Improved Ulnar Plate Positioning Using the WHAM (Wrist Henry Approach Modification) for Volar Rim Fixation of Distal Radius Fractures.

INTRODUCTION: Fixation of small, very distal volar rim fragments of the distal radius is technically demanding because the classic Henry approach may provide limited ulnar exposure. The Wrist Henry Approach Modification (WHAM) combines a limited volar incision with a dual-window exposure. HYPOTHESIS: The WHAM approach would enable more ulnar positioning of volar rim-specific plates than the classic Henry approach without increasing approach-related complications. MATERIALS AND METHODS: This retrospective single-center comparative study included 39 patients with distal radius fractures treate

Associations Between Area Deprivation Index and the Time to Presentation of Scaphoid Fractures.

PURPOSE: Scaphoid fractures are the most common carpal fractures and are prone to nonunion because of their retrograde blood supply. Delayed diagnosis or treatment increases the risk of nonunion and progression to scaphoid nonunion advanced collapse. Social determinants of health, measured by the area deprivation index (ADI), may contribute to such delays. This study examines the association between ADI, time to presentation, and the presence of nonunion at initial evaluation. METHODS: A retrospective chart review identified 168 patients with suspected scaphoid fractures between January 1, 20

Gunshot Wounds and Orthopaedic Injury at a Pediatric Level I Trauma Center.

BACKGROUND: Gunshot wounds (GSWs) and firearm-related injuries are a leading cause of pediatric unintentional injury and death. Orthopaedic firearm injuries are also associated with higher than anticipated mortality, disproportionately affecting socioeconomically and racially disadvantaged patients. The purpose of this study was to characterize GSW injuries at our institution and to identify associated risk factors for these injuries in affected populations. METHODS: A retrospective review was performed of patients aged 0-17 years who were treated for a GSW from April 2012 to October 2023 at

Specific fixation beyond the watershed line for distal radius fractures: A comparative study.

PURPOSE: Various fixation methods are available for intra-articular distal radius fractures, depending on column involvement. Some rely solely on ulnar hook plates, while others use specific "extra distal" plates that address both columns. To date, no comparative studies have evaluated these techniques. This study compared the fixation by ulnar hook plate combined with one or two percutaneous screws (Group 1) in the radial styloid with extra distal plates fixing both columns (Group 2). Functional outcomes were hypothesized to be similar, whereas complication rates were expected to differ. MET

The influence of plate removal on functional and patient reported outcomes after scaphoid nonunion surgery.

INTRODUCTION: Volar locking plate fixation with cancellous bone grafting is an effective treatment for scaphoid non-union. However, the plate can impinge on the volar rim of the radius, causing pain or restricted movement that often necessitates plate removal. The primary aim of this study was to evaluate the effect of hardware removal on wrist function and patient-reported outcomes after plate fixation for scaphoid non-union. METHODS: This prospective study assessed 49 of 113 patients who underwent plate fixation for scaphoid non-union and later required plate removal due to functional impai

Arthroscopic arthrolysis with hardware removal versus hardware removal alone after intra-articular distal radius fracture: A prospective comparative study.

INTRODUCTION: Post-traumatic wrist stiffness after intra-articular distal radius fractures rarely improves with hardware removal alone once arthrofibrosis is established. Arthroscopic arthrolysis allows minimally invasive release of intra-articular adhesions. HYPOTHESIS: Arthroscopic arthrolysis combined with hardware removal improves wrist motion and function compared with hardware removal alone. MATERIALS AND METHODS: This single-center prospective study compared arthroscopic arthrolysis with hardware removal (combined) versus hardware removal alone (isolated) in patients with post-traumat

Simple Excision of Proximal Scaphoid Nonunion: A Viable Option When the Scapholunate Ligament Is Intact.

PURPOSE: Because of the biological, anatomical, and biomechanical characteristics of the scaphoid, treating some proximal pole nonunions remains challenging. The location of the nonunion (proximal or distal to the dorsal apex) may influence its natural history. Based on this concept, patients with a proximal pole nonunion located proximal to the dorsal apex and maintaining an intact insertion of the dorsal scapholunate ligament in the distal fragment of the scaphoid may benefit from simple excision of the proximal pole. This study evaluated the clinical and functional outcomes of this treatmen

Treatment Outcomes for Distal Radius Fractures: A Meta-Analysis.

BACKGROUND: Distal radius fractures (DRFs) are among the most prevalent in upper-extremity injuries, particularly in older adults and individuals with osteoporosis. The common treatment options include surgical interventions such as volar locking plate (VLP) fixation and percutaneous pinning, as well as conservative strategies like splinting and bracing. However, the comparative effectiveness of these modalities remains unclear. This study aims to compare the functional outcomes and complication rates associated with VLP, percutaneous pinning, bracing, and splinting in the treatment of DRFs.
