[Does the disability of shoulder, arm and hand questionnaire (DASH) replace grip strength and range of motion in outcome-evaluation?].

Scoring systems currently used to evaluate functional disabilities in upper extremity conditions frequently combine so-called "objective" parameters such as grip strength and range of motion with "subjective" parameters such as pain. Since its introduction, the Disabilities of Arm, Shoulder and Hand Questionnaire has proven useful in the pre- and postoperative evaluation of functional limitations of the upper extremity. This study examines the relationship between DASH scores, the so-called "objective" parameters of grip strength and range of motion and those of "mixed scores" such as Krimmer

Strategies for nonrandomized clinical research in hand surgery.

Most clinical research questions in hand surgery may be effectively explored using a variety of nonrandomized study designs. The main advantage of any of these methods is that they are almost always more feasible than a prospective randomized, controlled trial. Although the level of evidence associated with nonrandomized designs is always lower than that of a randomized trial there are many instances in which the inferences based on these designs are sufficiently strong that important and meaningful conclusions can be made. The key considerations in using nonrandomized designs are to frame the

Occurrence of carpal osteoarthritis after treatment of scaphoid nonunion with bone graft and herbert screw: a long-term follow-up study.

PURPOSE: To determine the occurrence, progression, and clinical results of osteoarthritis (OA) after bone grafting and Herbert screw fixation for scaphoid nonunion. METHODS: Fifty patients were reviewed (mean follow-up period, 12.0 +/- 1.6 y) to analyze degenerative changes of the wrist after use of Herbert screws for scaphoid nonunion. RESULTS: Radiologic signs of OA were observed in 15 of 50 patients before surgery and in 23 of 50 after 10 years or more. The majority of degenerative changes were low grade. No OA at follow-up evaluation was seen in 27 of 35 patients with no preoperative deg

Successful delayed venous drainage in 16 consecutive distal phalangeal replantations.

The authors describe, in the first report of this type of replantation surgery, a high success rate using delayed venous anastomosis in 16 consecutive distal phalangeal replantations under digital block. Among these replantations, seven fingers (43.8 percent) showed postoperative venous congestion and five fingers were reoperated on with delayed venous drainage under digital block. All the reoperated fingers were successfully drained by additional single or double venous drainage with a vein graft. As a result, 13 fingers survived (81.3 percent success rate). All operations were performed unde

What symptoms are truly caused by median nerve compression in carpal tunnel syndrome?

OBJECTIVE: This study aims to identify the symptoms, signs and distributions which are associated with neurophysiological carpal tunnel syndrome (N-CTS), defined by the finding of a median nerve lesion at the wrist through neurophysiologic studies, and to compare them with those of patients with sensory or motor complaints in the upper limbs whose electrophysiologic investigation did not show evidence of this syndrome. METHODS: A cross-sectional study was performed, with prospective gathering of data, following a predetermined protocol. We included all patients older than 12 years who were re

Superficial radial nerve damage due to Kirschner wiring of the radius.

For displaced fractures of the radius, the use of Kirschner wires (K Wires) is accepted practice either alone or to supplement external fixation. Complications related to K wires include infection, migration and damage to tendons and nerves. We set out to investigate to incidence of superficial radial nerve damage due to radial styloid K wires. Injury to the sensory branch of the superficial radial nerve was seen in eight (20%) out of the 40 patients. K wiring is a popular technique to help maintain anatomic reduction of distal radial fractures. It has the advantage of being a semi-closed proc

Anatomical variations as potential risk factors for ulnar tunnel syndrome: a cadaveric study.

The aim of this study was to assess the anatomical variations, especially the anomalous muscles passing through Guyon's canal and the fibrous arch forming the piso-hamate hiatus, which may play a role in ulnar tunnel syndrome. We have also focused on the relation of these structures with specific concern to the ulnar nerve. Nineteen embalmed cadavers (37 hands and forearms) were dissected. A fibrous arch extending between the pisiform and the hook of the hamate was observed in 21 hands. In majority of the cases flexor digiti minimi muscle was found to originate only from this arch. An anomalou

Prevalence and severity of carpal tunnel syndrome (CTS) during pregnancy.

Carpal Tunnel Syndrome (CTS) is the entrapment of Median nerve at wrist. Pregnancy can affect prevalence and severity of CTS. In this study frequency and severity of CTS in pregnancy was evaluated. In this study 100 pregnant women were evaluated by Hand symptoms, CTS provocation tests (Tinel and Phalen) and for definite diagnosis of CTS standard EDX studies were used. For this purpose sensory and motor distal latency of Median were measured and compared with ulnar nerve. 17% of pregnant women had CTS with 23.5% bilateral involvement and 17.5% severe CTS. Prevalence of hand symptom and clinical

Complex injuries including flexor tendon disruption.

The treatment of tendon injury in combined complex injuries to the hand is dictated by the presence of concomitant injuries. Early range of motion is desirable. To achieve this, fractures must be stabilized and the soft tissue envelope and vascular integrity maintained or reconstituted. In those instances in which these conditions cannot be met, the surgeon and patient should be prepared for secondary surgeries, including reconstruction or tenolysis. Although nerve integrity is not necessary for early functional success following tenorrhaphy, nerve injuries should be repaired or grafted primar

Novel surgical approach to the carpal tunnel: cadaveric feasibility study.

Carpal tunnel syndrome is the most common entrapment neuropathy dealt with by the clinician. Multiple techniques have been used to surgically treat this pathological condition and all of these approach the carpal tunnel from the palmar surface of the hand or wrist. We have developed a novel endoscopic approach to the carpal tunnel utilizing a dorsal approach that necessitates a good appreciation of the anatomy of this region. This approach was carried out in 10 hands. Through a single dorsal incision we were able to transect the flexor retinaculum in all specimens without obvious damage to neu

Peripheral nerve injuries in weight training: sites, pathophysiology, diagnosis, and treatment.

Direct trauma, compression caused by muscle hypertrophy or other soft tissue changes, or excessive stretching of a peripheral nerve in the upper extremity may lead to uncommon-but potentially serious-complications. Clinicians are seeing more of these injuries as weight training, power lifting, bodybuilding, cross-training, and general physical conditioning with weights become more popular. Symptoms of pain, weakness, paresthesia, or palsy; physical exam findings; electromyography; and nerve conduction studies are used to make the diagnosis. Most conditions respond well to conservative measures

[Angiological and neurological problems after shunt surgery in haemodialysis patients].

Vascular access induced complications contribute to the morbidity of patients on regular dialysis treatment. Impaired peripheral perfusion, cardiac stress and nerve lesions are the most common of these complications. In the first part of this paper, angiological problems caused by creating arterio-venous shunts are described. Shunt volumes, blood circulation in the shunt arm and shunt-induced alterations of the circulatory blood volume are discussed as well as the conditions under which these parameters gain pathogenic importance. After describing the different shunt-specific circulatory distu

Does radial artery harvesting for coronary revascularization cause neurological injury in the forearm and hand?

OBJECTIVE: Radial artery (RA) is now used widely as a conduit of choice in coronary artery bypass grafting. Although RA removal is considered safe in the presence of adequate collateral arterial supply, there is still a considerable suspicion on the functional status of the forearm and hand. However, a neurological dysfunction may occur owing to either surgical trauma or ischemic neuropathy. This study was aimed to investigate the functional outcome of the donor forearm nerves of the patients who underwent coronary artery bypass grafting surgery with RA conduits. METHODS: A consecutive series

[Pseudo-ulnar palsy secondary to ischemic cerebral infarction].

INTRODUCTION: Paralysis of the fingers secondary to injury to the central nervous system or peripheral pseudoparalysis is an infrequent pathological condition. The most common form is the one affecting the ulnar area, although it has also been reported as affecting the radial area or even the index finger. CASE REPORT: We report a case of right-side pseudoparalysis of the ulnar with the fourth and fifth fingers in a bending posture in a 61-year-old male with risk factors for atherothrombosis. The definitive diagnosis was obtained by magnetic resonance (MR) imaging of the head, which revealed
