (PRC) is a motion preserving procedure which creates a new joint without arthrosis. It is a frequently used procedure in stage II of a posttraumatic degenerative arthrosis of the wrist after scaphoid nonunion or scapholunate ligament instability (SNAC-/SLAC-wrist). In this retrospective analysis the functional postoperative results of this operation are compared in light of a homogenous indication (SNAC-/SLAC-wrist stage II). In 38 patients PRC was performed for a stage II SNAC- (n = 29) or SLAC-wrist (n = 9) between June 1994 and March 2002. Postoperative examinati
