Surgical morbidity after mastectomy operations.

The records of 146 consecutive patients with mastectomy operations over a three year period were reviewed. Of these, ninety-three (63.6 per cent) had one or more complications secondary to surgery. There were 175 complications, of which complications, of which 107 (73.2 per cent) were wound-related. Seroma formation occurred most frequently but was of minor consequence. The incidence of flap necrosis was 19.1 per cent, with cellulitis or suppurative infection appearing in thirteen patients. Immediate postoperative edema of the arm was fairly rare (2.7 per cent) and appeared related to delayed

Fracture-dislocation of the radiocarpal joint.

In five patients with fracture-dislocation of a radiocarpal joint, the wrist injury was severe and associated with injuries to other systems. Typically there was neurovascular impairment and closed reduction was performed as soon as possible to relieve pressure to tension on vessels and nerves. Fracture of the radial and ulnar styloid processes, fracture of the dorsal rim of the radial articular surface, and a multitude of carpal and intercarpal injuries were present in all cases. Early open reduction, internal fixation of the fractures, and repair of all torn ligaments appeared to be the trea

Lateral-volar finger flap for the treatment of burn syndactyly.

To completely open the web space in burn syndactyly and provide tissue that is pliable and elastic, we describe a flap from the lateral and volar surfaces of an adjacent finger which will completely cover the opened defect. The results of this procedure in 24 web spaces are reported.

Neuropathy in the upper extremity after open-heart surgery.

Eleven patients with upper-extremity neurological abnormalities underwent open-heart surgery performed through a median sternotomy incision. Seven of the 11 patients were referred in the routine manner to evaluate and treat the neurological problem. The remaining four were part of a consecutively studied group of 11 patients examined prospectively to determine the possible presence of abnormalities. Two of these four patients were asymptomatic. All lesions could be postulated to occur within the brachial plexus, the most common area being the median cord, but lesions were also noted in the pos

Screw fixation for certain fractures of the phalanges and metacarpals.

Twenty-one patients with fractures of the bones of the hand were treated by open reduction and internal fixation with compression screws. The firm fixation allowed range-of-motion exercises in most patients to be begun on the third postoperative day. On follow-up mild restriction in digital motion was seen in only one patient, and significant disturbance in joint function was not encountered. In spite of these advantages, the method is indicated in only a small percentage of such fractures. Usually traditional methods of open reduction and internal fixation are preferred when an open procedure

Arterial occlusion in juvenile humeral supracondylar fracture.

Juvenile supracondylar fracture of the humerus is described briefly and divided into two groups: 1. Those cases amenable to safe closed treatment; 2. Those in which occlusion of the brachial artery associated with severe displacement renders closed treatment unsafe. A specific mechanism of entrapment of the brachial artery, which has not been described previously, is reported, with the nature and the solution of the problem found at primary open operation. A discussion of the surgical anatomy shows how recognition of the mode of occlusion of the brachial artery may contribute to safe and relia

Fractures of the medial humeral epicondyle in children.

A review of 50 children with fractures of the medial humeral epicondyle facilitated a simple classification of this injury and its management. The "apparent isolated" fracture of the medial humeral epicondyle is uncommon; it is more frequently associated with elbow dislocation, with or without spontaneous reduction, at the time of injury. Analysis of the results of treatment of medial humeral epicondylar fractures in this group of children showed that, in the case of those with an "apparent isolated" injury, treatment with a sling only gave good results in 90% whereas poorer results followed o

Fractures of the humeral shaft: a retrospective study of 240 adult fractures.

Of 240 humeral fractures, 60 per cent occurred in patients under 35 years old, 39 per cent of fractures being in the middle third of the shaft, 28 per cent were open with an infection rate of 4 per cent. Radial nerve palsy was present in 18 per cent; midshaft fractures accounting for 69 per cent, with 60 per cent of radial nerves recovering spontaneously; 24 per cent after 12 weeks post injury. One hundred and eleven fractures had documentation greater than 3 months. Closed treatment in 100 patients resulted in 5 non-unions and 15 delayed unions; 11 primary internal fixations resulted in 3 non
