Medial Epicondyle Fractures Treated with Diverse Fixation Techniques: A Case Series.

INTRODUCTION: Medial epicondyle fractures of the humerus, account for ~12-20% of pediatric elbow fractures and are often associated with elbow dislocations. Management is debated, especially for moderately displaced injuries. Conventionally, displaced fractures were sometimes treated non-operatively, yielding acceptable function but high rates of fibrous non-union. Recent evidence shows surgical fixation provides ~9-fold higher union rates without significant differences in long-term pain or nerve function. We present five patients treated with different fixation techniques to highlight manage

Safety and Efficacy of rhBMP-2 for Treating Acute Traumatic Fractures of the Upper and Lower Extremities: A Multicenter Prospective Study.

Background: Delayed or non-union fractures comprise 5-10% of cases, indicating the need for biologic interventions. Recombinant human bone morphogenetic protein-2 (rhBMP-2) is a potent osteoinductive agent; yet, collagen carrier-based uncontrolled release causes adverse events. We evaluated the safety and efficacy of a hydroxyapatite (HA) carrier-based rhBMP-2 delivery system for acute traumatic upper and lower fractures exhibiting bone defects. Methods: This prospective, multicenter, single-arm clinical trial enrolled 90 patients who underwent surgery using a hydroxyapatite (HA) carrier-based

Quality of surgical technique in patients with a humeral shaft fracture in the HUMMER study (HUMMER-Survey): an international survey among expert surgeons.

BACKGROUND: It is undetermined whether the quality of surgical technique in a humeral shaft fracture (reduction and fixation) affects outcomes. The aim of this study was to score the quality of surgical technique performed in patients with humeral shaft fractures and to assess whether the quality of surgical technique was associated with complications and (functional) outcomes. METHOD: A panel of expert orthopedic trauma surgeons, experienced in upper extremity surgery, assessed the quality of surgical technique (good, sufficient, or poor) in patients who participated in the HUMMER study on p

An Analysis of the Results of Intramedullary K-wire Fixation versus Plating in Fractures of the Metacarpal Shaft.

BACKGROUND: Fractures of the metacarpal bones are common injuries of the hand that can be managed surgically with K-wire, intramedullary screws or plates. The study aimed to compare the radiological and functional outcomes of metacarpal shaft fractures treated with K-wire and plate fixation. METHODOLOGY: A prospective study comprising 100 patients with metacarpal shaft fractures was recruited in a nonrandomized manner and divided into two groups ( n = 50 per group) and followed up for 9 and 14 months. Participants in one group were fixed using K-wire, while fractures in the other group were f

Superior versus anteroinferior plating for displaced midshaft clavicle fractures: a systematic review and meta-analysis of union, function, and complications.

BACKGROUND: Fixation of clavicular shaft fractures carries risks of nonunion, infection, and functional loss. Although superior and anteroinferior plating have widely been studied, the optimal approach remains debated. Previous literature has relied on indirect comparisons and been limited by early study cutoffs, high heterogeneity, and omission of key outcomes. This meta-analysis directly compared union, function, and complications between superior and anteroinferior plating. METHODS: This review followed the 2020 Preferred Reporting Items for Systematic Reviews and Meta-analyses guidelines.

Identifying surgeon-controlled factors associated with acromial fractures after reverse total shoulder arthroplasty: a case-control approach for individual surgeons.

PURPOSE: Patient characteristics are the major determinants of acromial stress fracture risk after reverse total shoulder arthroplasty; the contribution of surgeon-controlled arthroplasty geometry is less certain. This study presents a method by which an individual surgeon can analyze their own fracture cases by comparing the geometry of each case with matched non-fracture controls from the same practice. MATERIALS AND METHODS: Four cases of acromial stress fracture after primary reverse total shoulder arthroplasty by an individual surgeon were analyzed. Two were Levy Type IIB fractures that

Total elbow arthroplasty for unreconstructible distal humerus fracture sequelae in patients less than 40 year old. Long-term outcomes.

BACKGROUND: Total elbow arthroplasty (TEA) is rarely recommended in young patients because of a high risk of early failure. This study analyzes the long-term outcomes of a cohort of post-traumatic TEA patients under 40 years of age who received a linked semiconstrained implant to manage the sequelae of an unreconstructible distal humerus fracture. MATERIAL AND METHODS: A review of the patients who received a Coonrad-Morrey prosthesis between 1993 and 2013 to manage the sequelae of a distal humerus fracture identified 16 individuals with a minimum follow-up of 10 years who at the time of surge

L5 Lumbar Laminectomy in Aged C57BL/6J Mice to Model Postoperative Cognitive Dysfunction.

Postoperative cognitive dysfunction (POCD) is a common neurological complication following surgery and anesthesia, particularly in older adults, and is associated with delayed recovery, prolonged hospitalization, and reduced quality of life. Orthopedic and spinal procedures are linked to a relatively high risk of POCD; however, preclinical models that closely reflect the perioperative features of spinal surgery remain limited. Commonly used models, including laparotomy and tibial fracture, may produce persistent locomotor impairment and prolonged peripheral inflammation that can confound the i

Painful and weak shoulder following a generalized seizure: a missed odontoid fracture causing central cord compression-a case report.

BACKGROUND: Spinal fractures following seizures are reported but can be easily missed, as symptoms may be misinterpreted. Most seizure-induced spinal fractures occur in the thoracic and lumbar spine. Of all cervical spinal fractures, only 10% are odontoid fractures, making odontoid fractures following generalized convulsive seizures even rarer. Most odontoid fractures are unrecognized at the time of injury, as almost none are associated with neurological deficit at presentation. However, with time, it may result in incomplete or complete cervical cord compression owing to spinal instability, r

Treatment Outcomes for Distal Radius Fractures: A Meta-Analysis.

BACKGROUND: Distal radius fractures (DRFs) are among the most prevalent in upper-extremity injuries, particularly in older adults and individuals with osteoporosis. The common treatment options include surgical interventions such as volar locking plate (VLP) fixation and percutaneous pinning, as well as conservative strategies like splinting and bracing. However, the comparative effectiveness of these modalities remains unclear. This study aims to compare the functional outcomes and complication rates associated with VLP, percutaneous pinning, bracing, and splinting in the treatment of DRFs.

Percutaneous double-endobutton fixation for selected acute rockwood type III acromioclavicular joint disruptions: early and mid-term outcomes of a prospective case series.

BACKGROUND: The optimal management of acute Rockwood type III acromioclavicular joint (ACJ) disruptions remains controversial. Although conservative treatment is recommended for many patients, surgical stabilization may benefit selected high-demand individuals. This study evaluated the radiographic and functional outcomes of percutaneous double-endobutton fixation in selected patients with acute Rockwood type III ACJ disruptions. METHODS: A prospective observational case series was conducted involving 16 consecutive patients with acute Rockwood type III ACJ disruptions treated using percutane

Surgical treatment of coronal shear fractures: short- to mid-term results and risk factor analysis.

BACKGROUND: Coronal shear fractures of the distal humerus are rare but severe injuries. Reconstruction is often challenging, especially in comminuted cases, which is why many surgeons opt for an elbow arthroplasty in those cases. However, total elbow arthroplasty is associated with a variety of potential problems itself. Therefore, the aim of this study was to present the functional and clinical outcome of coronal shear fractures treated by osteosynthetic reconstruction in a short- to mid-term follow-up, and to identify possible risk factors for an inferior outcome. METHODS: We performed a re

Superior compared with anteroinferior plating for mid-shaft fractures of the clavicle : a meta-analysis.

AIMS: Superior plating has long been the routine surgical approach for mid-shaft fractures of the clavicle. However, concerns about the prominence of the plate and neurological risk have led to an increased interest in anteroinferior plating. Whether anteroinferior plating offers comparable or improved outcomes without compromising the stability of fixation has been investigated in several studies. The aim of this meta-analysis was to evaluate the clinical outcomes, complications, and complication-related outcomes following superior compared with anteroinferior plating of mid-shaft fractures o

Fibular allograft augmentation shows potential benefits in treating medial column comminuted proximal humeral fractures with osteoporosis: a secondary analysis of a randomized controlled trial.

BACKGROUND: Fibular allograft (FA) augmentation is proposed to address the lack of medial column support in plate-fixed proximal humeral fractures; however, the role of FA augmentation in these fractures remains inadequately elucidated. Our previous clinical trial did not show additional benefit in inserting an FA when treating adults with medial column comminuted proximal humeral fractures. Nonetheless, it is possible that some subgroups may still derive benefits from FA. This study aimed to investigate whether there were any subgroups benefit from FA. MATERIALS AND METHODS: Our randomized c
