Fatal Lower Respiratory Tract Infection Following a Dog Bite in a Cirrhotic Patient: A Case Report of Zoonotic Sepsis.

We report the case of a 66-year-old woman with a history of alcohol abuse and cirrhosis, living in poor housing conditions. She was found dead in her residence and was reported to have sustained a dog bite to the left upper limb approximately 20 days before death. According to police reports, she neither sought medical attention nor received antibiotic therapy following the incident. The wound was neglected, and external examination at autopsy revealed signs of cellulitis of the affected limb. A medico-legal postmortem examination was performed to determine the cause of death. The autopsy reve

Ultrasound-guided release of the fibro-osseous tunnels around the wrist and hand: a technical review.

Fibro-osseous tunnels allow the unhindered passage of the neurovascular bundle and sliding tendons across the mobile joint. Release of the fibro-osseous tunnels is performed for entrapment neuropathy and stenosing tenosynovitis which occur as a result of increase in the volume/pressure within these tunnels. The common surgical principle in all these pathologies is releasing the pressure by transecting the fascial covering. Primarily these pathologies are treated with open surgery but are potentially amenable to curative percutaneous ultrasound (US)-guided release procedures. US is the preferab

ANCA Negative Vasculitis Manifesting as Pulmonary-Renal Syndrome in a Patient with Chronic Osteomyelitis.

A man in his early 60s, with a history of obstructive airway disease and hypertension, had a fall from a height 1 year ago. He developed a fracture of the left distal tibia and fibula, for which open reduction and internal fixation (ORIF) was done. Since then, he has had persistent pain and discharge from the implant site, for which multiple antibiotics were given and debridement was done numerous times. Three months ago, he was admitted to an outside hospital with similar complaints, and a pus culture grew methicillin-resistant Staphylococcus aureus. The patient gave a history of sudden onset

[Surgical treatment of the medial epicondyle fracture of the humerus using the ''Inside-out'' technique. Description of the technique and results].

INTRODUCTION: the "Inside-out" technique is one of the options for medial epicondyle fracture fixation of the humerus. It involves preparing the fixation from the internal portion of the fracture to the external portion. Then, the fracture is fixed with a cannulated screw from the external to the internal portion. This study aims to describe our experience with this technique, its outcomes, and its complications. MATERIAL AND METHODS: this is a retrospective observational study describing a series of cases treated with the "Inside-out" technique for medial epicondyle fracture fixation from 20

Ultrasound-guided Percutaneous Ulnar Nerve Release at the Elbow: A Stepwise Approach.

Cubital tunnel syndrome is the second most common compressive neuropathy in the upper limb. A wide range of surgical techniques have been described, from simple in situ decompression to anterior transposition, both through open and endoscopic approaches. Recent advances in ultrasonography and the development of dedicated instrumentation have enabled the emergence of ultrasound-guided cubital tunnel release as a feasible, effective, minimally invasive alternative. However, concerns regarding potential complications and inconsistent outcomes have limited widespread adoption. The aim of this stud

Efficacy and Safety of Ultrasound-Guided Carpal Tunnel Release: A Systematic Review and Meta-Analysis.

BACKGROUND: Carpal tunnel syndrome is the most common upper extremity compression neuropathy, often requiring surgical intervention. Although open carpal tunnel release is effective, it has been associated with greater scarring and morbidity. Ultrasound-guided carpal tunnel release (USCTR) has emerged as a minimally invasive alternative. This systematic review and meta-analysis aimed to evaluate the efficacy and safety of USCTR compared with open techniques. METHODS: PubMed, Embase, ScienceDirect, and the CENTRAL databases were searched for studies describing USCTR with complete transverse ca

A rare case of acute postoperative upper extremity deep vein thrombosis and pulmonary embolus after humeral shaft fixation: A case report and review of the literature.

RATIONALE: Upper extremity deep vein thrombosis (DVT) accounts for only 1% to 6% of all DVT cases but is clinically significant due to risks of life-threatening pulmonary embolism and post-thrombotic syndrome. Despite its low incidence, upper extremity deep vein thrombosis warrants heightened clinical vigilance given its frequently occult presentation and potentially devastating sequelae, as underestimation often delays diagnosis and management. PATIENT CONCERNS: A 69-year-old woman developed acute-onset chest pain, tightness, and dyspnea on postoperative day 11 following open reduction and i

Patient-Directed Care After Carpal Tunnel Release Using Video Integration and Digital Education After Operations (VIDEO Trial): A Randomized Controlled Trial.

BACKGROUND: Carpal tunnel release (CTR) is a successful procedure with consistent outcomes. Routine in-person postoperative visits are of questionable value. It remains to be seen whether video education and patient-directed care after CTR can replace in-person postoperative visits. METHODS: Eighty-three patients undergoing CTR were analyzed in a prospective, randomized, controlled trial comparing in-person follow-up to an experimental program with Video Integration and Digital Education after Operations (VIDEO). Patients underwent open, wide-awake, local-anesthetic, no-tourniquet CTR in-offi

Treatment for ulnar neuropathy at the elbow.

BACKGROUND: Ulnar neuropathy at the elbow (UNE) is the second most common entrapment neuropathy after carpal tunnel syndrome. Treatment may be conservative or surgical, but optimal management remains controversial. This is an update of a review first published in 2011 and previously updated in 2012 and 2016. OBJECTIVES: To determine the effectiveness and safety of conservative and surgical treatment for ulnar neuropathy at the elbow (UNE). We intended to test whether: - surgical treatment is effective in reducing symptoms and signs and in increasing nerve function; - conservative treatment is

Endoscopic and Open Carpal Tunnel Release in Patients With Type II Diabetes Mellitus: Influence of Preoperative Semaglutide Use on Postoperative Outcomes.

PURPOSE: Patients with type II diabetes mellitus are at greater risk of carpal tunnel syndrome and of complications following carpal tunnel release (CTR). Although recent orthopedic literature suggests preoperative semaglutide use may reduce complication and reoperation rates, its impact has not been studied in hand surgery. Given the overall favorable complication profile of CTR, it remains unknown if similar risk modification exists. METHODS: Type II diabetes mellitus patients undergoing endoscopic carpal tunnel release (ECTR) and open carpal tunnel release (OCTR) were identified from the P

Disseminated Herpes Zoster Virus: A Severe Case Complicated by Radicular Mononeuropathy in a Clinically Immunocompetent Individual.

Herpes zoster incidence and severity increase with age due to immunosenescence. While typically presenting as a unilateral dermatomal rash, severe forms such as disseminated zoster and segmental zoster paresis can occur, especially in high-risk individuals. Despite the availability of a highly effective recombinant zoster vaccine, vaccination rates remain low in the United States. This case report is significant due to the presentation of rare complications, disseminated herpes zoster and segmental zoster paresis, in an elderly patient with chronic kidney disease (CKD), underscoring the clinic

The necessity of ulnar nerve exploration and translocation in open reduction of medial humeral epicondyle fractures in children.

AIMS: Medial humeral epicondyle fractures (MHEFs) are common elbow fractures in children. Open reduction should be performed in patients with MHEF who have entrapped intra-articular fragments as well as displacement. However, following open reduction, transposition of the ulnar nerve is disputed. The aim of this study is to evaluate the need for ulnar nerve exploration and transposition. METHODS: This was a retrospective cohort study. The clinical data of patients who underwent surgical treatment of MHEF in our hospital from January 2015 to January 2022 were collected. The patients were alloc

Corrective Osteotomy and Ligamentous Reconstruction in a Neglected Elbow Dislocation with Radial Neck Malunion: A Case Report.

CASE: Neglected elbow dislocation and radial neck malunion frequently result in chronic pain, instability, and early arthrosis. These complications are best prevented by early treatment with open reduction, corrective osteotomy, and ligament reconstruction, followed by early supervised physiotherapy. We present a peculiar case with neglected complex elbow dislocation and radial head malunion. In this case, we performed an open reduction of the elbow joint and radial neck corrective osteotomy, medial collateral ligament, annular ligament reconstruction, and lateral collateral ligament repair.
