Bilateral Sternoclavicular Joint Dislocation with Inferior Sternal Migration: Surgical Management and One-Year Outcome.

Bilateral sternoclavicular joint (SCJ) dislocation is an exceptionally rare injury, particularly when associated with inferior migration of the sternal body and multiple rib fractures. We report the case of a 51-year-old male who sustained high-energy thoracic crush trauma resulting in bilateral SCJ dislocation, inferior sternal displacement, rib fractures, and a Grade I open humeral shaft fracture. Surgical management required a multidisciplinary approach including SCJ reduction, ligament reconstruction with a semitendinosus tendon graft in a figure-of-eight configuration, clavicular fixation

An Investigation of the Etiology and Pattern of Spinal Injuries Among Patients Presenting to the Emergency Department in a Tertiary Care Hospital.

Background Spinal cord injuries contribute substantially to morbidity and mortality globally. There is a gross disparity in the causes of spinal injuries between developed and developing countries. Thus, understanding the etiology and pattern of spinal injury in our setting is important to plan strategies for effective management and prevention strategies at the national level. Methodology A prospective observational study was conducted among 75 patients with spinal cord injury visiting the Department of Emergency Medicine of a tertiary care hospital. The study aimed to investigate the etiolog

Return to riding after open reduction internal fixation of acute clavicle fractures in professional horse jockeys.

BACKGROUND: High-velocity, traumatic fracture of the clavicle is one of the most common injuries suffered by professional horse jockeys. Operative fixation represents the mainstay of treatment in this cohort. Despite this, there remains a paucity of evidence to guide post-operative recovery and time to return to riding. Accordingly, the aim of the present study was to establish the safety and efficacy of open reduction internal fixation of acute clavicle fractures in professional horse jockeys. METHODS: A prospectively maintained database was used to retrospectively identify professional hors

Posterior occupancy ratio-guided anterior cervical discectomy and fusion combined with unilateral biportal endoscopic decompression for pincer-type cervical spondylotic myelopathy: a case report and literature review.

BACKGROUND: Pincer-type cervical spondylotic myelopathy (CSM) represents a severe clinical phenotype characterized by simultaneous spinal cord compression from both ventral and dorsal structures, frequently necessitating circumferential decompression. Although traditional single-stage combined anterior-posterior (A-P) open surgery provides adequate decompression, it is often associated with extensive surgical trauma and an elevated risk of perioperative complications. A focused literature review was conducted in PubMed, Embase, and Web of Science (inception to December 31, 2025) using keywords

Outcomes and Complications of Non-operative Management of Isolated Olecranon Fractures.

Introduction Non-operative management of olecranon fractures may be indicated in specific populations or when patients opt for conservative treatment after shared decision-making. In these instances, thorough understanding of operative and non-operative management outcomes can equip upper extremity surgeons with requisite knowledge to inform practice-specific fracture management pathways. We hypothesized that non-operative management of unilateral, isolated olecranon fractures results in satisfactory patient outcomes in terms of function, range of motion, and pain. Methods After International

Unmasking Ehlers-Danlos Syndrome Through Reduction Mammaplasty: An Illustrative Case and a Systematic Literature Review.

Ehlers-Danlos syndrome (EDS) is characterized by joint hypermobility, skin hyperextensibility, and tissue fragility, which may worsen symptomatic macromastia. Concerns about elevated surgical risk have historically discouraged elective procedures in EDS patients; however, postoperative complications may themselves prompt an EDS diagnosis, as illustrated by our case. This relationship prompted the conduct of a systematic review examining outcomes of reduction mammaplasty in EDS. A 37-year-old woman underwent bilateral inferior pedicle reduction mammaplasty for symptomatic mammary hyperplasia. P

The Risk of Acute Carpal Tunnel Syndrome in Lunate and Perilunate Dislocations.

PURPOSE: Lunate and perilunate dislocations are uncommon wrist injuries that have a high reported association with acute median neuropathy, frequently referred to as acute carpal tunnel syndrome (ACTS). Previous investigations into the incidence of lunate and perilunate dislocation and acute median neuropathy have often been limited by small sample sizes. The purpose of our study was to report a relatively large cohort examining the incidence of ACTS in lunate and perilunate dislocations and assess predictive risk factors. METHODS: A retrospective review was performed of all lunate and perilu

Secondary Carpal Tunnel Syndrome Due to Flexor Tenosynovitis in a Diabetic Patient.

Carpal tunnel syndrome (CTS) is the most common entrapment neuropathy of the upper limb. Although many cases are idiopathic, structural or inflammatory pathology of the flexor tendon sheath can result in secondary median nerve compression. Diabetes mellitus predisposes to such changes due to chronic connective tissue alterations, but secondary CTS arising solely from noninfective flexor tenosynovitis remains uncommon. A 49-year-old woman with recently diagnosed type 2 diabetes presented with a 2-month history of progressive right-wrist pain, paresthesia, and volar swelling. Magnetic resonance

Introducing the Minimally Invasive Novel Open Revascularization - The "MINOR" Approach - To Improve Outcomes in Lower and Upper Limb Arterial Bypass Surgery.

BACKGROUND: Traditional open limb bypass surgery has risks of perioperative complications and prolonged recovery due to large wounds and extensive dissections. We describe a new concept in bypass revascularization, the minimally invasive novel open revascularization (MINOR) approach - which has 3 elements: shortest possible length of vein bypass, endoscopic vein harvesting (EVH), limited incisions to minimise tissue damage. METHODS: Analysis of prospectively collected data over 18 months on sequential, unselected patients treated by the MINOR approach. Primary outcomes were technical success,

Safety and Effectiveness of Ultrasound-Guided Carpal Tunnel Release: A Systematic Review.

Background: Carpal tunnel syndrome is the most common entrapment neuropathy of the upper limb and an important cause of pain, sensory disturbance, and hand dysfunction. Open and endoscopic carpal tunnel release are effective, but they may be associated with scar-related pain, wound complications, and delayed return to work. Ultrasound-guided techniques have been developed to reduce soft-tissue trauma, enable real-time visualization of neurovascular structures, and facilitate faster recovery. Methods: A systematic search of PubMed, PubMed Central, and Scopus was conducted according to PRISMA gu

Residential radon in Peru: challenges for health surveillance and lung cancer prevention.

The Journal of Genetics, started by William Bateson in 1910, played a distinguished role in the early years of genetics. However, it stopped publishing in 1978. The Indian Academy of Sciences revived it in 1985, and has published it regularly since then. To commemorate this landmark, I highlight one of the 17 articles published that year. 'The isolation and genetic analysis of a Caenorhabditis elegans… X-chromosome balancer' by András Fodor and Péter Deak, of the Institute of Genetics, Biological Research Centre, Hungarian Academy of Sciences, Szeged, Hungary. Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Inadequate soft tissue coverage and bone loss/comminution are the typical risk factors of surgical site infection in open fractures of the hand: A nomogram prediction model.

BACKGROUND: Different from other parts of long bone fractures, surgical site infections (SSI) often occurs in open fractures of the hand (OFH) due to the anatomical characteristics and injury mechanisms. Our aim of the study is to investigate the particular risk factors of SSI after emergency surgery in OFH and develop a prediction nomogram model. METHODS: In our traumatic center, patients with OFH not less than 18 years old were retrieved between October 2020 and April 2024. We excluded patients with other fractures, non-traumatic fractures or surgery before admission. The data of these pati

Subdermal contraceptive implant migration to pulmonary artery branch.

Subdermal contraceptive implants are now commonly used throughout the world. One of the rare complications of these implants is migration to the lungs due to misplacement of the implant during insertion, with only a limited number of cases documented. Here, we present a case where a subdermal contraceptive implant embolised in the subsegmental branch of the pulmonary artery within the anterobasal segment of the left lower lobe. Due to incorrect placement, the subdermal implant migrated from subcutaneous plane into one of the upper extremity veins. It then travelled the venous pathway to the he
